Accurate assessment of uncontrolled BP was higher in the cardiology fellow clinic compared to the internal medicine resident clinic (29.1% vs 10.3%, P less then 0.001), even after adjusting for baseline characteristics differences between the 2 clinics. Multivariate regression analysis revealed that the type of clinic (internal medicine, odds ratio [OR] 0.27, 95% CI 0.16-0.47; P less then 0.001), established diagnosis of hypertension (OR 2.06, 95% CI 1.06-3.99; P less then 0.001), and SBP (OR 1.16 per mmHg, 95% CI 1.11-1.22; P=0.031) were independently associated with the primary outcome. Conclusion Cardiology fellows were better at identifying hypertension diagnosis thresholds and BP treatment goals in accordance with 2017 ACC/AHA guidelines compared to internal medicine residents. Background Opioid-induced androgen deficiency (OPIAD) related to chronic, long-acting opioid use can be a significant detriment to patient quality of life. The aim of this study was to investigate the association between chronic opioid use and hypogonadism. Methods A single-center, retrospective, matched case-control analysis of 357 males (94 cases, 263 controls, aiming for 14 matching) was performed. Study subjects were ages 18 to 80 years and had a diagnosis of chronic opioid use. Patients with a hypogonadism diagnosis were matched to patients without a hypogonadism diagnosis by age, ethnicity, and body mass index. The maximum morphine equivalent daily dose (MEDD) was compared in each group. Results A significant linear association between MEDD and the odds of developing hypogonadism (P less then 0.001) in males with chronic use of opioids was observed, with an odds ratio of 1.44 (95% CI 1.16-1.78) by 100-unit difference in maximum MEDD. Conclusion Results showed a significant, positive linear association between chronic opioid dose and the odds of developing hypogonadism in males. This higher index of suspicion of OPIAD could lead to earlier recognition of symptoms and increase the positive predictive value of diagnostic laboratory tests. Background Various studies have reported a decrease in emergency department admissions in correlation with major sporting events. However, these studies limit the investigation of selected variables to the 24-hour periods on game days. For our study, we assessed the impact of New Orleans Saints football team games on medical admissions on game days and also on post-game days. Methods We collected Saints game results from the 2013-2014 to 2019-2020 season and admissions data from 3 local hospitals during the same 7-year period. We compared the average daily census of game days vs non-game days as our control group using a 2-sample t test with unequal variances. Results The results demonstrate a statistically significant reduction in the average number of internal medicine admissions on game days (19.6%, P=0.0001). This reduction in admissions was not observed on post-game days (5.84%, P>0.05). On post-game days following a Saints victory, we found a statistically significant increase in the average number of admissions compared to when the team lost the previous day (18.2%, P=0.0105). Conclusion To our knowledge, the relationship between a recurring major sporting event and inpatient internal medicine admissions has not been previously quantified. Our study of the impact of the New Orleans Saints football games on internal medicine admissions provides insights into the relationship between social sporting events and health outcomes in the city. While our results may not be directly generalizable to other locations-particularly those that are not in a National Football League team market-we showed that during a 7-year period, Saints football games correlated with a 19.6% decrease in internal medicine admissions on the day of the game. This finding suggests that Saints games may serve as a protective factor associated with decreased negative health outcomes among residents in the city. Background Postoperative wound pain is commonly observed in the pediatric postanesthesia care unit (PACU) following tonsillectomy, adenoidectomy, and adenotonsillectomy (adeno/tonsillectomy), which contributes to increased medical care costs and delayed facility discharge. The purpose of this study was to review the benefits of preoperative administration of Hycet elixir (2.5 mg hydrocodone and 108 mg acetaminophen per 5 mL) in a pediatric population aged 1 to 9 years following adeno/tonsillectomy. Methods Patient demographics, comorbidities, surgical and anesthetic times, need for postoperative rescue therapies, and PACU recovery and length of stay times were measured in pediatric patients who received preoperative administration of Hycet elixir (0.2 mg/kg hydrocodone) for adeno/tonsillectomy in an outpatient setting compared to a control group. Results The Hycet elixir group had significant reductions in PACU and hospital lengths of stay and significant reductions in the need for postoperative rescue analgesics. No significant differences were observed in emergence times or in the incidences of unplanned hospital admission between the control and Hycet elixir groups. Conclusion These data show that the preoperative administration of Hycet elixir is well tolerated in the pediatric patient population undergoing adeno/tonsillectomy and appears to significantly reduce the need for postoperative rescue analgesics and postoperative care times. These data support the use of preoperative administration of Hycet elixir in this patient population.Mobile map applications are typically used by a broad range of users. Users can be diverse in their context attributes (e.g. map use experience, activities during map use), and several previous user experience (UX) studies have focused on understanding how some contextual factors influence the UX for designing maps that satisfy users' needs. A need for research remains to evaluate the relationship between user context, UX, and variants of mobile map element design. In this article, we present our research investigating the interplay of these factors through an empirical user study with citizens