Prognostic nomograms for patients with resected extremity soft tissue sarcoma (STS) include the Sarculator and Memorial Sloan Kettering (MSKCC) nomograms. We sought to validate these two nomograms within a large, modern, multi-institutional cohort of resected primary extremity STS patients.

Resected primary extremity STS patients from 2000 to 2017 were identified across nine high-volume U.S. institutions. Predicted 5- and 10-year overall survival (OS) and distant metastases cumulative incidence (DMCI), and 4-, 8-, and 12-year disease-specific survival (DSS) were calculated with Sarculator and MSKCC nomograms, respectively. Predicted survival probabilities stratified in quintiles were compared in calibration plots to observed survival assessed by Kaplan-Meier estimates. Cumulative incidence was estimated for DMCI. Harrell's concordance index (C-index) assessed discriminative ability of nomograms.

A total of 1326 patients underwent resection of primary extremity STS. Common histologies included undifferenhort of resected primary extremity STS patients. External validation of these nomograms supports their ongoing incorporation into clinical practice.We evaluated and compared the mammographic density assessment of an artificial intelligence-based computer-assisted diagnosis (AI-CAD) program using inter-rater agreements between radiologists and an automated density assessment program. Between March and May 2020, 488 consecutive mammograms of 488 patients (56.2 ± 10.9 years) were collected from a single institution. We assigned four classes of mammographic density based on BI-RADS (Breast Imaging Reporting and Data System) using commercial AI-CAD (Lunit INSIGHT MMG), and compared inter-rater agreements between radiologists, AI-CAD, and another commercial automated density assessment program (Volpara®). The inter-rater agreement between AI-CAD and the reader consensus was 0.52 with a matched rate of 68.2% (333/488). The inter-rater agreement between Volpara® and the reader consensus was similar to AI-CAD at 0.50 with a matched rate of 62.7% (306/488). The inter-rater agreement between AI-CAD and Volpara® was 0.54 with a matched rate of 61.5% (300/488). In conclusion, density assessments by AI-CAD showed fair agreement with those of radiologists, similar to the agreement between the commercial automated density assessment program and radiologists.We surveyed users of a behavioral health helpline serving New York City and surroundings, to assess their helpline experiences, changes in psychological distress after contacting the helpline, and factors associated with differences in these measures. We surveyed users twice roughly 2 weeks following their helpline contact, from 4/2019 to 9/2019 (N = 1097 respondents) and again 6 months following contact, from 10/2019 to 3/2020 (N = 732 respondents). Eighty-nine percent of respondents reported that contacting the helpline helped them deal a little or a lot more effectively with their problems. Rates of psychological distress decreased from 41.3% 2 weeks following helpline contact to 29.0% 6 months after (P  less then  0.05). Improvements in psychological distress were found across a range of demographic characteristics and were greatest for repeat users. Users reported broadly positive experiences with the helpline and improved psychological distress 6 months later. We propose to find volatile organic compounds (VOCs) from both weeds causing attraction of the biocontrol agent, G. placida, in order to attempt to use the insect as a biological weed control. Behavioral responses of G. placida towards volatile blends characteristic of undamaged (UD), insect-damaged (ID), jasmonic acid-treated (JA) or mechanically-damaged (MD) plants were conducted by Y-tube olfactometer bioassays. Cuminaldehyde was predominant in VOCs of UD R. dentatus, ID P. glabrum, and both JA and MD R. dentatus and P. glabrum. Geraniol was predominant in VOCs of UD P. Bulevirtide clinical trial glabrum, while 1,3-diethylbenzene predominated in VOCs of ID R. dentatus. Females were more attracted towards volatile blends of ID plants compared to UD or JA plants. Females did not show attraction towards volatile blends of JA plants. We identified two bioactive synthetics blends, one comprised of seven compounds - 16.65 µg 1,3-diethylbenzene, 10.72 µg acetophenone, 6.52 µg 2,6-(E,Z)-nonadienal, 2.46 µg 1-nonanol, 4.19 µg decanal, 9.86 µg 4-ethylacetophenone and 3.34 µg 1-hexadecene dissolved in 25 µl CH2Cl2 and the other containing five compounds - 2.50 µg 2-octanol, 6.84 µg limonene, 0.64 µg dodecane, 6.63 µg 4-ethylacetophenone and 0.24 µg geranyl acetone dissolved in 25 µl CH2Cl2. These two blends of volatile compounds could be used to attract the biocontrol agent during early vegetative period of these two weeds, which could lead to eradication of weeds from crop fields.

