33 [1.22-17.2], reference 25-39 years), nulliparity (ORa = 3.49 [1.25-11.2]), and an abnormal fetal umbilical artery Doppler velocimetry (ORa = 3.50 [1.47-8.70]). The risk of poor neonatal condition did not differ significantly between women with vaginal and caesarean deliveries (2.3 % vs 7.3 %, P = 0.21).

When FGR is suspected at 36 weeks of gestation and later, induction of labour is a reasonable option, even if the cervix is unripe, as the risk of caesarean delivery appears acceptable and neonatal status is good and similar with both modes of delivery.

To evaluate a thrombin gel matrix (TGM), Floseal, for the prevention of lymphocele in patients with endometrial cancer who underwent pelvic lymphadenectomy.

A total of 79 consecutive patients with endometrial cancer were randomly allocated to one of two groups the TGM group and control group. After completion of the lymphadenectomy, 5 mL of Floseal was applied to the bilateral pelvic sidewalls, especially the femoral canal, obturator, and common iliac vessels areas. Computed tomography scans were obtained for lymphocele evaluation 2 months after the surgery.

Three patients from the TGM group, and four patients from the control group were lost during follow-up, and data from 36 participants from each group were analyzed. As the primary outcome, lymphocele developed in 12 patients in both groups (33 %). There was no significant difference between the groups in terms of lymphocele and symptomatic lymphocele development. Lymphocele localization was also not different between the two groups, but the diameter of the lymphoceles detected in the TGM group was significantly greater (p = 0.021). The mean drainage days was significantly shorter in the TGM group (p = 0.015). The amount of drainage was also less in the TGM group, but the difference was not statistically significant.

Thrombin gel matrix applied to the pelvic sidewalls does not reduce the incidence of symptomatic or asymptomatic lymphoceles after pelvic lymphadenectomy in endometrial cancer. However, it can reduce the amount of drainage and the number of drainage days so it can help to shorten hospitalization.

Uterine metastases from breast cancer are uncommon and rarely reported in the literature. We described the case of a 50 years-old-woman who developed a uterine metastasis, 6 years after the diagnosis of an invasive ductal breast carcinoma. Indeed, although the patient was asymptomatic, the monitoring imaging examinations, particularly the computed tomography (CT) and the positron emission tomography/computed tomography (PET/CT), showed a myometrial lesion. Non-conservative total hysterectomy was performed. The anatomo-pathological examination revealed a myometrial metastasis from an invasive ductal breast carcinoma. C75 Seventeen months after surgery, the patient had no pelvic recurrence, but lungs and bones metastases progressed despite chemotherapy. In the lack of guidelines of uterine metastases from breast cancer's management, we reviewed the existing literature with the aim to provide a rational framework for clinical presentation, diagnostic approach, histological findings and treatment of this rare and heterogeneous pathology. Uterine metastases of breast cancer are frequently revealed with metrorrhagia. They occur preferentially in tumours with initial lobular carcinoma, initial lymph node involvement and positive hormonal receptors.Hyperbranched polymers (HBPs), such as hyperbranched polyglycerols (HPGs) with a dendritic configuration, have been recognized for their excellent biocompatibility and multifunctionalization. HPGs have been studied for use in the delivery diagnostic, imaging and therapeutic molecules in the area of nanobiomedicine. They show superior characteristics to linear polymers and dendrimers, such as compact structure, a simple manufacturing process with easy functionalization ability, low viscosity, and high stability. Owing to these advantages, HPGs are now considered promising carriers for drug delivery, diagnostics, imaging, and theranostics applications for cancer treatment. In this review, we also discuss safety aspects of HPG-based nanoformulations in various animal models and the clinical translation status of such polymers for real-time applications.Optimum use of generic products would require equivalence, not only in terms of quality, safety, and efficacy in clinical studies, but also patient acceptability to not jeopardize treatment success because of non-adherence which would de facto limit the potential cost saving anticipated by their use. Although acceptability is a requirement for the authorization of pediatric innovator products, a survey of European Union (EU) regulatory authorities showed that few have a formal process for assessing patient acceptability of generic products during the registration processes. The current International Council for Harmonisation of Technical Requirements for Pharmaceuticals for Human Use (ICH) focus on unifying guidance for the development and scrutiny of generics but should include acceptability alongside the other factors being considered for harmonization.traumatic cataract is a common complication of ocular contusion. Due to various forms of trauma, its condition and prognosis are also different. Intraocular lens (IOL) implantation is the most effective treatment method at present, which can provide cataract patients with good visual quality and near, medium, and far vision. In this study, an adaptive medical ultrasonic image de-noising algorithm was constructed based on the biological vision mechanism and was applied to observe the IOL implantation and postoperative care of 30 patients with traumatic cataract. The results showed that the eyesight could be restored to the original healthy state after the IOL implantation and the eyesight of the patient was close to normal 3 months after surgery. The corresponding noise pixel block of the noise reduction algorithm was obviously the lowest. 30 patients (9 cases with contusion, 13 cases with intraocular foreign body, and 8 cases with perforation) were treated with aspheric and multifocal IOL to achieve the expected effects. In conclusion, the noise reduction algorithm constructed in this study could effectively reduce the speckle noise of ultrasonic images, improve the clarity of ultrasonic images, and realize the effective observation of medical nursing effect after traumatic cataract IOL implantation. After IOL implantation, the retinal nerves of cataract patients were affected to different degrees.

