Researchers have been advised to report the point estimate of either sensitivity or specificity and its 95% credible interval (CrI) for a fixed specificity or sensitivity value in the summary of findings (SoF) table for diagnostic test accuracy (DTA) when they use the hierarchical summary receiver operating characteristic (HSROC) model. However, there is no other tool that easily calculates the statistics.
We developed the calculator for the summary points from the HSROC model (CAST-HSROC), a web application for calculating the statistics easily. The existing graphical user interface software such as Review Manager and MetaDTA cannot calculate the statistics. Users should check whether convergence is reached before interpreting the results.

LAA perforation is a well-known complication of the WATCHMAN device placement. Here we present two cases of NVAF where oral anticoagulation was not advisable due to recurrent bleeding episodes. They underwent the WATCHMAN procedure for stroke prevention. During the placement of the WATCHMAN device into the left atrial appendage (LAA) in both cases, pericardial staining was noted that worsened over the next few minutes. It was decided to deploy the 27 mm WATCHMAN device into the LAA. In one case, satisfactory hemostasis was achieved with the device deployment eliminating the need for cardiothoracic surgery. However, the second case led to pericardial tamponade and was managed by the placement of a pericardial window. Rapid identification followed by aggressive management of the underlying etiology with a multidisciplinary team is critical to prevent long-term organ dysfunction, disability from amputation, and death. While bleeding is a common finding in DIC, anticoagulation must be considered if PF is present. We report a case of Escherichia coli--associated emphysematous pyelitis leading to bacteremia, septic shock, and PF with small- and medium-sized vessel thrombosis and acral ischemia.
