Avian eggshell pigmentation may provide information about a female's physiological condition, in particular her state of oxidative balance. Previously we found that female house wrens (Troglodytes aedon Vieillot, 1809) with lighter, less-maculated, and redder ground-colored shells were older and produced heavier offspring than females laying darker, browner eggs. The strong pro-oxidant protoporphyrin is responsible for this species' eggshell pigmentation, so differences in pigmentary coloration may be related to eggshell protoporphyrin content and reflect female oxidative balance and condition during egg-formation. Therefore, we tested the assumption that egg-surface coloration is related to the amount of protoporphyrin in the shell matrix. We analyzed digital photographs of eggs to determine maculation coverage as a measure of the overall ground coloration of the egg and its red-, green-, and blue-channel pixel values. Pigments were then extracted from these same eggs and analyzed using high-performance liquid chromatography. There was a strong, positive relationship between eggshell redness and protoporphyrin content of eggshells, but no relationship between percent maculation and protoporphyrin content. Thus, when older, larger females deposit more protoporphyrin in their eggshells, this may reflect a tolerance for high levels of circulating protoporphyrin or an effective mechanism for off-loading protoporphyrin into the eggshell matrix.The Covid-19 pandemic hit the transplant world in March 2020. Teams quickly organized themselves to optimize the management of their immunocompromised patients and to progress in the knowledge of this new disease. To do this, a French Registry was set up, listing all solid organ transplant patients who had developed a SARS Cov2 infection. Numerous studies carried out on the basis of these data have enabled us to describe the disease in transplant patients, to characterize its clinical and biological severity factors and to define its prognosis. The mortality of transplant patients hospitalized for Covid-19 is 23% at 60 days and renal insufficiency plays a major role in the poor prognosis in addition to the classic risk factors described in the general population. The advent of vaccination has been a great relief, but transplant patients have developed a poorer vaccine response than immunocompetent subjects, keeping them at risk of severe disease after an adapted vaccination schedule. Specific strategies had to be adopted in this particularly fragile population (increased number of vaccine doses, injection of monoclonal antibodies). The collaboration of the French transplantation centers under the impulse of the Société Francophone de Transplantation allowed us to carry out many collaborative projects, which were of great use for the care of the patients.The excellent results of Transplantation over the last decades have highlighted new challenges to be solved in the next years. (1) Modify the storage of harvested organs to improve their quality. (2) Modify strategies by taking into account the immunodeficiency in front of news infections like the one with Sars CoV-2. (3) Better understand the mechanisms of chronic rejection, in particular the role of innate immunity. (4) Rethink immunosuppressive strategies to prevent and to treat chronic rejections.Despite high initial response rates, acute myeloid leukemia (AML) treated with the BCL-2-selective inhibitor venetoclax (VEN) alone or in combinations commonly acquires resistance. We performed gene/protein expression, metabolomic and methylation analyses of isogenic AML cell lines sensitive or resistant to VEN, and identified the activation of RAS/MAPK pathway, leading to increased stability and higher levels of MCL-1 protein, as a major acquired mechanism of VEN resistance. MCL-1 sustained survival and maintained mitochondrial respiration in VEN-RE cells, which had impaired electron transport chain (ETC) complex II activity, and MCL-1 silencing or pharmacologic inhibition restored VEN sensitivity. In support of the importance of RAS/MAPK activation, we found by single-cell DNA sequencing rapid clonal selection of RAS-mutated clones in AML patients treated with VEN-containing regimens. In summary, these findings establish RAS/MAPK/MCL-1 and mitochondrial fitness as key survival mechanisms of VEN-RE AML and provide the rationale for combinatorial strategies effectively targeting these pathways.On November 25, 2021, the European Medicines Agency (EMA) authorized the presentation of Comirnaty vaccine (Pfizer-Biontech) for children between 5 and 11 years of age. In our country, this vaccination began on December 15, after it was approved by the Public Health Commission. A mathematical model has been developed to evaluate the possible impact of this vaccination and contribute to assessing the risk-benefit balance. The model parameters were adjusted including all those characteristics that may influence the impact of childhood vaccination. The results indicate this vaccination involves a significant reduction in the number of infections, and to a lesser extent in the number of hospitalizations and deaths. This reduction will be more important in a future epidemic wave, when this population has been fully vaccinated. Mathematical models can be very powerful tools to predict the impact of vaccination in different epidemiological situations, and help to adjust vaccination programs to be more efficient.BACKGROUND Myelosuppression is one of the most common chemotherapy-induced adverse events and results in a series of clinical symptoms. This study aimed to evaluate the effect of Shengbai decoction (SD) on chemotherapy-induced myelosuppression and survival of gastric cancer (GC) patients after radical resection. MATERIAL AND METHODS We retrospectively analyzed data from 115 patients with stage II-III GC who underwent adjuvant chemotherapy after radical resection between May 2015 and June 2017 in our hospital. Among these patients, 57 received Shengbai decoction along with adjuvant chemotherapy (SD group), while 58 received adjuvant chemotherapy alone (control group). Medical records, including adverse events, the treatment completion rate of adjuvant chemotherapy, 3-year overall survival (OS), and 3-year recurrence-free survival (RFS), were compared. RESULTS Patient characteristics did not differ significantly between the 2 groups. No adverse events related to Shengbai decoction were reported in the SD group. Patients in the SD group had less neutropenia (P=0.0430), thrombocytopenia (P=0.0323), and anemia (P=0.0497). The SD group had a significantly lower probability of dose reduction (P=0.0448). The completion rate of adjuvant chemotherapy of the SD group was considerably higher than that of the control group (P=0.0398). The SD group had a significantly better 3-year RFS (P=0.0369) and 3-year OS (P=0.0455) than the control group. CONCLUSIONS Shengbai decoction effectively improved postoperative survival of patients with GC by alleviating chemotherapy-induced myelosuppression and improving the completion rate of adjuvant chemotherapy.BACKGROUND Hodgkin lymphoma is a type of lymphoid neoplasm characterized by the presence of Reed-Sternberg cells in histopathology. It presents with swollen lymph nodes, and in advanced cases, it can involve bone marrow. Isolated and primary involvement of Hodgkin lymphoma in bone marrow is a very rare presentation, especially in an HIV-negative patient, and has a high mortality rate, even with appropriate management. CASE REPORT A 36-year-old male patient presented with a 6-month history of chills, night sweats, and generalized weakness. Laboratory investigations showed pancytopenia. A peripheral blood smear was unremarkable. A bone marrow biopsy revealed Reed-Sternberg cells, consistent with Hodgkin lymphoma. Further imaging did not show any lymphadenopathy, and the spleen was normal. Therefore, primary bone marrow Hodgkin lymphoma was diagnosed at stage IV, with an International Prognostic Score of 5 potential unfavorable factors. He was started on conventional chemotherapy for Hodgkin lymphoma and received the first cycle of ABVD (adriamycin, bleomycin, vinblastine, and dacarbazine) during his hospitalization. Meanwhile, he had fever without focus, and was managed empirically by broad-spectrum antibiotic treatment (piperacillin-tazobactam). Hospitalization lasted for 2 weeks. He continued to receive other cycles as an outpatient, with good clinical response. The patient remained in complete remission after 2 years. CONCLUSIONS Isolated Hodgkin lymphoma of bone marrow is a very rare scenario, especially in HIV-negative patients, with no established management for patients. Here, we report a patient successfully treated with a conventional chemotherapy protocol for Hodgkin lymphoma.

