Incidentally discovered adrenal masses are common and mostly benign and non-functioning adenomas. However, evolving evidence suggests that a notable proportion of these adrenal adenomas may demonstrate mild autonomous cortisol secretion (MACS), which has been associated with an increased risk for hypertension, hyperglycemia, obesity, dyslipidemia, vertebral fractures, adverse cardiovascular events, and mortality. To determine the proportion of cardiac rehabilitation programs (CRPs) in Canada that accept referrals for individuals with peripheral artery disease (PAD), eligibility criteria, and barriers/facilitators to inclusion.

CRPs across Canada were sent a web-based questionnaire.

Of 180 questionnaires sent, 98 CRP managers representing 114 CRPs (62.6% of CRPs in Canada) responded. Of respondents, 81.6% accepted referrals for people with PAD; however 44.6% reported that ≤10 patients participated in the previous calendar year; two CRPs had no participants. Of CRPs accepting PAD, 23.7% accepted patients only with coexisting cardiac disease, 68.4% accepted post-lower limb amputees with prosthesis and 53.9% without prosthesis (non-ambulatory). Further, 32.2% did not provide formal/informal PAD-specific education to patients and only 14.3% provided education to staff regarding PAD in the previous 3 years. Three (3) numerical pain scales were used to guide exercise intensity. Within these scales up to four pain thresnd information/brochures on benefits of CRPs for patients and referral sources should improve participation and delivery of secondary prevention strategies.

Uncertainties regarding the 3D ridge morphology of the posterior mandible can greatly increase the risk of surgical complications during dental implant placement. By using cone beam computed tomography (CBCT) imaging and a computer-guided implant treatment software program before any invasive procedure, it is possible to assess ridge morphology and understand the surgical complexity and risk level.

Three-dimensionally printed custom open trays have become a popular option in clinical implant dentistry because of advantages such as individualization, efficiency, and effectiveness. However, clinical evidence on their accuracy and feasibility is lacking.

The purpose of this clinical study was to evaluate the accuracy and feasibility for impressions of multiple implants by using 3D-printed custom open trays versus conventional custom open trays.

Twenty-two partially edentulous individuals needing impression making for restorations supported by multiple implants were enrolled. Two types of custom open trays were made for each participant, a 3D-printed tray (test) and a conventional tray (control). With a splinted technique, silicone definitive impressions were obtained with the 2 custom open trays and poured with Type IV dental stone. Impression accuracy (primary outcome) was evaluated by measuring linear distances and the marginal gaps between the implant replicas and verification devices on the test ultiple implants.

Within the limitations of this study, the 3D-printed custom open trays were clinically accurate, efficient, and cost-effective for impressions of multiple implants.

The Public Health Center (PHC)-known as hokenjo in Japan-assume a crucial role in disease control. Coronavirus disease 2019 (COVID-19) is one of many designated infectious diseases monitored by the agency. During the present pandemic, patients who suspected COVID-19 were instructed to call the Coronavirus Consultation Center in the PHC prior to visiting the hospital. The aim of this study was to elucidate the differences in polymerase chain reaction (PCR) positivity between PHC referrals and direct walk-in patients.

The present was a single-center, retrospective cohort study conducted at the Tokyo Metropolitan Hospital from March to September, 2020. Patients who received a PCR test for SARS-CoV-2 were included and categorized into the PHC referral or direct walk-in groups. The outcomes included the total number of patients undergoing PCR tests and the percentage of PCR positivity in each group.

We identified 1680 patients (781 PHC referred and 899 direct walk-in groups). The percentage of PCR positivity did not significantly differ between the PHC referral and direct walk-in groups during the first wave (30.5% vs. 29.2%; p=0.78). PCR positivity was significantly higher in the PHC referral group than the direct walk-in group during the second wave (30.1% vs. 23.1%; p=0.051) and entire study period (30.2% vs. 24.7%; p=0.011).

Primary care practices employing nurse practitioners (NPs) can play an important role in improving access to high quality health care services. However, most studies on the NP role in health care use administrative data, which have many limitations.

In this paper, we report the methods of the largest survey of primary care NPs to date.

To overcome the limitations of administrative data, we fielded a cross-sectional, mixed-mode (mail/online) survey of primary care NPs in six states to collect data directly from NPs on their clinical roles and practice environments.

While we were able to collect data from over 1,200 NPs, we encountered several challenges with our sampling frame, including provider turnover and challenges with identification of NP specialty.

In this revision, attention was placed on the importance of certified nursing assistants (CNAs) to resident care and the need to empower these frontline workers. Phase II mandates that CNAs be included as members of the nursing home interdisciplinary team that develops care plans for the resident that are person-centered and comprehensive and reviews and revises these care plans after each resident assessment. While these efforts are laudable, there are no direct guidelines for how to integrate CNAs in the interdisciplinary team. Wild boar populations can act as reservoirs of numerous infectious agents that affect pig livestock, including PCMV. The aim of this work was to assess the circulation of this virus in free-living wild boars that inhabit Northeastern Patagonia (Buenos Aires and Río Negro Provinces), Argentina. Nested-PCR assays were conducted to evaluate the presence of PCMV in samples of tonsil tissue collected from 62 wild boar individuals. The adaptability of 31 strains of lactic bacteria (LB) in an oats/water system (OWS/SAA) was evaluated, measuring growth, acidification and fermentation activity (impedimetric method; detection time [DT], maximum rate of conductance change [MRCC/VMCC] and percentage of conductance change [PCC]). Moreover, the content of phenolic compounds (PC) was determined using the Folin-Ciocalteu method (gallic acid equivalents [GAE]), free peptides and amino acids and free radical scavenging activity (DPPH• and ABTS•+ methods) of methanolic and aqueous extracts obtained from fermented OWS/SAAs (fOWS/SAAf) were determined. Six strains have shown the best adaptability to SAA, with high values of VMCC (0.34-0.47 μS/min) and PCC (53.6-66.6%), and low values of DT (≤ 3 h). In these f/OWS/SAAf the chemical composition was also modified (PC concentration, peptides and free amino acids) with strain-dependent behavior. The PC content in f/OWS/SAAf using these six strains (29.1-36.9 μg GAE/ml) was higher than the control content in OWS/SAA control (17.1 ± 1.9 μg GAE/ml). An increase (9-25.5%) in antioxidant activity in f/OWS/SAAf methanolic extracts was detected using both methods. Minor modifications were observed in the peptide and free amino acid content of SAA and their antioxidant activity. Our results show LB ability to adapt to oat as fermentation substrate and increase the content of its antioxidant compounds.
