Results indicated that better client-rated (r = 0.20) and other-rated (i.e., provider- or observer-rated; r = 0.25) alliance were associated with better treatment engagement. Treatment type and sample race/ethnicity, but not age, gender, or timing of alliance rating moderated the association between other-rated alliance and engagement. Further, better other-rated alliance was related to improvements in positive (r = -0.14) and negative (r = -0.22) symptoms. A strong therapeutic alliance is important for both engaging clients with SSDs and early psychosis in treatment and facilitating improvements in positive and negative symptoms. Delivery and monitoring of treatments for this population should include assessment of the therapeutic alliance from multiple perspectives.Three-dimensional (3D) integrated renal structures (IRS) segmentation targets segmenting the kidneys, renal tumors, arteries, and veins in one inference. Clinicians will benefit from the 3D IRS visual model for accurate preoperative planning and intraoperative guidance of laparoscopic partial nephrectomy (LPN). However, no success has been reported in 3D IRS segmentation due to the inherent challenges in grayscale distribution low contrast caused by the narrow task-dependent distribution range of regions of interest (ROIs), and the networks representation preferences caused by the distribution variation inter-images. In this paper, we propose the Meta Greyscale Adaptive Network (MGANet), the first deep learning framework to simultaneously segment the kidney, renal tumors, arteries and veins on CTA images in one inference. It makes innovations in two collaborate aspects 1) The Grayscale Interest Search (GIS) adaptively focuses segmentation networks on task-dependent grayscale distributions via scaling the window width and center with two cross-correlated coefficients for the first time, thus learning the fine-grained representation for fine segmentation. 2) The Meta Grayscale Adaptive (MGA) learning makes an image-level meta-learning strategy. It represents diverse robust features from multiple distributions, perceives the distribution characteristic, and generates the model parameters to fuse features dynamically according to image's distribution, thus adapting the grayscale distribution variation. This study enrolls 123 patients and the average Dice coefficients of the renal structures are up to 87.9%. Fine selection of the task-dependent grayscale distribution ranges and personalized fusion of multiple representations on different distributions will lead to better 3D IRS segmentation quality. Extensive experiments with promising results on renal structures reveal powerful segmentation accuracy and great clinical significance in renal cancer treatment.Effects were evaluated in Bos indicus cows of eCG and FSH on follicular growth, estrous expression, and pregnancy per artificial insemination (P/AI) as a result of fixed-time artificial insemination (TAI). In Experiment 1, extent of timing-of-ovulation synchronization among cows was evaluated after imposing an estrogen/progesterone-based treatment regimen. At progesterone device removal (D8), cows were administered eCG, or FSH or served as untreated Controls. In Experiment 2, percentage of cows P/AI was evaluated when the Experiment 1-treatment regimen was imposed. On D10, all cows were artificially inseminated. In Experiment 3, cows were assigned to two treatment groups (Control and eCG) on D8 to evaluate percentage of cows P/AI and estrous expression. In Experiment 1, follicular dynamics were similar among treatment groups. In Experiment 2, follicular growth was greater (P = 0.0001) with the eCG treatment. There was an interaction of treatment × parity (P = 0.007) on percentage of cows P/AI. There was a greater percentage of primiparous cows P/AI in the eCG-treated than Control and FSH-treated cows. There was a greater percentage of eCG-treated multiparous cows pregnant as a result of TAI than Control cows. There was an interaction of treatment × parity (P = 0.005) on P/AI in Experiment 3, in which the eCG effect was more pronounced in primiparous cows. Treatment with FSH, therefore, was not as effective as eCG in stimulation of follicular growth or enhancing percentage of cows pregnant as a result of TAI. Physiological effects of eCG, however, were also more evident in primiparous cows.
In December 2019, the global outbreak of Corona Virus Disease 2019 (COVID 19) was reported. The purpose of this study was to understand the working experience of triage nurses in the emergency department (ED) of a large teaching general hospital in Shenzhen (Guangdong province, China) during the COVID-19 epidemic. This will provide a basis for improving the emergency nursing strategies and the epidemic response capabilities of triage nurse.
Ten triage nurses were selected as subjects by objective sampling for in- depth interviews, and the data were analyzed by the Colaizzi seven-step analysis method.
There were four themes in the working experience of triage nurses, including fear of infection and transmission, job stress, gratitude, and expectations of managers.
During the COVID-19, the work experience of triage nurses mainly included the fear of infection and transmission, the high work pressure, the sense of team strength and the care of leaders. It was suggested that nursing managers should ensure the human resources of triage nurses, increase training, strengthen emergency drills, improve emergency nursing countermeasures, and improve the response capability of triage nurses during the epidemic.
