The automatic needle centroid localisation improves the usability of the algorithm and has the potential to decrease the fiducial localisation and target registration errors associated with the guided-US calibration method.Automatic recognition of instruments in laparoscopy videos poses many challenges that need to be addressed, like identifying multiple instruments appearing in various representations and in different lighting conditions, which in turn may be occluded by other instruments, tissue, blood, or smoke. Considering these challenges, it may be beneficial for recognition approaches that instrument frames are first detected in a sequence of video frames for further investigating only these frames. This pre-recognition step is also relevant for many other classification tasks in laparoscopy videos, such as action recognition or adverse event analysis. In this work, the authors address the task of binary classification to recognise video frames as either instrument or non-instrument images. They examine convolutional neural network models to learn the representation of instrument frames in videos and take a closer look at learned activation patterns. For this task, GoogLeNet together with batch normalisation is trained and validated using a publicly available dataset for instrument count classifications. They compared transfer learning with learning from scratch and evaluate on datasets from cholecystectomy and gynaecology. The evaluation shows that fine-tuning a pre-trained model on the instrument and non-instrument images is much faster and more stable in learning than training a model from scratch.Neurovascular surgery aims to repair diseased or damaged blood vessels in the brain or spine. There are numerous procedures that fall under this category, and in all of them, the direction of blood flow through these vessels is crucial information. Current methods to determine this information intraoperatively include static pre-operative images combined with augmented reality, Doppler ultrasound, and injectable fluorescent dyes. Each of these systems has inherent limitations. Cediranib clinical trial This study includes the proposal and preliminary validation of a technique to identify the direction of blood flow through vessels using only video segments of a few seconds acquired from routinely used surgical microscopes. The video is enhanced to reveal subtle colour fluctuations related to blood pulsation, and these rhythmic signals are further analysed in Fourier space to reveal the direction of blood flow. The proposed method was validated using a novel physical phantom and retrospective analysis of surgical videos and demonstrated high accuracy in identifying the direction of blood flow.Optical colonoscopy is known as a gold standard screening method in detecting and removing cancerous polyps. During this procedure, some polyps may be undetected due to their positions, not being covered by the camera or missed by the surgeon. In this Letter, the authors introduce a novel convolutional neural network (ConvNet) algorithm to map the internal colon surface to a 2D map (visibility map), which can be used to increase the awareness of clinicians about areas they might miss. This was achieved by leveraging a colonoscopy simulator to generate a dataset consisting of colonoscopy video frames and their corresponding colon centreline (CCL) points in 3D camera coordinates. A pair of video frames were used as input to a ConvNet, whereas the output was a point on the CCL and its direction vector. By knowing CCL for each frame and roughly modelling the colon as a cylinder, frames could be unrolled to build a visibility map. They validated their results using both simulated and real colonoscopy frames. Their results showed that using consecutive simulated frames to learn the CCL can be generalised to real colonoscopy video frames to generate a visibility map.Non-invasive reconstruction of electrophysiological activity in the heart is of great significance for clinical disease prevention and surgical treatment. The distribution of transmembrane potential (TMP) in three-dimensional myocardium can help us diagnose heart diseases such as myocardial ischemia and ectopic pacing. However, the problem of solving TMP is ill-posed, and appropriate constraints need to be added. The existing state-of-art method total variation minimisation only takes advantage of the local similarity in space, which has the problem of over-smoothing, and fails to take into account the relationship among frames in the dynamic TMP sequence. In this work, the authors introduce a novel regularisation method called graph-based total variation to make up for the above shortcomings. The graph structure takes the TMP value of a time sequence on each heart node as the criterion to establish the similarity relationship among the heart. Two sets of phantom experiments were set to verify the superiority of the proposed method over the traditional constraints infarct scar reconstruction and activation wavefront reconstruction. In addition, experiments with ten real premature ventricular contractions patient data were used to demonstrate the accuracy of the authors' method in clinical applications.Esophagogastroduodenoscopy (EGD) has been widely applied for gastrointestinal (GI) examinations. However, there is a lack of mature technology to evaluate the quality of the EGD inspection process. In this Letter, the authors design a multi-task anatomy detection convolutional neural network (MT-AD-CNN) to evaluate the EGD inspection quality by combining the detection task of the upper digestive tract with ten anatomical structures and the classification task of informative video frames. The authors' model is able to eliminate non-informative frames of the gastroscopic videos and detect the anatomies in real time. Specifically, a sub-branch is added to the detection network to classify NBI images, informative and non-informative images. By doing so, the detected box will be only displayed on the informative frames, which can reduce the false-positive rate. They can determine the video frames on which each anatomical location is effectively examined, so that they can analyse the diagnosis quality. Their method reaches the performance of 93.
