The few reported cases have been associated with severe and refractory dermatomyositis (DM), sometimes in association with cancers. We aimed to determine if oedematous myositis is a homogeneous subtype based on clinical, serological and pathological features. METHODS This is a retrospective observational study performed between 2008 and 2015 in the French national referral center for myositis. All adult patients with an inflammatory muscle biopsy and upper limbs oedema were included as well as IIM cases without limb oedema as controls. Clinical, biological and pathological features were collected. RESULTS Seventeen oedematous myositis were included and compared to 174 IIM without oedema, including 50 DM controls. Oedema was the first manifestation in 23% of patients. Muscle weakness was severe and symmetric, 71% of patients presented dysphagia and a restrictive ventilatory pattern was found in 40%. Chronic health issues require integrated services from health and social services. Navigator positions are effective in assisting chronic disease patients to access services. This family program in a small rural town in Western New South Wales targeted marginalised families with children under five years of age with a chronic health issue. The navigator developed a cross-sectoral care plan to provide services to address family issues. The study aimed to identify navigator factors supporting improved family outcomes. METHODS Participants included parent/clients (n = 4) and the cross-sectoral professional team (n = 9) involved in the program. During the interview, participants were asked about their perspective of the program. Interview transcripts were thematically analysed informed by the Chronic Care Model underpinned by Health Promotion Theory. RESULTS The program improved client family's lives in relation to children's health and other family health and social issues. Trust in the care navigator was the most important factor for parents to join and engage with the program. The care navigator role was essential to maintaining client engagement and supporting cooperation between services to support families. CONCLUSION Essential care navigator skills were commitment, ability to persuade and empower parents and other professionals. SO WHAT? This descriptive study demonstrated the positive influence of the care navigator and the program on high risk families in a small isolated community. KGaA, Weinheim.BACKGROUND Bacillus thuringiensis Cry3 toxins exhibit specific toxicity against several coleopteran larvae. However, owing to its low toxicity to M. alternatus, Cry3A toxin is not useful for managing M. alternatus larvae. Here we assessed the proteolytic activation of Cry3Aa toxin in M. alternatus larval midgut and increased its toxicity by molecular modification. RESULTS Our results indicated that insufficient processing of Cry3Aa protoxin and non-specific enzymatic digestion of Cry3Aa toxin in the midgut of M. alternatus larvae led to low toxicity. The results of transcriptome analysis, enzymatic assay with fluorogenic substrates, and multiplex substrate profiling by mass spectrometry showed that the main digestive enzymes in M. alternatus larval midgut were trypsin-like proteases that preferentially cleaved peptides with arginine and lysine residues. Consequently, trypsin recognition sites were introduced into the domain I of Cry3Aa protoxin in the loop regions between α-helix 3 and α-helix 4 to facilitate proteolytic activation. Multiple potential trypsin cleavage sites away from the helix sheet and functional regions in Cry3Aa proteins were also mutated to alanine to prevent non-specific enzymatic digestion. Bioassays indicated that a modified Cry3Aa-T toxin (K65A, K70A, K231A, K468A, and K596A) showed a 9.5-fold (LC50 = 12.3 μg/mL) increase in toxicity to M. alternatus larvae when compared to native Cry3Aa toxin. CONCLUSION This study highlights an effective way to increase the toxicity of Cry3Aa toxin to M. alternatus, which may be suitable for managing the resistance of transgenic plants to other pests, including some of the most important pests in agriculture. All rights reserved.Biocomposite films containing graphene oxide (GO), poly(vinyl alcohol) (PVA), and chitosan (CS) were prepared using the casting/evaporation technique. These as-prepared films were characterized by Fourier transform infrared spectroscopy, thermogravimetric analysis, and scanning electron microscopy. Results showed that these three component films could be well mixed in certain conditions and the GO sheets were well dispersed. The better performances such as the water resistance, thermal stability, and mechanical properties have been improved. Meanwhile, good antibacterial activity and antibiofilm formation activity of the prepared CS/PVA/GO films were exhibited against Pseudomonas aeruginosa PAO1. The resulting biocomposite films with combination of CS, PVA, and GO advantages have desirable properties and should be suitable for food and medical packaging applications. © 2020 Wiley Periodicals, Inc.