in Austria. Oleic We created an online survey and generated 84 map variations, combining seven map-related tasks, three base map styles, two map detail densities, and two time-pressure variants. We tested these map variations with 107 survey participants and related their UX to user context. Map-related tasks emerged as a dominant factor modifying the map design UX. Further results showed that interactivity loaded map-related tasks were aided when paired with low detail-dense base maps, contrasting overlay features. We recommend future research to analyze an extended set of context attributes with additional participant data, to evaluate dynamic variations in context, and to find ways to dynamically monitor mobile map design UX.As a material having high rubber content, latex reclaim (white reclaim) has been used in the production of premium grade rubber products like tyres, retreading materials, etc. Introduction of latex reclaim (LR) is also an ideal method to reduce the cost of rubber products. In the present work, natural rubber (NR), butadiene rubber (BR), and latex reclaim (LR) combinations were prepared to develop cost efficient tread materials. LR was mixed with NR/BR at various proportions to produce tread materials which will comply with national specifications. The blends were prepared and the cure and mechanical properties were investigated. Results have indicated that the scorch time and cure time had decreased with the increase of reclaim loading. The mechanical properties like tensile strength, tear strength, abrasion resistance decrease with the increase in the LR content. It was found that 70-80% of the mechanical properties were retained even after addition of 30 phr of LR. The thermal behaviour and activation energy of NR/BR/LR system was investigated using thermogravimetry TGA analysis and increased activation energy showed that the thermal stability has increased when the amount of LR is high. SEM studies had indicated the morphology change due to the viscosity mismatch between NR and BR especially in the presence of LR.The subjective life satisfaction of individuals reveals valuable information about the overall well-being of a society. Furthermore, large international migration flows have led to the importance of studying the life satisfaction of immigrants within host countries, including Canada. This study uses secondary data from the 2013 Canadian General Social Survey, Cycle 27, to assess the life satisfaction of immigrants in Canada compared to Canadian-born individuals, and to determine the overall impact of immigrants' year of arrival. Using bivariate and multivariate statistical models, the results reveal that the life satisfaction of immigrants does not significantly differ from the Canadian-born population, and that year of arrival has no significant effect. Income, however, seems to be a more important determinant of immigrants' life satisfaction in Canada. The ongoing COVID-19 emergency clearly presents novel challenges, both in terms of difficulties for maintaining public health and in assuring that governmental responses are ethically sound. Centrally, responses must respect, as best as possible, fundamental human rights and human values. Conflicts among values arise in response to the crisis, and public officials have no choice but to prioritize some while sacrificing others. Utilizing the concepts of effectiveness and legitimacy within the framework of post-normal science (PNS), we investigate and recommend processes and measures to address COVID-19 that support increased public health, while upholding established rights and values. The effectiveness and legitimacy of science-led policymaking requires investigation of how that policy ought to be made (e.g. concepts of policymaking and PNS), as well as how it ought to interact with diversely-constituted publics (e.g. public inclusion in policymaking and policy communication).A novel heterogeneous catalytic nanomaterial based on a molybdenum cluster-based halide (MC) and a single-layered copper hydroxynitrate (CHN) was first prepared by colloidal processing under ambient conditions. The results of the elemental composition and crystalline pattern indicated that CHN was comprehensively synthesized with the support of the MC compound. The absorbing characteristic in the ultraviolet and near-infrared regions was promoted by both of the ingredients. The proper chemical interaction between the materials is a crucial reason to modify the structure of the MCs and only a small decrease in the magnetic susceptibility of CHN. The heterogeneous catalytic activity of the obtained MC@CHN material was found to have a high efficiency and excellent reuse when it is activated by hydrogen peroxide (H2O2) for the degrading reaction of the organic pollutant at room temperature. A reasonable catalytic mechanism was proposed to explain the distinct role of the copper compound, Mo6 compound, and H2O2 in the production of the radical hydroxyl ion. This novel nanomaterial will be an environmentally promising candidate for dye removal.Asthma is a mysterious disease with heterogeneity in etiology, pathogenesis, and clinical phenotypes. Although ongoing studies have provided a better understanding of asthma, its natural history, progression, pathogenesis, diversified phenotypes, and even the exact epigenetic linkage between childhood asthma and adult-onset/old age asthma remain elusive in many aspects. Asthma heritability has been established through genetic studies, but genetics is not the only influencing factor in asthma. The increasing incidence and some unsolved queries suggest that there may be other elements related to asthma heredity. Epigenetic mechanisms link genetic and environmental factors with developmental trajectories in asthma. This review provides an overview of asthma epigenetics and its components, including several epigenetic studies on asthma, and discusses the epigenetic linkage between childhood asthma and adult-onset/old age asthma. Studies involving asthma epigenetics present valuable novel approaches to solve issues related to asthma.
