Patients with epilepsy can experience different neuropsychiatric symptoms related (peri-ictal) or not (interictal) with seizures. Peri-ictal symptoms can precede (pre-ictal) or follow (post-ictal) the seizure, or even be the expression of the seizure activity (ictal). Three-dimensional finite element analysis was used to simulate stress distribution under static loading in non-resorption and resorption scenarios (3-mm vertical bone loss) in implants restored with single crowns and C/I ratios of 11, 1.51, and 21 were evaluated. Different crown materials were used porcelain-fused to metal, porcelain-fused to zirconia, monolithic zirconia, and zirconia-based crown veneered with indirect composite resin. The C/I ratio, the peri-implant bone resorption, and the loading conditions were the key factors affecting the generated stress in short implants. In non-resorption models, von Mises stress ranged between 50 and 105 MPa whereas in resorption models, the values ranged from 168 to 322 MPa, both increasing with the higher C/I ratio under oblique forces. Under axial loading, the C/I ratio did not influence the stress values as the presence of resorption was the only parameter increasing, 57 MPa for the non-resorption models and 101 MPa for the resorption models, respectively. Preference of a prosthetic material was ineffective on the distribution of stress in the bone and implant structure under static loading in any models. The peri-implant bone resorption and a higher C/I ratio in short implants increase the stress values under both axial and oblique forces, whereas the crown material does not influence stress distribution in the surrounding bone and implant structure.In this paper, we evaluate the effects of mindfulness meditation training in electrophysiological signals, recorded during a concentration task. Longitudinal experiments have been limited to the analysis of psychological scores through depression, anxiety, and stress state (DASS) surveys. Here, we present a longitudinal study, confronting DASS survey data with electrocardiography (ECG), electroencephalography (EEG), and electrodermal activity (EDA) signals. Twenty-five university student volunteers (mean age = 26, SD = 7, 9 male) attended a 25-h mindfulness-based stress reduction (MBSR) course, over a period of 8 weeks. There were four evaluation periods pre/peri/post-course and a fourth follow-up, after 2 months. All three recorded biosignals presented congruent results, in line with the expected benefits of regular meditation practice. Traditionally, Klatskin tumor resection is performed using an "open" approach until recently when the minimally invasive technique becomes popular due to its proven advantages. The laparoscopic technique had been reported; however, the majority of resections were types I and II without the need for ipsilateral hepatectomy. Inherent limitations of straight laparoscopic instruments result in significant technical difficulties in performing precise tissue dissection and vessel repair and creating a fine bilioenteric anastomosis. In this didactical video, we described our technique of type IIIB Klatskin tumor (B3-L perihilar cholangiocarcinoma) resection utilizing a robotic technology. The use of a robotic platform facilitates precise porta hepatic dissection, bleeding control, and creation of a fine bilioenteric anastomosis at the level of the hilar plate. We believe that the robotic platform provides an alternative method for resection of perihilar cholangiocarcinoma with excellent short-term outcomes.

Lymphoma is the one of the most common cancer diagnoses among adolescents and young adults (AYAs) aged 15-39. Despite significant advances in outcomes observed in older adults and younger children, improvements in AYAs have lagged behind. The reasons for this are likely multifactorial including disparities in access to health insurance, low rates of enrollment to clinical trials, potential differences in disease biology, and unique psychosocial challenges. Here we will review Hodgkin lymphoma (HL) and primary mediastinal B cell lymphoma (PMBCL), two of the most common aggressive lymphomas that occur in AYAs. We will discuss the current knowledge about disease biology in AYAs, adult and pediatric treatment strategies, novel targeted therapies, and ongoing AYA clinical trials in these lymphoma subtypes. We also will review unique considerations for treatment-related toxicities in AYAs and psychosocial issues relevant to this population.

Pediatric and adult trials in HL and PMBCL have demonstrated that treatc regimens with or without radiation results in high cure rates but can also be associated with long-term toxicity which must be considered in this young population. Novel targeted agents such as the antibody-drug conjugate brentuximab vedotin and/or antibodies targeted against PD-1/PD-L1 have demonstrated activity in the relapsed setting and are currently being evaluated in the upfront setting, which may reduce our reliance on therapies associated with long-term toxicity. AYA-focused clinical trials are currently underway to better elucidate the optimal therapy for lymphomas in this age group. There is an urgent need for clinical trials including AYAs in order to increase the knowledge of age-specific outcomes, toxicities, disease biology, and the need to develop comprehensive AYA care models that meet the unique and complex care needs of this patient population.
