Paraventricular hypothalamus (PVH) is demonstrated to regulate stress, feeding behaviors, and other related homeostatic processes. However, no direct evidence has been investigated for the role of PVH in swallowing function. Acupuncture therapy at Lianquan (CV23) acupoint has been reported to improve the swallowing function in clinical trials, but its underlying mechanism still needs to be uncovered. Thus, we aimed to explore whether PVH involved the acupuncture mediated regulating swallowing function. Chemogenetics, electromyography (EMG) recording, and immunofluorescence staining methods were combined to demonstrate that neurons in PVH could be activated by electroacupuncture (EA) stimulation at CV23, and this neuronal cluster was represented as excitatory neurons. Furthermore, we mapped both the inputs and outputs of PVH neurons using viral tracing. The neurons in PVH projected with the brain regions, including parabrachial nucleus (PBN) and the solitary tract nucleus (NTS), which both participated in the swallowing process. The EA function regulating the swallowing was attenuated after inhibiting the neurons in PVH in the post stroke dysphagia. In conclusion, this study suggested that EA at CV23 could regulate swallowing function involving the excitatory neurons in PVH.Ornate dog tick, Dermacentor reticulatus is an important vector of Babesia canis, and Rickettsia spp. and other pathogens of veterinary and public health interest. The current study is the first to investigate the long-term changes in prevalence of these pathogens in expanding tick populations in Central Europe. Molecular techniques (PCR, sequencing) were applied for the detection of pathogen DNA in adult (n = 2497) and juvenile ticks (1096 larvae and 410 nymphs). DNA of Rickettsia spp. was identified in 35% of adults and 12.6% of juvenile ticks. DNA of B. canis was detected in 3% of adult ticks and only in ticks from the Eastern region (regional prevalence 6%). As previously, no B. canis-positive ticks were found in Western Poland, including ticks from Wrocław area (n = 298). DNA of B. canis was identified in 0.33% of juvenile ticks (in 3 pools of larvae and 2 nymphs) from the Eastern region. In the current study we confirmed high occurrence of R. raoultii in adults ticks from all four zones and relatively high prevalence of B. canis in the Eastern population of D. reticulatus, corresponding well with high incidence of canine babesiosis in this area of Poland. Finally, we confirmed R. raoultii and B. canis infection in all life stages of D. reticulatus ticks.To examine medical students' perceptions of leadership and explore their implications for medical leadership education. We conducted a qualitative analysis of the essays submitted by students in the medical leadership course from 2015 to 2019. We categorised the essays by the characteristics of the selected model leaders (N = 563) and types of leadership (N = 605). A statistically significant proportion of students selected leaders who were of the same gender as themselves (P less then 0.001), graduate track students chose leaders in science (P = 0.005), while; military track students chose leaders in the military (P less then 0.001). Although the highest proportion of students chose politicians as their model leaders (22.7%), this number decreased over time (P less then 0.001), and a wider range of occupational groups were represented between 2015 and 2019. Charismatic leadership was the most frequently selected (31.9%), and over time there was a statistically significant (P = 0.004) increase in the selection of transformational leadership. Students tended to choose individuals whose acts of leadership could be seen and applied. Medical leadership education should account for students' changing perceptions and present a feasible leadership model, introducing specific examples to illustrate these leadership skills.The Active Pulley Hypothesis (APH) is based on modern functional anatomical descriptions of the oculomotor plant, and postulates behaviors of the orbital pulleys proposed to be positioned by the extraocular muscles (EOMs). A computational model is needed to understand this schema quantitatively. We developed and evaluated a novel biomechanical model of active horizontal rectus pulleys. The orbital (OL) and global (GL) layers of the horizontal rectus EOMs were implemented as separate musculoskeletal strands. Pulley sleeves were modeled as tube-like structures receiving the OL insertion and suspended by elastic strands. Stiffnesses and orientations of pulley suspensions were determined empirically to limit horizontal rectus EOM side-slip while allowing anteroposterior pulley travel. Independent neural drives of the OL greater than GL were assumed. The model was iteratively refined in secondary gazes to implement realistic behavior using the simplest mechanical configuration and neural control strategy. Simulated horizontal rectus EOM paths and pulley positions during secondary gazes were consistent with published MRI measurements. Estimated EOM tensions were consistent with the range of experimentally measured tensions. This model is consistent with postulated bilaminar activity of the EOMs, and the separate roles of the GL in ocular rotation, and OL in pulley positioning.Diagnosis of chronic disease in a child can result in unresolved grief (UG) in parents. This study aimed to evaluate the efficacy of psychological insight-oriented therapy (IOT) as a treatment for UG compared to disease related education in parents of children with cystic fibrosis (CF). Sequence of delivery, first IOT then disease related education (or vice versa) was also examined, to let all participants experience both interventions. Parents were screened for UG. Parents with UG were randomised to either five 1-h sessions of IOT or