Hidradenitis suppurativa is a chronic inflammatory skin disorder associated with inflammatory bowel disease. However, it can arise as a paradoxical side effect of anti-TNF treatment.
The article reports on three patients with Crohn's disease who developed hidradenitis suppurativa during the treatment with adalimumab.
Case 1 A 38-year-old female exhibited an infiltrative lesion in the inguinal region and vulva, consistent with hidradenitis suppurativa, after three months of adalimumab. These lesions were treated with partial vulvectomy. Case 2 After adalimumab treatment, a 27-year-old female, originally diagnosed with ileocolonic Crohn's disease, went into clinical and endoscopic remission. The patient eventually presented two hyperchromic nodules in the inguinal region, which were diagnosed as hidradenitis suppurativa. The patient showed improvement after treatment with oral doxycycline and local therapy. Case 3 A 34-year-old female with fistulizing and stenosing ileocolonic Crohn's disease, started ada reaction to anti-TNF therapy. Clinical teams must be aware of this type of complication. Early diagnosis and treatment are essential for controlling the disease and preventing the onset of complications.
Keloids represent chronic fibroproliferative skin disorders in which there is deposition of extracellular components, especially type 1 collagen, fibronectin and elastin, in excessive amounts. NEDD4 is associated with fibrosis found in abnormal wound healing through increased fibroblast proliferation and regulation of type 1 collagen expression. The exact etiology of keloid formation is undefined, but the role of genetic factors was demonstrated.
To investigate the polymorphism of the NEDD4 gene rs8032158 in a sample of Egyptian patients who have keloids.
The current case-control study was conducted in 160 unrelated subjects; 100 keloid patients and 60 ages and sex coincided with apparently healthy controls. All subjects underwent a complete history, and weight and length were measured to calculate body mass index (BMI). The Vancouver Scar Scale (VSS) was used to assess keloid severity. An analysis of the polymorphism of the NEDD4 gene rs8032158 T/C was performed using taqman allelic discrimination (real-time PCR).
The rs8032158 CC genotype was observed significantly in keloid patients and increased the risk of keloid development by approximately 2 times (p = 0.003, OR = 1.80). The C allele significantly increased the risk of keloid development by approximately 2 times (P = 0.002, OR = 2.08). The carriers of the CC genotype were significantly associated with severe keloid form and with the highest VSS values.

Patient-reported measures are of importance in chronic dermatological conditions where psychosocial consequences and quality of life impairment are common. The current study aimed to evaluate patient-reported disease severity and quality of life in Arabic patients with psoriasis.
Arabic psoriatic patients were invited to complete an online survey that collected patients' demographics, self-assessed Simplified Psoriasis Index (saSPI) and the Dermatology Life Quality Index (DLQI). Disease severity and quality of life were compared in relation to patients' demographics. Correlation between patient-reported measures was calculated using Spearman's rank correlation test.
A total of 221 patients with psoriasis, from 12 Arabic-speaking countries, participated in the study. The mean (95% CIs) saSPI-severity score was 10.9 (9.6-12.2). Female patients reported higher saSPI-psychosocial impact scores compared to males (P=0.04) while patients with longer disease duration reported higher saSPI-past history and interventions scores (P=0.0001). The mean (95% CIs) DLQI score was 11.2 (10.2-12.1). Patients with severe disease reported significantly higher DLQI scores [18.5 (10.5-2)] (P=0.0001). DLQI was strongly correlated with saSPI-psychosocial impact score (rho= 0.63).
Arabic psoriatic patients participating in this study showed mild to moderate disease severity which had a very large impact on patients' quality of life. Higher disease severity was associated with more impaired quality of life.

To make sure whether GLP-1 and insulin play different roles in human adipose-derived stem cells (hADSCs).
We examined the in vitro roles and molecular mechanisms of liraglutide, a GLP-1 analogue, and human insulin on hADSCs isolated from subcutaneous adipose tissue. Different concentrations (0, 0.1, 1, 10, 100nM) of liraglutide and insulin were added to proliferation and differentiation medium of hADSCs, respectively.
Liraglutide inhibits while insulin promotes the proliferation and differentiation at the concentration of 100nM. Moreover, the levels of GSK-3 increase during differentiation and liraglutide could down-regulate it when compared with insulin. We also find that the activation of phosphorylated GSK-3α and GSK-3β is involved in the differentiation roles.
