Pediatric readiness scores may be a useful measure of a hospital's preparedness to care for children. However, there is limited evidence linking these scores with patient outcomes or other metrics, including the need for interfacility transfer. This study aims to determine the association of pediatric readiness scores with the odds of interfacility transfer among a cohort of noninjured children (< 18 years old) presenting to emergency departments (EDs) in small rural hospitals in the state of California.

Data from the National Pediatric Readiness Project assessment were linked with the California Office of Statewide Health Planning and Development's ED and inpatient databases to conduct a cross-sectional study of pediatric interfacility transfers. Hospitals were manually matched between these data sets. Logistic regression was performed with random intercepts for hospital and adjustment for patient-level confounders.

A total of 54 hospitals and 135,388 encounters met the inclusion criteria. EDs with a high pediatric readiness score (>70) had lower adjusted odds of transfer (aOR 0.55, 95% CI 0.33-0.93) than EDs with a low pediatric readiness score (≤ 70). The pediatric readiness section with strongest association with transfer was the "policies, procedures, and protocols" section; EDs in the highest quartile had lower odds of transfer than EDs in the lowest quartile (aOR 0.54, 95% CI 0.31-0.91).

Pediatric patients presenting to EDs at small rural hospitals with high pediatric readiness scores may be less likely to be transferred. Additional studies are recommended to investigate other pediatric outcomes in relation to hospital ED pediatric readiness.

The present study extended previous research with a pilot trial of TM as a treatment for PTSD via a single-blinded, randomized controlled design. veterans with PTSD (N = 40) were assigned to a TM intervention or treatment-as-usual (TAU) control group. Participants in the TM group engaged in 16 sessions over 12 weeks, primarily in a 60-min group format. Change in PTSD symptoms, measured via the Clinician-Administered PTSD Scale for DSM-5 (CAPS-5) was the primary outcome. Secondary outcomes included self-reported PTSD symptoms, depression, anxiety, sleep difficulties, anger, and quality of life (QoL). Assessments were conducted at baseline and 3-month follow-up. Mean CAPS-5 score decreases were significantly larger for participants in the TM group (M = -11.28, 95% CI [-17.35, -5.20]), compared to the TAU group (M = -1.62, 95% CI [-6.77, 3.52]), p = .012, d = -0.84. At posttest, 50.0% of veterans in the TM group no longer met PTSD diagnostic criteria as compared to 10.0% in the TAU group, p = .007. The US Preventive Services Task Force recommends lung cancer screening with Low-Dose Computed Tomography (LDCT) in high-risk individuals. Our objective was to identify demographic, health, and financial factors associated with screening uptake, with a focus on urban-rural differences.

We analyzed data from the 2018 and 2019 Behavioral Risk Factor Surveillance System and its optional Lung Cancer Screening Module to examine factors associated with screening uptake among 20 states that administered the optional module. We compared differences in factors associated with uptake overall and by geographical regions and conducted multivariable logistic mixed-effects regression, accounting for participant clustering by state to assess the impact of these factors on uptake.

Overall 1,268 participants underwent LDCT screening with no significant differences observed between rural (16.3%) and urban residents (17.7%, p = 0.67). In multivariable models, rural residents did not differ significantly in their LDCT screein veterans' health settings. Additionally, reducing structural barriers to care related to insurance and income will be key to reducing disparities.The aim of this study was to characterize and determine how days in milk (DIM) affect the milk fatty acid (FA) profile of grazing donkeys. Donkey milk is very similar to human milk, containing bioactive molecules such as FA and proteins. However, there is a lack of scientific and technical information on the changes in the FA profile of asinine milk in pasture-feeding systems. Seven multiparous Pega donkeys, maintained in an exclusively extensive system, were used. Milking was undertaken twice a day, once a week for 16 weeks. Milk samples were composed according to the average lactating days as follows 55, 110, 165, 220 and 275 DIM. A descriptive analysis of the milk fat composition was performed, and the linear and quadratic effects of DIM on the milk FA profile were tested. The milk FA profile of grazing donkeys is influenced by lactation days and is characterized by high concentrations of polyunsaturated FA (PUFA) with a low n-6 to n-3 ratio (0.66 g/100 g), mainly due to the high level of linolenic acid (16.8 g/100 g). Most of the individual FAs did not change during lactation, but stearic and oleic acid linearly decreased (p less then .05), and total polyunsaturated FA (PUFA) and n-3 FA increased (p less then .05) with DIM. The milk profile of grazing donkeys is influenced by the day of lactation and is characterized by a high concentration of n-3 PUFA (mainly α-linolenic acid) and a lower n-6 to n-3 ratio. As the number of days of lactation increased, the concentration of n-3 PUFA also increased. Therefore, higher milk quality may be associated with higher days in milk.
