Since its debut in late 2018, researchers have successfully used CatBoost for machine learning studies involving Big Data. We take this opportunity to review recent research on CatBoost as it relates to Big Data, and learn best practices from studies that cast CatBoost in a positive light, as well as studies where CatBoost does not outshine other techniques, since we can learn lessons from both types of scenarios. Furthermore, as a Decision Tree based algorithm, CatBoost is well-suited to machine learning tasks involving categorical, heterogeneous data. Recent work across multiple disciplines illustrates CatBoost's effectiveness and shortcomings in classification and regression tasks. Another important issue we expose in literature on CatBoost is its sensitivity to hyper-parameters and the importance of hyper-parameter tuning. One contribution we make is to take an interdisciplinary approach to cover studies related to CatBoost in a single work. This provides researchers an in-depth understanding to help clarify proper application of CatBoost in solving problems. The rapid spread of the coronavirus disease 2019 (COVID-19) epidemic has significantly impacted global health. So far, the evidence regarding the risk factors that predict the outcomes of COVID-19 patients is limited. In this study, we identified several risk factors that are associated with increased mortality in COVID-19 patients.

We performed a retrospective review of electronic medical records of the patients admitted with an initial diagnosis of COVID-19. We extracted several patient variables (including demographics, lab results, and pre-existing conditions) and examined for their association with increased mortality.

Of the 487 people included in the study, 340 survived and 147 expired. Significant differences existed in demographics and underlying comorbidities between the two groups. A higher proportion of patients were age 65 and older (87.76% vs 53.24%, p<0.001), and were predominantly male (63.27% vs 52.94%, p=0.0351). Multivariate analysis showed five variables to be the predictors for mortality age ≥65 [OR=3.87, 95% CI (2.01, 7.46), p<0.001], initial presentation with dyspnea [OR=1.71, 95% CI (1.03, 2.82), p=0.037], history of cardiomyopathy [OR=3.33, 95% CI (1.07, 10.41), p<0.038], positive initial chest imaging findings [OR=2.24, CI (1.26, 3.97), p=0.006], and acute kidney injury (AKI) [OR=3.33 CI (2.10, 5.28), P<0.001].

Silent hypoxia is an entity that has been described in patients diagnosed with COVID-19. It is typically described as objective hypoxia in the absence of proportional respiratory distress. The physiological basis for this phenomenon is controversial, and its prognostic value is unclear. We present a case below, of a 66-year-old female presenting with severe hypoxia that was managed without mechanical ventilation.

A 66 year old female with multiple comorbidities initially presented with a cough, fever and an oxygen saturation of 70% on room air in the absence of respiratory distress or altered mentation. She subsequently tested positive for COVID-19 and was admitted to the intensive care unit; received oxygen via high flow nasal cannula and continuous positive pressure mask. The patient remained in the intensive care unit for 40 days under close observation and exhibited multiple episodes of silent hypoxia on weaning oxygen. She was discharged on room air with an oxygen saturation >90% after 56 days. Th intubation further displays the need for clearer analysis of the situation. Our case demonstrates a favorable outcome of the latter approach, yet emphasizes a case-by-case approach until clearer recommendations are available.

A case-series of 61 patients admitted to our intensive care unit (ICU) between August 12 and September 12, 2020 with confirmed COVID-19 pneumonia and rapidly evolving ARF requiring oxygen support therapy and/or mechanical ventilation was retrospectively analyzed. We examined whether intravenous administration of tocilizumab, a monoclonal interleukin-6 receptor antibody, was associated with improved outcome. All patients received empiric antivirals, dexamethasone 6mg/day for 7 days, antibiotics, and prophylactic anticoagulation. Tocilizumab was administered at a dosage of 8mg/kg [two consecutive intravenous infusions 12h apart]. Outcome measures such as mortality on day-14, ICU length of stay, and rate of nosocomial acquired bacterial infections were also analyzed. Tocilizumab could be an adjunct safe therapy in rapidly evolving COVID-19 pneumonia and associated critical illness.Homelessness continues to challenge service providers in the United States where 600,000+ individuals are without their own place to stay on any given night. With significant barriers preventing access to food, homeless persons remain at risk for experiencing long-term food insecurity. As such, the primary intent of this paper was to examine specific hypotheses that explored the impact of both risks/stressors and resources on the reported food insecurity among homeless adults. Using a stratified quota sample, data were collected from homeless adults residing in two counties in Northwest Arkansas. One hundred and sixty-eight face-to-face interviews were conducted with homeless persons living in sheltered and unsheltered environments. We found a strong positive association with persons who had been arrested and reported food insecurity; anxiety symptoms and physical health symptoms were both positively related to food insecurity. Additionally, there were significant resources (access to medical services, and community connectedness) that were negatively related to food insecurity among homeless adults.
