Normothermic machine perfusion (NMP) has been used to recondition marginal organs; however, mechanisms by which NMP might benefit organs are not well understood. Using pairs of human kidneys obtained from the same donor, we compared the effect of NMP with that of cold storage on the global kidney transcriptome. We found that cold storage led to a global reduction in gene expression, including inflammatory pathway genes and those required for energy generation processes, such as oxidative phosphorylation (OXPHOS). In contrast, during NMP, there was marked upregulation OXPHOS genes, but also of a number of immune and inflammatory pathway genes. Using biopsies from kidneys undergoing NMP that were subsequently transplanted, we found that higher inflammatory gene expression occurred in organs with prolonged delayed graft function (DGF). Therefore, we used a hemoadsorber (HA) to remove pro-inflammatory cytokines. This attenuated inflammatory gene expression increased OXPHOS pathway genes and had potentially clinically important effects in reducing the expression of a DGF-associated gene signature. Together, our data suggest that adsorption of pro-inflammatory mediators from the perfusate represents a potential intervention which may improve organ viability.

Complex regional pain syndrome (CRPS), formerly known as reflex sympathetic dystrophy (RSD), is a difficult to treat condition characterized by debilitating pain and limitations in functional ability. Neuromodulation, in the form of spinal cord stimulation (SCS) and peripheral nerve stimulation (PNS), have been traditionally used as a treatment for CRPS with variable success.

This chart review describes the use of implantable PNS systems in the treatment of CRPS of the upper and lower extremities spanning nearly three decades.

A retrospective chart review was performed on 240 patients with PNS implanted between 1990 and 2017 at our institution. Of these, 165 patients were identified who had PNS systems implanted for a diagnosis of CRPS. Patient profile, including baseline characteristics, comorbidities, past/current interventions/medications and targeted nerves, was descriptively summarized through standard summary statistics. Patients' pain scores and opioid consumptions at baseline (preimplant), 1 mon (81%) continue to use their PNS.

To examine the effect of filler contentTo examine the effect of filler content on structural coloration of experimental flowable resin composites.

Experimental composites containing spherical fillers and different filler contents (30, 40, 50, 60 and 70 wt%), a structural colored resin composite (Omnichroma), and a nanohybrid flowable resin composite (Gracefil LoFlo) were used. The optical characteristics of the specimens were measured on standard black and white backgrounds using a spectrophotometer under CIE D65 illuminant and the L*a*b* values were determined. The color matching abilities to artificial teeth were evaluated using the ΔE*ab and ΔE00 color difference formulas. One-way analysis of variance and Tukey's post hoc test were used. The spectral reflectance of experimental composites showed different trends in different backgrounds, plate thickness, and filler contents. In particular, higher spectral reflectance was observed on the white background at wavelengths ranging from 450 to 700 nm in both 1.5 and 3.0 mm plates when compared to the black background. The percent reflectivity tended to decrease with a decrease of the filler contents. The color matching abilities of the experimental composites improved with the increase in the filler contents.
