Wrist trauma is common in children, typically requiring radiography for diagnosis and treatment planning. However, many children do not have fractures and are unnecessarily exposed to radiation. Ultrasound performed at bedside could detect fractures prior to radiography. Modern tools including three-dimensional ultrasound (3DUS) and artificial intelligence (AI) have not yet been applied to this task. Our purpose was to assess (1) feasibility, reliability, and accuracy of 3DUS for detection of pediatric wrist fractures, and (2) accuracy of automated fracture detection via AI from 3DUS sweeps. Children presenting to an emergency department with unilateral upper extremity injury to the wrist region were scanned on both the affected and unaffected limb. Radiographs of the symptomatic limb were obtained for comparison. Ultrasound scans were read by three individuals to determine reliability. An AI network was trained and compared against the human readers. Thirty participants were enrolled, resulting in scans from fifty-five wrists. Readers had a combined sensitivity of 1.00 and specificity of 0.90 for fractures. AI interpretation was indistinguishable from human interpretation, with all fractures detected in the test set of 36 images (sensitivity = 1.0). The high sensitivity of 3D ultrasound and automated AI ultrasound interpretation suggests that ultrasound could potentially rule out fractures in the emergency department.Upper cross syndrome (UCS) is a condition caused from prolonged poor posture manifesting as thoracic hyperkyphosis with forward head and shoulder postures. This is a case report of a 35-year-old female presenting to clinic with a sharp pain in the neck, upper back, and sternum area for 4 weeks and gastroesophageal reflux disease (GERD). She had been working at home for several months after the shelter at home order was issued. Following evaluation and corrective treatment with cervical adjustment and soft tissue massage, the patient's posture improved and reported full pain resolution. Manipulative and preventive therapies aimed at treating and preventing UCS should be more widely adopted to prevent secondary conditions.Essentially, every failure of a short fiber reinforced composite (SFRC) under tension is induced from a matrix failure, the prediction of which is of fundamental importance. This can be achieved only when the homogenized stresses of the matrix are converted into true values in terms of stress concentration factors (SCFs) of the matrix in an SFRC. Such an SCF cannot be determined in the classical way. In this paper, a closed-form formula for the longitudinal tensile SCF in the SFRC is derived from the matrix stresses determined through an elastic approach. The other directional SCFs in an SFRC are the same as those in a continuous fiber composite already available. A bridging model was used to calculate the homogenized stresses explicitly, and a failure prediction of the SFRC with arbitrary fiber aspect ratio and fiber content was made using only the original constituent strength data. Results showed that the volume fraction, the aspect ratio, and the orientation of the fiber all have significant effect on the tensile strength of an SFRC. In a certain range, the tensile strength of an SFRC increases with the increase in fiber aspect ratio and fiber volume content, and the strength of the oriented short fiber is higher than that of the random short fiber arrangement.
