Sarcopenia and metabolic syndrome (MetS) are associated with the prognosis from malignant tumors. However, evidence of the relationship between sarcopenia and MetS among gastric cancer (GC) patients following radical gastrectomy is lacking. This study assessed the association between preoperative sarcopenia and MetS among GC patients and analyzed the prognosis of patients with different malnutrition statuses.
We prospectively assessed the preoperative statuses of sarcopenia and MetS among patients who underwent radical gastrectomy from July 2014 to December 2017. We combined sarcopenia and MetS to generate four groups MetS-related sarcopenia group (MSS), sarcopenia group (S), MetS group (MS), and normal group (N).
A total of 749 patients with resectable GC were included in this study. Preoperative MetS was associated with sarcopenia (p<0.001). Multivariate logistic regression presented that MetS-related sarcopenia (OR=2.445; p=0.010) and sarcopenia alone (OR=2.117; p=0.001) were independent predictoria and MetS require more medical interventions.
Data regarding regionally metastatic cutaneous squamous cell carcinoma of the head and neck (cSCCHN) is limited and derived almost exclusively from Australian and United States (US) institutions. We report the first United Kingdom perspective, with the aims of benchmarking survival outcomes and identifying clinically relevant prognosticators.
Ninety-one patients with regionally recurrent cSCCHN treated with curative intent over a ten-year period (2009-2018) were studied retrospectively. Time-to-event analyses were used to estimate oncological outcomes, and log-rank statistics and Cox proportional hazards models used to examine potential prognosticators. Receiver operating characteristics were also used to analyse the influence of nodal disease burden.
Parotid involvement (with or without neck involvement) was most common (79.2%), and time to recurrence in those with parotid disease alone significantly shorter than for any other disease distribution (p=0.034). The circumferential resection margin (CRM) is a primary predictor of local recurrence and survival in rectal cancer, and an important consideration in guiding treatment. CRM is usually predicted preoperatively, so optimal management of an unexpected pathologic positive CRM involvement is debatable. We aimed to investigate the postoperative management of T3N0 rectal cancers with a positive pathologic CRM, and the impact of each strategy on survival.
The NCDB was reviewed for pathological T3N0 rectal cancer cases from 2010 to 2015, that received neoadjuvant chemotherapy, had surgical resection with pathological clear margins, but a positive pathologic CRM(disease≤2mm from radial margin). The main outcomes were the incidence, treatment modalities used, and impact of each modality on survival. Univariate analysis evaluated the demographic and provider characteristics across treatment groups. Kaplan-Meier and Cox regression analysis assessed survival and factors associated with overall survival (OS).

A reduced-order state-space model based on the moving boundary approach is proposed for the canonical cycle, which eases the controller design. The optimal cycle (that satisfying the cooling demand while maximizing efficiency) is defined by three variables, but only two inputs are available, therefore the controllability of the proposed model is studied. It is shown through optimization simulations how optimal cycles for a range of the cooling demand turn out not to be achieved by keeping the degree of superheating to a minimum. The Practical NMPC and a well-known feedback-plus-feedforward strategy from the literature are compared in simulation, both showing trouble in reaching the optimal cycle, which agrees with the controllability study.The information on data packet loss occurrences in a networked control system (NCS) is useful for improving the control performance and detecting communication failures or cyber-attacks. This study considers the simultaneous estimation of the plant state and packet loss occurrences at each time step. After formulation of the problem, two solutions are proposed. In the first one, an input-output representation of the NCS model is used to design a recursive filter for estimation of the packet loss occurrences. This estimation is then used for state estimation through Kalman filtering. In the second solution, a state-space model of NCS is used to design an estimator for both the plant state and the packet loss occurrences, which employs a Kalman filter. The effectiveness of the proposed solutions is shown during an example, and comparisons are made with another solution based on the interacting multiple model approach to estimation.The basolateral amygdala (BLA) is critical in the control of the sympathetic output during stress. Studies demonstrated the involvement of the renin-angiotensin system components in the BLA. Angiotensin-(1-7) [Ang-(1-7)], acting through Mas receptors, reduces stress effects. Considering that angiotensin-converting enzyme 2 (ACE2) is the principal enzyme for the production of Ang-(1-7), here we evaluate the cardiovascular reactivity to acute stress after administration of the ACE2 activator, diminazene aceturate (DIZE) into the BLA. this website We also tested whether systemic treatment with DIZE could modify synaptic activity in the BLA and its effect directly on the expression of the N-methyl-d-aspartate receptors (NMDARs) in NG108 neurons in-vitro. Administration of DIZE into the BLA (200 pmol/100 nL) attenuated the tachycardia to stress (ΔHR, bpm vehicle = 103 ± 17 vs DIZE = 49 ± 7 p = 0.018); this effect was inhibited by Ang-(1-7) antagonist, A-779 (ΔHR, bpm DIZE = 49 ± 7 vs A-779 + DIZE = 100 ± 15 p = 0.04). Systemic treatment with DIZE attenuated the excitatory synaptic activity in the BLA (Frequency (Hz) vehicle = 2.9 ± 0.4 vs. DIZE =1.8 ± 0.3 p less then 0.04). NG108 cells treated with DIZE demonstrated decreased expression of l subunit NMDAR-NR1 (NR1 expression (a.u) control = 0.534 ± 0.0593 vs. DIZE = 0.254 ± 0.0260) of NMDAR and increases of Mas receptors expression. These data demonstrate that DIZE attenuates the tachycardia evoked by acute stress. This effect results from a central action in the BLA involving activation of Mas receptors. The ACE2 activation via DIZE treatment attenuated the frequency of excitatory synaptic activity in the basolateral amygdala and this effect can be related with the decreases of the NMDAR-NR1 receptor expression.
