To facilitate effective personalized medicine, primary health care needs better methods of assessing and monitoring chronic obstructive pulmonary disease (COPD).

This cohort study aims to investigate how biomarkers relate to clinical characteristics and COPD patients' subjective needs over time.

Patients (n=750) in different COPD severity according to the GOLD criteria and age- and sex-matched controls (n=750) will be recruited over a period of 5 years from 15 primary health care centers in Region Stockholm, Sweden, and followed for 10 years in the first instance. Data on patients' subjective needs will be collected via telephone/email, data on clinical/physiological variables (eg, symptoms, exacerbations, comorbidities, medications, smoking habits, lung function) from existing databases that are based on medical records, and data on biomarkers via repeated blood sampling. Quantitative and qualitative methods will be used. Initial results are expected after 2 years (feasibility test), and a larger body of evidence after 5 years.

The study is expected to provide definitive and clinically useful scientific evidence about how biomarkers relate to clinical variables and patients' subjective needs. This new evidence will facilitate accurate, and personalized COPD management by the use of valid biomarkers. It will provide useful tools for primary care professionals and may facilitate optimal self-management.

This phenomenological study was aimed at exploring principal physicians' (participants') experience of attending to COPD patients and motivating their self-management, in light of the GOLD clinical guidelines of COPD therapy.

Interviews were conducted with nine physicians, who had referred patients to PR, five general practitioners (GPs) and four lung specialists (LSs). The interviews were recorded, transcribed, and analyzed through a process of deconstruction and reconstruction.

The participants experienced several ethical dilemmas in being principal physicians of COPD patients and motivating their self-management; primarily in the balancing act of adhering to the Hippocratic Oath of promoting health and saving lives, while respecting their patients' choice regarding non-adherence eg, by still smoking. It was also a challenge to deal with COPD as a nicotine addiction disease, deal with patients' denial regarding the harm of smoking and in motivating patient mastery of the disease. To evaluate the feasibility of a high-volume and high-intensity functional training programme in patients with multiple sclerosis (MS), and to explore whether functional capacity improves. A further objective was to explore changes in muscle strength and aerobic capacity.

This pilot study comprised a 12-week intervention, with an 8-week follow-up period. The intervention consisted of 3 weekly 3-h training sessions, comprising functional resistance-, endurance-, and skills training. Feasibility (questionnaire), functional capacity (Timed Up and Go Test, 10-Meter Walk Test, and 6-Minute Walk Test), aerobic capacity (cardiopulmonary exercise test) and muscle strength (1 repetition maximum (RM) leg press) were evaluated.

Seven patients completed the study. Patients attended a mean of 93% of the training sessions. One adverse event was reported, which was not related to the training programme. Patients scored positive or very positive on 86% of the feasibility aspects and scored an overall grade of 8.9 on a scale of 1-10 regarding satisfaction with the training programme.
