As a novel oral agent in treating anemia of chronic kidney disease (CKD), several clinical trials of vadadustat have been conducted to compare with darbepoetin alfa. This study systematically reviews and investigates the efficacy and safety of vadadustat in the anemia treatment with different duration in both nondialysis-dependent CKD (NDD-CKD) and dialysis-dependent CKD (DD-CKD).

Several main databases were searched for randomized controlled trials (RCTs) reporting vadadustat vs darbepoetin alfa for anemia patients with CKD. The outcome indicators were focused on hemoglobin (Hb), the percentage of patients within the target Hb, the need for RBC (Red Blood Cell) transfusions, and serious adverse events (SAEs).

Four eligible studies with 8,026 participants were included. The changes of Hb levels from the baseline in the darbepoetin alfa group were significantly higher than that in the vadadustat group with DD-CKD (mean difference (MD) -0.19, [95% confidence interval (CI), -0.21 to -0.17], p < 0.0001).d CKD, especially in NDD-CKD patients. As the application of vadadustat is still under exploration, future research should compensatefor the limitations of our study to estimate the vadadustat's value.

Compared to darbepoetin alfa, vadadustat had the same effect in raising the hemoglobin level in NDD-CKD patients in the short term. Vadadustat may become an effective and safe alternative for the treatment of patients with anemia and CKD, especially in NDD-CKD patients. As the application of vadadustat is still under exploration, future research should compensate for the limitations of our study to estimate the vadadustat's value.

Acute kidney injury (AKI) is a severe complication of sepsis, and is strongly correlated with MicroRNAs (miRNAs). However, the mechanism of miR-22 on sepsis-induced AKI is not clearly understood. The study aimed to explore the role and mechanism of miR-22 on AKI.

The AKI models were established by cecal ligation and puncture (CLP) surgery in SD rats and lipopolysaccharide (LPS) induction in HBZY-1 cells. In AKI rats, the content of serum creatinine (SCr) and blood urea nitrogen (BUN) were detected. Kidney tissues were pathologically examined by H&E and PAS staining. The LPS-induced HBZY-1 cells were transfected with mimics miR-22, si-HMGB1, or oe-HMGB1. miR-22 and HMGB1 expression was detected in vivo and in vitro. In transfected cells, HMGB1/TLR4/NF-κB pathway-related protein expressions were measured by Western blot. The relationship between miR-22 and HMGB1 was assessed by a dual-luciferase gene report. Inflammatory cytokine levels in serum and cells were assessed by ELISA.

Diabetic nephropathy (DN) is a severe microvascular complication of diabetes mellitus and a primary reason for end-stage renal disease (ESRD). Diabetes mellitus is a chronic metabolic disorder commonly encountered in orthopedic patients. Both type 1 and type 2 diabetes mellitus increase fracture risk and impair fracture healing. This review examines complex etiology of impaired fracture healing in diabetes.

Recent findings point to several mechanisms leading to orthopedic complications in diabetes. Hyperglycemia and chronic inflammation lead to increased formation of advanced glycation end products and generation of reactive oxygen species, which in turn contribute to the disruption in osteoblast and osteoclast balance leading to decreased bone formation and heightening the risk of nonunion or delayed union as well as impaired fracture healing. The mechanisms attributing to this imbalance is secondary to an increase in pro-inflammatory mediators leading to premature resorption of callus cartilage and impaired bone formation due to compromised osteoblast differentiation and their apoptosis. Among these people 1,352 respondents indicated that they gambled in casinos at least once in the past 12 months. Over 90% (1,228) of the respondents reported that they gambled in Macao's casinos and slot lounges. The three most popular forms of casino gambling were baccarat, Sic Bo, and slot machines. The monthly median expenditure on gambling was HKD 1,845 with a range from HKD 2 to HKD 375,175. The prevalence rates for problem gambling and pathological gambling were 15.1% and 7.1%, respectively. However, the cognitive profile of AP and its differences and similarities with that of Parkinson's Disease (PD) are still a matter of debate. The present meta-analysis aimed at identifying patterns of cognitive impairment in AP by comparing global cognitive functioning, memory, executive functions, visuospatial abilities, language, non-verbal reasoning, and processing speed test performances of patients with AP relative to healthy controls and patients with PD. All investigated cognitive domains showed a substantial impairment in patients with AP compared to healthy controls. When AP syndromes were considered separately, their cognitive functioning was distributed along a continuum from Multiple Systemic Atrophy at one extreme, with the least impaired cognitive profile (similar to that observed in PD) to Progressive Supranuclear Palsy, with the greatest decline in global cognitive and executive functioning (similar to Corticobasal Syndrome). These findings indicate that widespread cognitive impairment could represent an important clinical indicator to distinguish AP from other movement disorders.In the present study, via a 180-day field trial, the indicators of soil total cadmium, DTPA-Cd, organic matter, and plant cadmium extraction were tested after the application of chelate tetrasodium glutamate diacetate (GLDA) to investigate the potential of GLDA combined with Tagetes patula L. to remediate cadmium-contaminated soil. To do so, five GLDA treatments (e.g., 0, 292.5, 585, 1170, and 2340 kg hm-2) were practiced. For each treatment, the total GLDA was divided into two applications with 15-day intervals (0.25, 0.47, and 0.61 mg·kg-1) under T. patula plantation. Compared with the control, our results showed that GLDA application significantly increased the biomass of aerial parts of T. patula by 21.9% (p  less then  0.05). Likewise, Cd content in aboveground and underground parts of T. patula increased by 94.7% and 60.5%, respectively, compared with the control (p  less then  0.05). GLDA application caused significant increases in Cd accumulations in cell soluble fraction and cell wall by 290% and 123%, respectively (p  less then  0.05); soil pH and DTPA-Cd content increased with the increase of total application of GLDA. Co-application of GLDA (2340 kg hm-2) and T. patula reduced the total soil Cd content by 12.87% compared with the soil background. Altogether, our findings conclude on the efficacy of GLDA application for the remediation of Cd-contaminated farmlands under T. patula cultivation.Environmental media discourse analysis allows researchers to explore thematic patterns in media coverage of environmental issues through qualitative observations and quantitative coding. We analyzed 930 articles from 2007 through 2019 to determine common patterns in how these issues are covered by Florida media, including patterns in the causes of environmental problems, potential solutions, and stakeholders mentioned. Findings suggest that water quality problems are often framed as unpredictable or episodic rather than chronic, and as requiring (and only potentially being remediated by) state-level centralized technological or infrastructural solutions.