five 1-h sessions of education. Measures were assessed pre-intervention, after the first intervention period (primary efficacy assessment), and after the second intervention period (swapping intervention). Forty-seven parents were screened of which 46.8% (22/47) had UG. Median duration of UG was 5 years (range 6 months-14 years). Anxiety (50% vs. 20%, p = 0.03) and stress (59% vs. 28%, p = 0.03) were significantly more prevalent in parents with UG. There was no difference between arms in the odds of UG resolving either following the first intervention period (OR 0.88; 95% CI 0.5, 1.5) or the second intervention period (OR 0.91; 95% CI 0.5, 1.6). While not statistically significant, adjusted mean values for seven of the eight mental health measures were lower in the IOT (first) arm compared to the ED (first) arm, following the first intervention period. UG is a significant burden for families affected by CF. Provision of disease related education and psychological support, regardless of sequence, can result in resolution of grief.Trial registration number ACTRN12621000796886, date of registration 24/06/2021, retrospectively registered.Consuming wildmeat may protect against iron-deficiency anemia, a serious public health problem globally. Contributing to debates on the linkages between wildmeat and the health of forest-proximate people, we investigate whether wildmeat consumption is associated with hemoglobin concentration in rural and urban children ( less then 5 years old) in central Brazilian Amazonia. Because dietary practices mediate the potential nutritional benefits of wildmeat, we also examined whether its introduction into children's diets is influenced by rural/urban location or household socio-economic characteristics. Sampling 610 children, we found that wildmeat consumption is associated with higher hemoglobin concentration among the rural children most vulnerable to poverty, but not in the least vulnerable rural, or urban children. Rural caregivers share wildmeat with children earlier-in-life than urban caregivers, potentially because of cultural differences, lower access to domesticated meat, and higher wildmeat consumption by rural households (four times the urban average). If wildmeat becomes unavailable through stricter regulations or over-harvesting, we predict a ~ 10% increased prevalence of anemia among extremely poor rural children. This modest protective effect indicates that ensuring wildmeat access is, alone, insufficient to control anemia. Sustainable wildlife management could enhance the nutritional benefits of wildlife for vulnerable Amazonians, but reducing multidimensional poverty and improving access to quality healthcare are paramount.Lake Urmia, the twentieth largest lake in the world, is the most valuable aquatic ecosystem in Iran. The lake water level has decreased in recent years due to human activities and climate change. this website Several studies have highlighted the significant roles of climatic and anthropogenic factors on the shrinkage of the lake. Management policies for water resources harvesting must be adopted to adapt to climate change and avoid the consequent problems stemming from the drought affecting Lake Urmia, and rationing must be applied to the upstream water demands. This study analyzes strategies and evaluates their effectiveness in overcoming the Urmia Lake crisis. Specifically, system dynamics analysis was performed for simulating the water volume of Lake Urmia, and the Hadley Centre coupled model was applied to project surface temperature and precipitation for two future periods 2021-2050 and 2051-2080. Six management scenarios were considered for decreasing the allocation of agricultural water demand corresponding to two options (1) one-reservoir option (Bukan reservoir only), and (2) six-reservoir option. The net inflow of Urmia Lake was simulated for the two future periods with the IHACRES model and with artificial neural network models under the six management scenarios. The annual average volumes of Lake Urmia would be 30 × 109 and 12 × 109 m3 over the first and second future periods, respectively, without considering the management scenarios. The lake volumes would rise by about 50% and 75% for the first and second periods, respectively under the management scenarios that involve strict protective measures and elimination of the effect of all dams and their reservoirs. Implementing strict measures would increase the annual average lake volume to 21 × 109 m3 in the second period; yet, this volume would be less than the long-term average and strategic volume. The human water use would be completely eliminated under Scenario 6. Nevertheless, Lake Urmia would experience a considerable loss of storage because of drought.Dense data acquisition for 3-D high-resolution aquifer mapping through heliborne transient electromagnetic (HTEM) survey is continually not possible due to various technical and administrative constraints. Consequently, we apply ground geophysical surveys at possibly closer spacing to collect the sub-surface information in the no-fly area, which provides only a regional aquifer picture. In the area near Patna of Northern India, an extent of 18% was covered under the HTEM survey, and the rest was surveyed by ground geophysical methods. Both data are integrated using the theory of regionalized variables. The parameters of multi-aquifers i.e., top of the first aquifer, top of the separating clay layer, top and the bottom of second aquifer, are estimated together with their respective resistivities. The estimations are made at an interval of 250 m, practically an appropriate distance at which dense data generation was carried out using the HTEM survey. The integrated approach generates the data in the no-fly area with the same spatial density as the flown area.