The emergence of triptans represented a breakthrough in the treatment of migraine, but in clinical practice, patients describe symptoms that resemble those of a hangover after taking them. We propose the use of the Hangover Symptoms Scale (HSS) to evaluate this syndrome in patients that take triptans, which may help identify patients at higher risk of presenting these adverse effects that may interfere with therapeutic compliance.A cross-sectional observational pilot study with prospective data collection through a clinical-demographic questionnaire and the HSS was carried out on patients with migraine treated in headache units in 3 tertiary hospitals in Madrid.Sixty-six patients were included in the study. The median HSS was 4 and all symptoms were present in at least 15% of the patients, with difficulty to concentrate being the most frequent (57.6%). No significant differences were found between the presence of a higher HSS score and the sociodemographic characteristics of the patient or his migraine. The clinical features could be more frequently associated with the appearance of these adverse effects that, however, are not related to any particular patient profile.

High-quality exercise training improves outcomes in cardiac rehabilitation (CR), but little is known about how most programs prescribe exercise. Thus, the aim was to describe how current CR programs prescribe exercise.

We conducted a 33-item anonymous survey of CR program directors registered with the American Association of Cardiovascular and Pulmonary Rehabilitation. We assessed the time, mode, and intensity of exercise prescribed, as well as attitudes about maximal exercise testing and exercise prescription. Results were summarized using descriptive statistics. Open-ended responses were coded and quantitated thematically.

Of 1470 program directors, 246 (16.7%) completed the survey. In a typical session of CR, a median of 5, 35, 10, and 5 min was spent on warm-up, aerobic exercise, resistance training, and cooldown, respectively. The primary aerobic modality was the treadmill (55%) or seated dual-action step machine (40%). Maximal exercise testing and high-intensity interval training (HIIT) were infrestandardize exercise intensity in CR.

This study explored the perceived barriers and facilitators to participation in patients who did and did not attend virtual cardiac rehabilitation (CR) education sessions.

A mixed-methods approach was used. Virtual patient education was delivered during the coronavirus-19 pandemic. Phase 1 included a cross-sectional online survey completed by individuals who did and did not participate in these sessions. For phase 2, six virtual focus group sessions were conducted using the social-ecological framework to guide thematic analysis and interpretation of findings.

Overall, 106 online surveys were completed; 60 (57%) attended Cardiac College Learn Online (CCLO) sessions only, one (1%) Women with Heart Online (WwHO) only, 21 (20%) attended both, and 24 (22%) did not attend virtual sessions. Half of the participants who attended virtual sessions viewed between one and four sessions. Most participants were from Canada (95%) and included the Toronto Rehabilitation Institute/Toronto Western Hospital centers (76%).
