, given the small size of the trial.Aim To evaluate the clinical picture and factors associated with unfavorable outcomes in admitted patients with COVID-19.Material and methods This study included all patients admitted to the COVID Center of the National Research Center of Cardiology of the Russian Ministry of Health Care from May 1 through May 31, 2020. Clinical demographic, laboratory, and instrumental indexes and associated factors were studied with one-way and multivariate logistic regression analysis.Results This study included 402 patients aged 18 to 95 years (mean age, 62.9±14.6 years); 43.0 % of them were older than 65 years. COVID-19 was frequently associated with chronic comorbidities, including arterial hypertension (74.4 %), obesity (41.6 %), history of ischemic heart disease (12.9 %), atrial fibrillation (18.9 %), type 2 diabetes mellitus (DM) (13.0 %), and oncological diseases (9.2 %). 13.0 % of patients were smokers; less than 10% had chronic lung diseases. 3.9% of patients had a combination of COVID-19 and acute coronary patholulmonary disease, impaired kidney function (creatinine clearance estimated by Cockcroft-Gault <60.0 ml/min), type 2 DM, oncological diseases, and dementia.Conclusion This study established factors associated with unfavorable outcomes in admitted patients with COVID-19. This will allow identifying in advance patients with a high risk of complications that require increased attention to take more active diagnostic and therapeutic measures at prehospital and hospital stages.The factors that influence patient healthcare experiences are complex and connected to place. In northern Canada, the socio-historical context and the inequitable distribution of health services are unique influences on patients. The objective of this study was to examine the characteristics of patient healthcare experiences as reported through news media in the Northwest Territories. We used a case series design to examine patient healthcare experiences reported in news media articles. We conducted a systematic search for articles published between 2008 and 2017 in the online database of a media outlet in the Northwest Territories. We used descriptive statistics to summarise the article characteristics and thematic analysis to understand patient experiences in 128 articles related to 71 cases. Most often, cases involved women, concerned mental health, suicidality, or chronic diseases, and were predominantly negative. Patient experiences included problems associated with medical travel, communication difficulties with providers, lack of cultural safety, and barriers in accessing care resulting in poor-quality care, particularly for Indigenous patients. Broadly, these experiences are rooted in the colonial history in the North. Understanding patient experiences and including Indigenous patients in health system decision-making can help focus policies and clinical care on cultural safety and equity.We aimed to explore factors for optimizing antimicrobial treatment in emergency departments. A single-day point prevalence survey was conducted on January 18, 2020, in 53 referral/tertiary hospitals in 22 countries. 1957 (17%) of 11557 patients presenting to EDs had infections. The mean qSOFA score was 0.37 ± 0.74. Sepsis (qSOFA ≥ 2) was recorded in 218 (11.1%) patients. The mean qSOFA score was significantly higher in low-middle (1.48 ± 0.963) compared to upper-middle (0.17 ± 0.482) and high-income (0.36 ± 0.714) countries (P  less then  0.001). Eight (3.7%) patients with sepsis were treated as outpatients. The most common diagnoses were upper-respiratory (n = 877, 43.3%), lower-respiratory (n = 316, 16.1%), and lower-urinary (n = 201, 10.3%) infections. 1085 (55.4%) patients received antibiotics. One of the main risk factors involved in increasing driving accidents at night is due to the veiling luminance of the car's headlights reducing the contrast of images in the retina and, thus, reducing the visual performance of drivers. The aim of this study is to investigate the veiling luminance caused by the headlights of the best-selling vehicles in Iran. Methods. Using the Stiles-Holiday disability glare equation, the veiling luminance of headlights of common vehicles at distances of 10-100 m for both high-beam and low-beam modes of oncoming vehicles was measured on a road for healthy (young, middle-aged and older) drivers. Results. Under high-beam illumination, as the cars approached each other, the average illuminance of the car headlights gradually increased. Moreover, age had a significant impact on the veiling luminance. This study also showed that, in same-age groups, the Samand Souren and Peugeot Pars cars had the highest and lowest mean veiling luminance, respectively. Conclusion. The findings showed that the illuminance (at large distances between two cars) and the angle between the source of glare and the eyes of drivers (as two cars approach to each other) have a dominant influence on the disability glare.Youths - defined as children under the age of 18 - are frequently involved in agricultural work. This research utilized a qualitative approach to analyze why youths choose to make hazardous decisions regarding grain bin entry and factors that played a role in youths' decisions. Because we know that youth are working on the farm, the study's objective was to examine the parent-youth relationship to determine the conditions under which youth would follow or refuse their parents' orders. Ten students who attended a Midwestern land-grant institution were interviewed. Interviewees had experience working inside grain bins, were highly involved in their family farming operations while under 18 years old, and worked under frequent parental supervision. Interviewees shared conditions where they followed their parents' orders even when they knew the task was unsafe. Moreover, participants indicated when they would refuse orders from parents. Findings showed that the primary source of agricultural safety knowledge came from the interviewees' parents. Interviewees did not necessarily demonstrate a "blind trust" in their parents, as they were not willing to follow all orders they were asked to complete. However, all interviewees indicated that their parents would not ask them to do anything unsafe. Many barriers to farm safety were emphasized by the interviewees, which may contribute to youths choosing to make hazardous decisions regarding grain bin entry under some conditions. The research implications are emphasized relative to youth safety outcomes on family farms.Introduction Vector-borne diseases (VBDs) are a serious threat in many Indian states, including Kerala. Community-based decentralized planning and engagement are effective strategies that can make positive behavioral changes to control VBDs. Methods This community-based implementation research was conducted during November 2016 - October 2018 in Alappuzha municipality in Kerala, India. It was conducted in two phases. In the first phase, formative research was conducted to know the community's profile and perceptions and thus to plan and develop an appropriate intervention. Baseline data on some entomological indicators were also collected. These data were used to assess the impact of the intervention by comparing with the post-intervention data. In the second phase, an intervention through the community's engagement was implemented in selected wards. The activities included the formation of community committees and the vector control and source reduction activities with the community engagement and inter-sectoral coordination. Results The intervention resulted in a positive change among the community to engage in vector control activities. These efforts along with inter-sectoral coordination resulted in successfully implementing vector source reduction activities. In both wards, pre- and post-intervention entomological data (house index 16.7 vs 6.0 and 64.2 vs. 8.6; container index 24.8 vs. 12.1 and 37.7 vs. 18.1; and Breteau index 21.3 vs. 7.3 and 47.7 vs. 8.6) revealed a considerable vector source reduction. Conclusion The findings of this study suggest considering and including community engagement in public health policy as the main thrust to control VBDs.

This study evaluated whether the consumption of a cereal bar combining different phytoestrogens could contribute to the reduction of climacteric symptoms in women.

This is a clinical, prospective, randomized, simple-blind trial. Forty-eight women, aged 40-65 years, with climacteric symptoms, from a city in southwestern Paraná, Brazil. Participants were randomly assigned into two groups; Phytoestrogens group (PHY = 24), which received for 90-day period a cereal bar containing 80.73 milligrams of soybean and flaxseed phytoestrogens, and the placebo group (PLA = 24), which consumed rice flakes biscuit. Clinical, sociodemographic and anthropometric data were collected and climacteric symptoms were assessed using the Kupperman Index (KI).

Forty-three women were analyzed (PHY = 21 and PLA = 22). There were significant reductions in the overall KI score in both groups at the end of the intervention period (

 < 0.05). However, the comparison between the groups using linear regression models presented expressively better symptom improvement in the PHY group -6.
