Both pembrolizumab and atezolizumab have showed to improve OS compared to chemotherapy in unselected patients (HR 0.86, 95% CI 0.80-0.93, P = .0001, I2 = 60%), while the difference was not significant in PD-L1 selected patients (HR 0.91, 95% CI 0.77-1.07, P = .23, I2 = 0%). The various anthropogenic sources of salts and their locations in a watershed may result in secondary salinization of river and stream networks through multiple inputs. We developed a watershed predictive assessment to investigate the degree to which topology, land-cover, and land-use covariates affect stream specific conductivity (SC), a measure of salinity. We used spatial stream network models to predict SC throughout an Appalachian stream network in a watershed affected by surface coal mining. During high-discharge conditions, 8 to 44% of stream km in the watershed exceeded the SC benchmark of 300 μS/cm, which is meant to be protective of aquatic life in the Central Appalachian ecoregion. During low-discharge conditions, 96 to 100% of stream km exceeded the benchmark. The 2 different discharge conditions altered the spatial dependency of SC among the stream monitoring sites. During most low discharges, SC was a function of upstream-to-downstream network distances, or flow-connected distances, among the sites. Flow-connected distances are indicative of upstream dependencies affecting stream SC. During high discharge, SC was related to both flow-connected distances and flow-unconnected distances (i.e., distances between sites on different branches of the network). Flow-unconnected distances are indicative of processes on adjacent branches and their catchments affecting stream SC. With sites distributed from headwaters to the watershed outlet, the extent of impacts from secondary salinization could be better spatially predicted and assessed with spatial stream network models than with models assuming spatial independence. Importantly, the assessment also recognized the multi-scale spatial relationships that can occur between the landscape and stream network.

The NdYAG laser has emerged as a promising modality for the management of nail psoriasis owing to its ability for deep penetration of the skin surface, which has the advantage of destroying deep vessels.

To assess the efficacy and safety of NdYAG laser in treating nail psoriasis.

The present study was a randomized controlled study, conducted on 20 patients of both sexes (age older than 12 years) with mild to moderate psoriasis with nail involvement. We found no statistically significant difference in total Nail Psoriasis Severity Index (NAPSI) and nail bed scores before and after treatment among the treated group. link2 However, there was statistically significant improvement in nail matrix score after treatment. Surveys of practitioners working with school psychology trainees are limited as discussed by Lewis et al. (2005). The purpose of this exploratory study is to seek to understand the perceptions of specialist-level site-based supervisors in relation to practicum students' knowledge in several competency areas. Utilizing the National Association of School Psychologists' Model for Comprehensive and Integrated School Psychological Services and Standards for Graduate Preparation of School Psychologists as reported by NASP (2020b) and commonly reported school psychology practices as discussed by McNamara et al. (2019), a survey was developed and administered to specialist-level practicum supervisors. Results of Chronbach's alpha analyses examining internal consistency for each of the seven domains ranged from good to excellent (0.85-0.95) although mean differences in domain ratings were not found to be significant. Results of the study are presented along with study limitations and directions for future research.Objectives  The aim of the study is to synthesize the evidence and evaluate the total cell-free deoxyribonucleic (cfDNA) associated with the prediction of preeclampsia (PE). Total cfDNA is constituted by both cell-free fetal DNA (cffDNA) originated mainly from the placenta, and maternal cfDNA derived from maternal leukocytes. Methods  A systematic review was conducted by searching PubMed and Medline. Literature reporting levels of total cfDNA in the development of PE was included. Studies that only reported cffDNA, but no cfDNA concentrations were not included in this review. Results  Eight studies were included. Seven reported values of cfDNA in PE patients, regardless of early or late onset PE, six of which demonstrated a significant increase of cfDNA in patients who subsequently developed PE. Seven studies evaluated cfDNA levels in the first trimester, six of which showed significant increase of cfDNA concentrations in women who later developed PE. Five studies investigated cfDNA levels in the second trimester, all presenting increased total cfDNA levels in the PE group compared with normal controls. Conclusion  Total cfDNA may play a role as a biochemical marker of PE, compared with fetal cfDNA. Large prospective studies with homogeneous populations and standardized methodology are needed to further confirm its predictive value.There are no program evaluation approaches designed for a crisis, such as the COVID-19 pandemic. There was no consensus for the value of similar instructional/assessment adaptations. link3 The evaluation model can be adapted by other medical schools to fit systems-based needs.This paper proposes a new dimension reduction algorithm based on low-dimension ordered orthogonal projection (LOOP), which is used for cancer detection with terahertz (THz) images of freshly excised human breast cancer tissues. A THz image can be represented by a data cube with each pixel containing a high dimension spectrum vector covering several THz frequencies, where each frequency represents a different dimension in the vector. The proposed algorithm projects the high-dimension spectrum vector of each pixel within the THz image into a low-dimension subspace that contains the majority of the unique features embedded in the image. The low-dimension subspace is constructed by sequentially identifying its orthonormal basis vectors, such that each newly chosen basis vector represents the most unique information not contained by existing basis vectors. A multivariate Gaussian mixture model is used to represent the statistical distributions of the low-dimension feature vectors obtained from the proposed dimension reduction algorithm. The model parameters are iteratively learned by using unsupervised learning methods such as Markov chain Monte Carlo or expectation maximization, and the results are used to classify the various regions within a tumor sample. Experiment results demonstrate that the proposed method achieves apparent performance improvement in human breast cancer tissue over existing approaches such as one-dimension Markov chain Monte Carlo. The results confirm that the dimension reduction algorithm presented in this paper is a promising technique for breast cancer detection with THz images, and the classification results present a good correlation with respect to the histopathology results of the analyzed samples.

The aim of this retrospective study was to evaluate the outcomes of older patients with 2-part proximal humerus fractures (PHFs) with medial column disruption stabilized using a proximal humeral internal locking system (PHILOS) plate plus oblique insertion of autologous fibula as a primary procedure.

Data involving 112 patients (112 shoulders) sustaining 2-part PHFs with medial column disruption treated with PHILOS plate plus oblique insertion of autologous fibula as a primary procedure during 2012-2019 were identified. The median follow-up was 36 months (range 11.2-43.5 months). The primary endpoint was the Constant scores and American Shoulder and Elbow Surgeons (ASES) scores. The secondary endpoint was the main orthopedic complication rate.

The median Constant and ASES scores were 78 (range, 52-95) and 77 (range, 62-96) at the final follow-up, respectively. The main orthopedic complication rate was 10.7% (12/112). Twelve orthopedic complications in 8 patients were detected, and they involved loss of reduction, varus collapse, aseptic loosening, mal-union, revision, and intolerable shoulder pain. Of these complications, 3 (2.6%) involved loss of reduction, 2 (1.7%) involved varus collapse, 3 (2.6%) involved aseptic loosening, 1 (0.8%) involved mal-union, 2 (1.7%) required revision surgery, and 1 (0.8%) presented intolerable shoulder pain.
