Medical treatment of mild to moderate migraines include non-prescription non-steroidal anti-inflammatory drugs, acetaminophen, or aspirin and caffeine-containing combination analgesics. Additionally, moderate to severe migraines and those that are mild to moderate that have not responded to analgesics can be treated with triptans, which are drugs specific for migraine treatment. Non-pharmacological treatments include cognitive behavioral therapy and relaxation training. Medications for the prevention of migraines have also been developed since they are more affective in offsetting the symptoms. selleck chemicals llc Ubrogepant's high specificity and selectivity for calcitonin gene-related peptide (CGRP) sets it apart from certain other drugs, which previously limited the treatment of migraines with or without aura due to their decreased selectivity. The most frequently reported side effects are oropharyngeal pain, nasopharyngitis, and headache. Most studies found that participants receiving Ubrogepant were free from pain within 2 h when compared to placebo. selleck chemicals llc Patients taking Ubrogepant should avoid taking it when pregnant or with end stage renal disease. link2 In summary, Ubrogepant has good tolerability and an overall favorable safety profile. It appears to hold promise for the acute treatment of migraines with or without aura in adults. Many applications of plant pathology had been enabled by the evolution of artificial intelligence (AI). link2 For instance, many researchers had used pre-trained convolutional neural networks (CNNs) such as the VGG-16, Inception, and Google Net to mention a few, for the classifications of plant diseases. The trend of using AI for plant disease classification has grown to such an extent that some researchers were able to use artificial intelligence to also detect their severities. The purpose of this study is to introduce a novel approach that is reliable in predicting severities of the maize common rust disease by CNN deep learning models. This was achieved by applying threshold-segmentation on images of diseased maize leaves (Common Rust disease) to extract the percentage of the diseased leaf area which was then used to derive fuzzy decision rules for the assignment of Common Rust images to their severity classes. The four severity classes were then used to train a VGG-16 network in order to automatically classify the test images of the Common Rust disease according to their classes of severity. Trained with images developed by using this proposed approach, the VGG-16 network achieved a validation accuracy of 95.63% and a testing accuracy of 89% when tested on images of the Common Rust disease among four classes of disease severity named Early stage, Middle stage, Late Stage and Healthy stage. The scientific literature has shown that the renowned Italian Renaissance artist and genius of human anatomy Michelangelo Buonarroti (1475-1564) included his self-portrait into some of his most famous works. It has been suggested that the various self-portraits and self-caricatures the artist used in his works over the years may offer some insight into Michelangelo's physical form and, consequently, provide evidence of his health at different stages of his life. Accordingly, this manuscript presents new evidence [based on facial features described by Daniele of Volterra (1509-1566) and Giorgio Vasari (1511-1574)] that Michelangelo may have inserted his self-portrait into one of the figures that make up the Epifania cartoon, made by the artist in 1553, which is currently in the collection of the British Museum in London, England. Thus, the information contained in this manuscript is not only useful for future analyses of Michelangelo's health [based on facial features] when he was approximately 78 years old, but also how the artist, who was known for being very introspective, saw himself physically in his old age.N/A.A new cause of POS hás been recently described SARS-CoV-2 pneumonia. Important aspects regarding the case are presented and clarifications requested from the authors. Dr Howard B. One of the most significant innovations contained in the "Gelli" Law is a new provision in case of medical malpractice ('imperizia'), where the healthcare professional will avoid liability if he or she can show that they have acted in accordance with recommended guidelines published under law. In the absence of these guidelines the professional must adhere to principles of good practice.1 Guidelines provided by the law aim to standardize medical acts. They are based on the principles of evidence-based medicine, an approach to medical practice aimed at optimizing the decision-making process. link2 In literature many physicians and jurists have commented, in a discordant manner, the choices of the Legislator. In this article a historical perspective is proposed, finding analogies between the principles of the "Gelli" law and positivist thought. The evolution that Positivism has had in the medical field has been analyzed, in particular studying the evolution of the thought of the Italian physician and strong positivist Paolo Mantegazza (1831-1910). In light of the facts it is concluded that the physician must always keep in mind that medicine is a science applied to man, and any medical act must be a consequence of a doctor-patient concordance that inevitably takes into consideration every single individual in his uniqueness. Thus, although difficult the next step of the legislator should be to favor the establishment of high-quality medical-patient relationships. In any case, if the physician has been most careful in managing a good relationship with each individual patient and in understanding her/his situation before performing medical acts, she/he may perhaps fear less potential judicial repercussions.