Sciatic hernias are rare pelvic floor hernias that occur through the sciatic foramen and often present as abdominal or pelvic pain, particularly in women. Historically, they were repaired using an open approach, with limited reports on their laparoscopic treatment.

Here we present the case of an 85-year-old woman who had repeated abdominal pain and was referred to our hospital for sciatic hernia surgery after conservative treatment. We laparoscopically observed the deep pelvis and identified the right sciatic hernia. When an extraperitoneal space was dissected, an ureterohypogastric nerve fascia (UNF) and a vesicohypogastric fascia (VF) were identified. Moreover, the maneuver to mobilize the fasciae inside from the pelvic wall made it possible to separate the ureter and urinary bladder, which might have otherwise incarcerated in the hernia. We repaired the defect of the sciatic foramen with a mesh plug and patch. The patient had an uneventful recovery, and the absence of sciatic herniation recurrence was confirmed 1year after surgery.

A laparoscopic repair of a sciatic hernia could permit detailed non-invasive observations of the deep pelvis and be performed effectively by recognizing an UNF and a VF located near the sciatic foramen.The aim of this study was to investigate the effect of red laser (660 nm) photobiomodulation (PBM) with different energies on tumor necrosis factor-alpha(TNF-α) expression for random skin flap viability in rats. Twenty-four Wistar rats were divided into three groups sham group (SG), PBM laser group with an energy dose of 0.29 J (0.29G), and PBM laser group with an energy dose of 7.30 J (7.30G). A cranially based dorsal skin flap measuring 10 × 4 cm was raised and a plastic barrier was placed between the flap and its bed. PBM was applied in 3 timepoints in the immediate postoperative period, in the 1st and in the 2nd postoperative days; the animals were euthanized on the 7th postoperative day. The assessments included TNF-α expression of 3 different flap areas (proximal, medial and distal), by immunohistochemistry; percentage of skin flap necrosis area, by the paper template method. The statistical analysis was performed through the Kruskal-Wallis and Mann-Whitney tests, the level of significance adopted was 5% (p less then 0.05). TNF-α expression was significantly lower for 7.30G in the proximal area, reduced for SG in the medial point, and larger for 7.30G in the distal area. The percentage of flap necrosis area was significantly reduced for 7.30G. Higher energy doses are more efficacious than lower energy doses for modulating TNF-α expression. PBM with an energy dose of 7.30 J was effective in reducing the expression of TNF-α and increase skin flap viability.

Little is understood about child welfare involvement (CWI) in cases where the birth mother has experienced human trafficking.

The aim of this study was to explore provider perceptions of the impact of CWI for the trafficked mother.

Participants were selected among providers caring for trafficked birth mothers. Semi-structured interviews were conducted with providers and qualitative content analysis was conducted.

Interviewees reported reasons for CWI, positive and negative impacts of CWI and provided recommendations for systems improvement.

Recommendations from this exploratory study include mechanisms to support trafficked mothers, train hospital social workers, and systems change. During the prenatal period, strategies to support the trafficked mother may include addressing gaps in social determinants of health, ensuring appropriate medical and mental health care, early screening and referral to substance use treatment services, enhancing community support, and working to develop safety plans for ening and referral to substance use treatment services, enhancing community support, and working to develop safety plans for survivors and their families. Enhanced engagement of social workers and all providers to improve understanding of the unique complexity of trafficked mothers is needed. Education should include an understanding that judgement of a caretaker's ability to parent should be current and holistic and not reflexive based on history in the electronic medical record. An exploration of the child welfare system itself should also be undertaken to identify and modify discriminatory laws and policies. Finally, efforts to address social determinants of health in the community and enhance the trauma-informed nature of child welfare referrals could improve the lives of trafficked mothers.
