Objectives.To evaluate the tolerability and efficacy of Dermatophagoides pteronyssinus /Dermatophagoides farinae mixture subcutaneous immunotherapy (SCIT). Methods.Patients received an abbreviated build-up schedule. The aims were number, percentage, and severity of adverse reactions. Secondary outcomes included changes in immunoglobulin titers and changes in dose-response skin prick tests. Results.Out of 289 administrations, 17% elicited any clinically relevant adverse reaction. Most of them were local reactions (LR) (9.4%) and the rest (7.6%) were systemic. Significant increases in sIgG and sIgG4 were detected in serum samples. Cutaneous reactivity decreased significantly. Abstract Les Theileria bovins sont des parasites protozoaires transmis par les tiques qui envahissent les érythrocytes et les lymphocytes des bovins. Trois espèces principales de Theileria de bovins ont été identifiées en Chine, T. selleck chemicals llc orientalis, T. sinensis et T. annulata. Pour examiner la prévalence de la theilériose bovine à Yanbian, un total de 584 échantillons de sang bovin ont été collectés dans cinq localités de 2017 à 2019 et analysés par PCR. Six paires d’amorces oligonucléotidiques dirigées contre le gène d’ARNr 18S de Theileria spp., le gène Tams-1 de T. annulata et le gène MPSP de T. orientalis et T. sinensis, ont été utilisées pour détecter ces parasites. Une analyse de séquence des gènes amplifiés a confirmé que les espèces de Theileria étaient T. orientalis et T. sinensis, sans T. annulata. La prévalence globale des Theileria chez les bovins était de 42,81 % (250/584). Sur les 584 échantillons, 159 (27,23 %) et 157 (26,88 %) étaient positifs pour T. sinensis et T. orientalis, respectivement, et le taux d’infection mixte était de 11,30 % (66/584). La prévalence totale des espèces bovines de Theileria à Helong, Hunchun, Longjing, Yanji et Dunhua était respectivement de 66,28 %, 49,68 %, 23,81 %, 28,15 % et 0 %. Ces résultats fournissent des données épidémiologiques pour la prévention et le contrôle des espèces de Theileria de bovins à Yanbian, en Chine.INTRODUCTION Post-chemotherapy retroperitoneal lymph node dissection (PCRPLND) has a well-established role in the management of residual retroperitoneal masses >1cm in patients with advanced non-seminomatous germ cell tumor (NSGCT). Herein, we report our single-surgeon surgical experience in a Canadian tertiary hospital. METHODS We identified 57 patients with NSGCT who received primary chemotherapy and PCRPLND from 2010-2016. Surgical complication rate was graded with Clavien-Dindo classification. Chi-squared testing was used in testing for differences in proportion of PCRPLND tumor histology vs. the historical cohorts. Chi-squared testing was also used to analyze the association between primary orchiectomy tumor histology and post-chemotherapy residual mass (PCRM) tumor histology. RESULTS The overall complication rate was 23% (n=13), of which four were Clavien-Dindo grade IIIb and one was grade IVa. Fourteen percent of patients required additional procedure for resection of adjacent organs intraoperatively. There was a statistically significant difference in the distribution of PCRPLND tumor histologies (chi-squared p=0.0187), with a lower rate of viable tumor (7%) and higher rate of teratoma (63%) compared to historical cohorts. The absence of teratoma in the primary orchiectomy specimen was associated with the findings of fibrotic/necrotic tissue in the PCRM (chi-squared p=0.0005). CONCLUSIONS Our series demonstrated that the rate of viable tumor in PCRM appears lower than published historical series, and this possibly reflects the improvement in chemotherapy delivery in a contemporary series. The high rate of teratoma in the PCRM calls for ongoing need for PCRPLND. Grade III and IV surgical complications are considered rare in our series.INTRODUCTION Digital rectal examination (DRE) is part of the clinical evaluation of men on active surveillance (AS). The purpose of the present study is to analyze the value of DRE as a predictor of upgrading in a population of men with prostate cancer (PC) treated with AS. METHODS We used the prostate biopsy (PBx) database from an academic center, including PBx from 2006-2018, and identified 2029 confirmatory biopsies (CxPBx) of men treated with AS, of which 726 men had both diagnostic (initial) and CxPBx information available. We did a descriptive analysis and evaluated sensitivity, specificity, and predictive values of DRE for the detection of clinically significant PC (csPC). Multivariable regression analysis was done to identify predictors of csPC. The primary outcome was to evaluate DRE as a predictor of the presence of csPC at CxPBx. RESULTS Among the 2029 patients with a CxPBx, 75% had PC, and of these, 30.3% had upgrading to ISUP Grade ≥2. Thirteen percent of men had a suspicious DRE (done by their treating physician).