We present the cases of two adult male patients with painfully delayed union of proximal tibia diaphyseal fracture after intramedullary nailing. Patients underwent to nail dynamization and Poller blocking screw augmentation at 3 and 5 month, respectively, after the index surgery. Both patients were pain-free after the surgery and bone-union were radiographically evident after 3 months. At 12-month follow-up, patients returned to their previous activities.

Although nail dynamization is the choice treatment to obtain fracture compression for delayed union of tibial shaft fractures, augmentation with Poller blocking screw could provide additional mechanical stiffness in unstable supra-isthmic shaft fractures of the tibia and favour bone union.

Several etiopathogenetic mechanisms connecting thyroid diseases and thrombocytopenia have been described. Thrombocytopenia is often documented in patients with Graves' disease, due to reduced platelet life span in hyperthyroidism, immune dysregulation and genetic predisposition (HLA B8 presence). We present the case of a 14-years old girl, who was referred to our Pediatrics Unit, because of contemporary appearance of weight loss, profuse sweating and episodes of recurrent epistaxis. A complete health team, made up of hematologists and endocrinologists, met in consultation in order to reach a diagnosis. A suppression of serum TSH concentrations, the presence of anti-TSH receptor antibodies, and at the same time an immune thrombocytopenia with positive anti-platelet antibodies, have been detected. Furthermore, a positive direct and indirect Coombs test without hemolytic anemia, ANA positivity, and a C4 consumption have been documented. The patient started treatment with thiamazole with progressive improvement of thyroid function and thrombocytopenia, requiring only an intravenous immunoglobulin infusion on one time. link3 A multidisciplinary follow-up has been scheduled, in order to monitor the multi-organ immune dysregulation. This report documents a significant improvement of thrombocytopenia after antithyroid treatment in a young subject affected with Graves' disease. Background and aim of the work In the orthopaedic and traumatological fields septic non-unions represent a severe complication, hard to manage and treat. link3 Traditionally, the surgical technique consists in to two sequential steps debridement with administration of local and systemic antibiotics associated with temporary stabilization of the fracture and subsequent reconstruction of bone and soft tissues. Recently, the use of some devices to treat septic non-union by one-step surgery have been introduced with encouraging resultsMethods We reported our experience with a case treated by one-step procedure using a custom-made intramedullary antibiotic cement-coated carbon nail. We reviewed the literature and described the surgical technique employed in this case. Results At 6 months from surgery the patient was able to perform full weight-bearing and carry out the normal activities of daily living. Serum inflammatory markers normalized and radiographic controls showed the presence of a mechanically good bone callus at the non-union site. link3 The bone resection carried out determined a limb length discrepancy of 3 cm, that was corrected through a temporary shoe lift, currently well tolerated. The patient regained full ROM of the right knee. Conclusion Intramedullary antibiotic cement-coated nail associated with systemic antibiotic therapy proved to be an effective treatment to control the infection and provide immediate stability at the septic non-union/fracture site, allowing a rapid functional recovery. It represents a valid option especially in patients who refuse external devices or surgical additional procedures, as in our case. (www.actabiomedica.it).

The involvement of myocardium and pericardium at the same time is very uncommon as first manifestation of juvenile idiopathic arthritis with systemic onset (soJIA).

A fourteen years-old boy, referred with symptoms of acute gastroenteritis, developed a perimyocarditis as first manifestation of Still's Disease, after only one day from the admission. The rheumatologic disease was not responding to glucocorticoid treatment. The use of anakinra was the key point of the therapy and after its administration the patient started to recover fastly.

This case report describes cardiac involvement as first sign of soJIA and the successful use of anakinra inducing remission of soJIA not-responding to steroid therapy.
