Functional appliances (FA) have a positive effect on the upper airway volume and minimal cross-sectional area (MCA) in children. An association between morphologic deviations of the upper spine (MDUS) and reduced treatment response was found in appliances used to treat adults with obstructive sleep apnea. This study aimed to (1) compare airway changes after FA treatment in children with and without MDUS and controls; (2) identify if MDUS causes a smaller upper airway.
Pretreatment and posttreatment cone-beam computed tomography scans were included from 21 children with MDUS and 42 without MDUS treated with a fixed FA, along with a pair-matched control group (matched for chronological age, skeletal age, gender, and mandibular inclination) who received orthodontic treatment for minor malocclusions without an FA. The influence of MDUS on changes in upper airway volume and MCA were evaluated with 3-dimensional cone-beam computed tomography scans using standardized, previously validated methods and mixed-effects linear regression.
There was a significantly increased volume and MCA in the FA groups with and without MDUS compared with control (P= 0.003 and P= 0.049) and in the FA group without MDUS compared with the MDUS group (P= 0.008 and P= 0.011) after treatment. There was no significant pretreatment difference in airway dimensions between the MDUS and non-MDUS FA groups.

To identify markers of Parkinson's disease (PD) related anxiety, using high density electroencephalography (hd-EEG).
108 patients participated in the study. They were divided into two groups with and without clinically relevant anxiety, according to their score on the Parkinson Anxiety Scale. Resting-state hd-EEG was recorded. Spectral and functional connectivity characteristics were compared between the two groups.
Thirty-three patients (31%) had significant anxiety symptoms. In the spectral analysis, relative power in the alpha1 frequency band in the right prefrontal cortex was lower in patients with anxiety than without. Functional connectivity analysis showed a stronger connectivity between the left insula and several regions of the right prefrontal cortex in patients with anxiety than in those without.
This study shows the pivotal role of the insula and frontal cortex in the pathophysiology of anxiety in PD and extends the results of previous studies using magnetic resonance imaging or positron emission tomography imaging.

Auditory nerve injury is one of the most common nerve injury complications of skull base fractures. However, there is currently a lack of auxiliary examination methods for its direct diagnosis. The purpose of this study was to find a more efficient and accurate means of diagnosis for auditory nerve injury.
Through retrospectively analyzing the results of brainstem auditory evoked potential (BAEP) and high-resolution CT (HRCT) in 37 patients with hearing impairment following trauma from January 1, 2018 to July 31, 2020, the role of the two inspection methods in the diagnosis of auditory nerve injury was studied. Inclusion criteria were patient had a clear history of trauma and unilateral hearing impairment after trauma; while exclusion criteria were (1) severe patient with a Glasgow coma scale score ≤5 because these patients were classified as severe head injury and admitted to the intensive care unit, (2) patient in the subacute stage admitted 72h after trauma, and (3) patient with prior hearing impairmen16.22%) were both negative for BEAP and HRCT test, and 10 patients (27.03%) were BAEP-negative but HRCT-positive all the 16 patients were considered as non-neurological injury. The rest 1 case (2.70%) was BAEP-positive but HRCT-negative, which we speculate may have auditory nerve concussion.
By way of BAEP combining with skull base HRCT, we may improve the accuracy of the diagnosis of auditory nerve injury. Such a diagnostic strategy may be beneficial to guiding treatment plans and evaluating prognosis.

In this study, we assessed whether systemic thrombolysis decreases hemodynamic decompensation and mortality in a cohort of unselected patients with IHR compared with patients with conventional anticoagulation.

Historically, intubation success rates have depended on human factors, such as provider familiarity with intubation. There has been relatively little literature examining intubation success rates as a factor of EMS transport environment, despite there being important human factors differences between the different environments.
We queried a national database of EMS calls in the United States. Inclusion criteria were oral endotracheal intubations performed in 2019 where providers documented whether or not the intubation was successful and what mode of transport they were assigned to.

In recent years, they have been used for patient selection for proton beam therapy (PBT) for some anatomical tumour sites. This review synthesizes the published evidence regarding the use of NTCP models to predict the toxicity of PBT, for different end points in patients with brain tumours. A search of Medline and Embase using the Patients, Intervention, Comparison, Outcome (PICO) criteria was undertaken. In total, 37 articles were deemed relevant and were reviewed in detail. Nineteen articles on NTCP modelling of toxicity end points were included. Of these, 11 were comparative NTCP studies of PBT versus conventional photon radiotherapy (XRT), which evaluated differences in plan dosimetry and then assumed that XRT-derived literature estimates of NTCP would be applicable to both. Seven papers derived NTCP models based on PBT outcome data, two of which provided model parameters. Among analysed end points, the reduced risk of secondary tumours with PBT as compared with XRT is estimated - through modelling studies - to be considerable and was highlighted by most authors.
