SEAs identify specific types of adverse events and high-risk conditions, describe their underlying causes, and recommend steps to reduce risk and prevent future occurrences. Health care organizations should consider the information in SEAs when designing or redesigning processes, including specific suggestions contained in the alerts to mitigate risks. Recently, The Joint Commission decided that co-publishing SEAs in the Journal would help expand dissemination efforts by reaching beyond health care organizations to the broader quality and safety community, including researchers and policymakers. In this issue, we are proud to publish Sentinel Event Alert 63 Optimizing Smart Infusion Pump Safety with DERS (dose error reduction software). The SEA describes actions health care organizations can take to reduce the risk of errors caused by the misuse of smart infusion pumps, especially errors that can be avoided by the optimal use of DERS. We will co-publish future SEAs that we think would be of interest to the Journal's audience. We hope you enjoy this new addition to the Journal.

, in the third trimester of the pregnancy (28-40weeks gestation), and receiving regular prenatal care from a licensed provider.

A typology analysis chart was used to group participants' responses from their interviews in the areas of positive or negative feelings regarding their prepregnancy body and weight, and positive or negative feelings regarding their pregnant body.

Data were analyzed using a four-square matrix by identifying those participants who felt positive regarding their prepregnancy body/weight and pregnant body (n= 6), felt negative regarding their prepregnancy body/weight and pregnant body (n= 3), felt negative regarding their prepregnant body but positive regarding their pregnant body (n= 8), and felt positive regarding their prepregnant body but negative about their pregnant body (n= 3).

Bioburden control in ambulances, through cleaning and disinfection, is crucial to minimize hospital-acquired infections, cross contamination and exposure of patients and ambulances' crew. In Portugal, firefighter crews are responsible, besides fire extinction, for first aid and urgent pre-hospital treatment. This study assessed the bioburden in Portuguese firefighters' ambulances with a multi-approach protocol using active and passive sampling methods. Fungal resistance profile and mycotoxins detection in ambulances' ambient, and S. aureus (SA) prevalence and resistance profile in ambulances' ambient and colonization in workers were also investigated. Toxigenic fungi with clinical relevance, namely Aspergillus section Fumigati, were found on ambulance's air in the hazardous dimension range. Estradiol Interestingly, surface contamination was higher after cleaning in several sampling sites. Prevalence of S. aureus was 3% in environmental samples, of which 2% were methicillin-sensitive (MSSA) and 1% methicillin-resistant (MRSA). About 2.07 fungal species were able to grow in at least one azole, ranging from one (44% samples) to five (6% samples) species in each azole. Mycotoxins were detected in mops and electrostatic dust cloths. Colonization by S. aureus in the firefighter crew was observed with a high associated prevalence, namely 48%, with a 24% prevalence of MSSA (8/33) and 21% of MRSA (7/33). Additional studies are needed to determine the potential risk of infection transmission between different vehicle fleets and under varying conditions of use. This will strengthen the paramedic sector's mission to save lives without putting their own health and safety at risk.Accumulating evidence from both experimental and nonexperimental human studies in the last 15 years indicates that exposure to high levels of the trace element selenium increases the risk of type 2 diabetes. However, the relation of dose to effect is not well understood because randomized controlled trials used only one dose (200 μg/day) of selenium supplementation. While no new trial on this topic has been published since 2018, several nonexperimental studies have appeared. We therefore updated a previous meta-analysis to include recently published observational studies, and incorporated the recently developed one-stage random-effects model to display the dose-response relation between selenium and type 2 diabetes. We retrieved 34 potentially eligible nonexperimental studies on selenium and diabetes risk up to April 15, 2021. The bulk of the evidence indicates a direct relation between blood, dietary and urinary levels of selenium and risk of diabetes, but not with nail selenium, which may be considered a less reliable biomarker. The association was nonlinear, with risk increasing above 80 μg/day of dietary selenium. Whole blood/plasma/serum selenium concentrations of 160 μg/L corresponded to a risk ratio of 1.96 (95% CI 1.27-3.03) compared with a concentration of 90 μg/L (approximately 60 μg of daily selenium intake). The cohort studies, which are less susceptible to reverse causation bias, indicated increased risk for both blood and urine selenium levels and dietary selenium intake, whereas no such pattern emerged from studies relying on nail selenium content. Overall, the nonexperimental studies agree with findings from randomized controlled trials, indicating that moderate to high levels of selenium exposure are associated with increased risk for type 2 diabetes.

Short-duration exposure to ambient particulate matter (PM) air pollution is associated with cardiac autonomic dysfunction and prolonged ventricular repolarization. However, associations with sub-chronic exposures to coarser particulates are relatively poorly characterized as are molecular mechanisms underlying their potential relationships with cardiovascular disease.

The lockdown effect on carbon emissions has been studied by many researchers using statistical approaches. However, the lockdown effect on atmospheric carbon dioxide (CO2) on an urban scale remains unclear. Here we present CO2 concentration and carbon isotopic (δ13C) measurements to assess the impact of COVID-19 control measures on atmospheric CO2 in Xi'an, China. We find that CO2 concentrations during the lockdown period were 7.5% lower than during the normal period (prior to the Spring Festival, Jan 25 to Feb 4, 2020). The observed CO2excess (total CO2 minus background CO2) during the lockdown period was 52.3% lower than that during the normal period, and 35.7% lower than the estimated CO2excess with the effect of weather removed. A Keeling plot shows that in contrast CO2 concentrations and δ13C were weakly correlated (R2 = 0.18) during the lockdown period, reflecting a change in CO2 sources imposed by the curtailment of traffic and industrial emissions. Our study also show that the sharp reduction in atmospheric CO2 during lockdown were short-lived, and returned to normal levels within months after lockdown measures were lifted.Workers in marble processing plants are at high risk of exposure to high levels of marble dust containing silica, but there are limited studies evaluating the genotoxicity and oxidative stress parameters of workers occupationally exposed to marble dust. In this study, we aimed to clarify how marble dust affects genotoxicity and immunotoxicity mechanisms alongside oxidative stress in the workers in the marble processing plants of Iscehisar, Turkey. The oxidative stress and immune system parameters were determined spectrophotometrically using commercial kits. Genotoxicity was evaluated by Comet and micronucleus (MN) assays in the lymphocytes and buccal cells, respectively. The enzyme activities of superoxide dismutase, catalase, glutathione reductase, glutathione peroxidase, and the levels of glutathione, and Clara cell secretory protein CC16 in workers (n = 48) were significantly lower than in controls (n = 41), whereas the levels of malondialdehyde, 8-oxo-7,8-dihydro-2'-deoxyguanosine, tumor necrosis factor-alpha, interleukin-1beta were significantly higher in workers. DNA damage in workers were significantly higher than in controls and there was a clear correlation between the increase in DNA damage and the duration of exposure. Marble workers had significantly higher MN frequencies when compared to controls. The results indicate the possibility of immunotoxic and genotoxic risks to workers in marble industry.
