Unlike in the cat, the sartorius muscle receives branches from the saphenous (N. saphenous) and femoral nerves (N. femoris), and the lateral head of the gastrocnemius and superficial digital flexor muscles are innervated by a branch of the soleus muscle.Purpose Development and validation of a non-invasive imaging system to capture spontaneous blinking and characterise blink dynamics using a custom image analysis algorithm. Methods A pilot study investigated the influence of illumination type on blink dynamics. Spontaneous blinking was recorded in eight subjects using a high-speed camera under two illumination conditions white light and infrared. Custom semi-automated software assessed palpebral aperture height (PAH), blink rate, blink speed, blink completeness and blink duration. The main clinical study compared two different image analysis techniques. Spontaneous blinking was recorded in 20 subjects using a high-speed infrared camera. Blink speed and duration were determined using two techniques manual tracking and semi-automated tracking. Agreement between the two techniques was assessed using Bland-Altman analysis. Coefficients of repeatability (COR) were calculated for the semi-automated technique. Results There were significant differences between white light and infrared illumination for PAH (p less then 0.0001), blink rate (p = 0.04), closing speed (p = 0.009), opening speed (p less then 0.0001) and blink duration (p = 0.0003). The mean differences (95% limits of agreement) between the two techniques were 0.6 mm s-1 (-15.9 to 17.0) closing speed, 1.5 mm s-1 (-6.8 to 9.8) opening speed, 2.4 ms (-6.8 to 11.5) closed phase duration and 5.0 ms (-19.4 to 29.3) total blink duration. COR values were 10.1 mm s-1 closing speed, 3.7 mm s-1 opening speed, 6.7 ms closed phase duration and 11.2 ms total blink duration. Conclusions This study has shown that spontaneous blinking can be characterised using a non-invasive imaging system. The semi-automated analysis provides a rapid characterisation of blink dynamics, allowing its application in large-scale trials in a number of clinical areas.Mammalian reproductive success depends on gonadotrophin-releasing hormone (GnRH) neurones to stimulate gonadotrophin secretion from the anterior pituitary and activate gonadal steroidogenesis and gametogenesis. Genetic screening studies in patients diagnosed with Kallmann syndrome (KS), a congenital form of hypogonadotrophic hypogonadism (CHH), identified several causal mutations, including those in the fibroblast growth factor (FGF) system. This signalling pathway regulates neuroendocrine progenitor cell proliferation, fate specification and cell survival. Indeed, the GnRH neurone system was absent or abrogated in transgenic mice with reduced (ie, hypomorphic) Fgf8 and/or Fgf receptor (Fgfr) 1 expression, respectively. Moreover, we found that GnRH neurones were absent in the embryonic olfactory placode of Fgf8 hypomorphic mice, the putative birthplace of GnRH neurones. These observations, together with those made in human KS/CHH patients, indicate that the FGF8/FGFR1 signalling system is a requirement for the ontogenesis of the GnRH neuronal system and function. In this review, we discuss how epigenetic factors control the expression of genes such as Fgf8 that are known to be critical for GnRH neurone ontogenesis, fate specification, and the pathogenesis of KS/CHH.Four pivotal randomized placebo-controlled trials have demonstrated that adjunctive therapy with cannabidiol (CBD) improves seizure control in patients with Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS). Between 47% and 68% of patients allocated to CBD treatment in these trials were receiving clobazam (CLB), which shows complex interactions with CBD resulting, in particular, in a 3.4- to 5-fold increase in plasma concentration of the active metabolite norclobazam. This raises concern as to the role played by these interactions in determining the reduction in seizure frequency in CBD-treated patients, and the question of whether CBD per se has clinically evident antiseizure effects. We appraised available evidence on the clinical consequences of the CBD-CLB interaction, focusing on subgroup analyses of seizure outcomes in patients on and off CLB comedication in the pivotal CBD trials, as provided by the European Medicines Agency Public Assessment Report. Evaluation of the results of individual trials clearly showed that improvement in seizure control over placebo was greater when CBD was added on to CLB than when it was added on to other medications. However, seizure control was also improved in patients off CLB, and despite the small sample size the difference vs placebo was statistically significant for the 10 mg/kg/d dose in one of the two LGS trials. Stronger evidence for an antiseizure effect of CBD independent of an interaction with CLB emerges from meta-analyses of seizure outcomes in the pooled population of LGS and DS patients not receiving CLB comedication. Although these results need to be interpreted taking into account methodological limitations, they provide the best clinical evidence to date that CBD exerts therapeutic effects in patients with epilepsy that are independent of its interaction with CLB. Target compounds were screened for their anticancer activity according to the NCI-USA protocol toward a panel of 60 cancer cell lines. Compounds 12a and 12b displayed the best antiproliferation activity against many cell lines. An effective response to domestic violence in India needs to be transformative in nature, supporting the empowerment of survivors of violence and empowering communities to support survivors. Various studies have highlighted the importance of settings like grassroots organizations for promoting empowerment of members through community organizing. This qualitative study builds on this growing body of research using a grassroots agency in India as an exemplar to understand (a) empowering processes at the individual and community level associated with the agency's community organizing efforts; and (b) salient mechanisms associated with social change and action in the response to domestic violence in the community. At the individual level, the agency's organizing efforts provided knowledge, skills and resources, and opportunities for participation and leadership. At the community level, the agency engages in capacity building and increases social capital of members. The mechanisms that emerged as salient were increased critical consciousness of members, an emphasis on breaking the silence around domestic violence, community trust and cohesion, and capacity for informal social control. Information on how to translate the obtained score into extent, severity and impact strata (mild-moderate-severe) is still lacking. Stratification is helpful to define inclusion criteria for trials, enables comparison and pooling of trial results and can be used for epidemiological research. Objectives The aim was to develop extent, severity and impact strata for the SA-VES based on validated anchor based questions. Methods In total, 316 patients with vitiligo (non-segmental; age ≥16) recruited at the Ghent University Hospital (Belgium) completed a questionnaire that was conducted in cooperation with the Dutch Society for Vitiligo patients to ensure content validity. First 3 anchor questions included in the questionnaire [Patient Global Assessment (PtGA) for vitiligo extent, severity and impact] were assessed for content validity, construct validity and intrarater reliability. Subsequently, the PtGAs were used to stratify the SA-VES based on ROC analysis. Results For all PtGAs (PtGA extent, PtGA severity, PtGA impact) at least 75% of hypotheses evaluated for construct validity were confirmed. Intrarater reliability of all PtGAs were good to excellent (ICCs PtGAPtGA extent 0.623; PtGA severity 0.828; PtGA impact 0.851). The optimal cut-off values of the SAVES between the 3 global categories (mild/limited - moderate - severe/extensive) were 1.05% and 6.45% based on PtGA extent, 2.07% and 4.8% based on PtGA severity and 2% and 3.35% based on PtGA impact. Conclusion This study provides the first guide for the interpretation of the numerical output obtained by the SA-VES (vitiligo extent) and enables the translation into a global vitiligo grading for extent, severity and impact. As patients' interpretation of vitiligo extent, severity and impact may vary amongst patients worldwide, future international studies will be required.