Sporadic vestibular schwannoma (VS) is a benign primary tumor that arises from the vestibular nerve. Growing VS can negatively compress the brain stem, which can lead to death. MicroRNAs (miRNAs) can negatively regulate target genes at the post-transcriptional level and are critical in tumorigenesis. Studies have demonstrated the tumor suppressive function of microRNA-205-5p (miR-205) across many cancers, but no studies have evaluated the role of miR-205 in sporadic VS. We conducted this study to examine the role of miR-205 in sporadic VS cell proliferation.

We evaluated miR-205 expression in sporadic VS tissues and normal great auricular nerve by real-time quantitative polymerase chain reaction. Then, we transfected miR-205 mimics and control oligonucleotides into sporadic VS primary cells to examine the functional significance of miR-205 expression at a cellular level by CCK8 and colony formation and used dual-luciferase reporter assays to find the target gene of miR-205.

We determined that miR-205 levels were downregulated in sporadic VS tissues in comparison to normal controls. In functional assays, miR-205 suppressed proliferation and colony formation ability of sporadic VS cells. CDK14 (cyclin-dependent kinase 14) was identified as a target gene of miR-205 by bioinformatics, and validated using dual-luciferase reporter assays. Moreover, miR-205 overexpression inhibited levels of phosphorylated PI3K and Akt.

These findings suggested that miR-205 suppressed sporadic VS proliferation by targeting CDK14 and may be considered as a potential drug therapy for sporadic VS treatment in the future.

Anterior cervical discectomy and fusion (ACDF) are increasingly performed at ambulatory surgical centers (ASCs). Academic centers lacking dedicated ASCs must perform these at large university hospitals, which pose unique challenges to cost savings and efficiency.

To describe the safety and cost of outpatient ACDF at a major academic medical center without a dedicated ASC.

ACDFs performed from 2015 to 2018 were retrospectively reviewed. Cases were performed at the major tertiary university hospital or a satellite university hospital dedicated to outpatient surgery. Patient demographics, surgical characteristics, perioperative complications, fusion at 12 months, and cost were collected.

A total of 470 patients were included. The mean age was 56 years, with 255 women (54.3%). When comparing same-day discharge, overnight observation, or inpatient admission, there were no differences in age, gender, or number of levels fused. Same-day and overnight observation cases were associated with shorter procedure d appropriately selected patients into same-day or overnight observation care pathways. This strategy can improve resource utilization and preserve precious hospital resources for the most critically ill patients while also allowing these centers to build viable outpatient spine practices. The paper used toremifene to study the characteristics of hemodynamic changes before and after neoadjuvant chemotherapy in breast cancer by using resting state functional MRI quantitative method, and to analyse the effect of toremifene on the recent quality of life of advanced breast cancer. Methods A total of 100 patients who received endocrine therapy after breast cancer surgery in our hospital from January 2016 to January 2019 were collected as the research objects. They were randomly divided into observation group and control group, with 50 cases in each group. The observation group was treated with tamoxifen Combined toremifene treatment, the control group was treated with toremifene. Before and after chemotherapy, the same scheme was used to perform breast dynamic MRI enhancement scan, using 1.5T superconducting MRI imager, 3ml/s bolus injection of adiphenine meglumine 0.2mmol/kg; semi-quantitative blood flow measurement was completed on the workstation, and before and after chemotherapy Compare the results.
