01). Chronic hypoxia and exercise training increased SpO2 measured after acute hypoxic exposure. Training blunted the decrease in V ˙ O2 peak, time of exhaustion, and maximum speed associated with chronic exposure to hypoxia. Chronic hypoxia led to a right ventricular hypertrophy, which was not corrected by 1-month exercise training. Altogether, by decreasing hematocrit, reducing body weight, and limiting performance decrease, training in hypoxia may have a beneficial effect on excessive erythropoiesis in chronic hypoxia. Therefore, regular exercise training might be beneficial to avoid worsening of CMS symptoms in high altitude dwellers and to improve their quality of life.
To investigate whether a metabolic cart using a flowmeter in the upper range of accepted resistance to airflow (<1.5cmH
O∙L
∙s
for flows up to 14 L∙s
, American Thoracic Society) negatively impacts exercise performance in healthy individuals.
16 recreationally active males (age 25±1years, height 180±6cm, weight 73.5±5.8kg, all mean ± SD) performed two incremental tests on a bicycle ergometer on each of two visits, using a metabolic cart with a flowmeter of either low (Oxycon Pro) or high (Innocor) airflow resistance. Mouth pressures, gas exchange, blood lactate concentration [La
], perception of breathlessness, respiratory, and leg exertion were assessed throughout the tests.
Tests performed with the Innocor were significantly shorter (15.3±3.2 vs. 15.8±3.3min, p<0.0001) and showed higher maximal flow resistance (1.3±0.2 vs. 0.3±0.0 cmH

, p<0.0001). At end-exercise, peak oxygen consumption (-200±220ml.min
, p<0.0001), minute ventilation (-19.9±10.5 L.min
, p<0.0001), breathing frequency (-5.4±5.2 breaths.min
, p<0.0001), heart rate (-2.1±3.6bpm, p=0.002) and [La
] (-0.7±1.0mmol.L
, p<0.0001), but not tidal volume (-0.1±0.2 L, p=0.172) were lower with the Innocor, while the perception of breathlessness was higher (+3.8±5.1 points, p<0.0001).
Airflow resistance in the upper range of current guidelines can significantly affect exercise performance and respiratory pattern in young, healthy males during incremental exercise. The present results indicate the need to revisit guidelines for devices used in ergospirometry.

The phase III PROSELICA (NCT01308580) and FIRSTANA (NCT01308567) trials investigated taxane chemotherapy among men with postdocetaxel metastatic, castration-resistant prostate cancer (mCRPC) or chemotherapy-naïve mCRPC, respectively. We present a post hoc analysis of patient-reported health-related quality of life (HRQL) among patients with or without a clinical (pain, tumor, or prostate-specific antigen [PSA]) response.
PROSELICA and FIRSTANA HRQL and pain data were collected and analyzed using protocol-defined Functional Assessment of Cancer Therapy-Prostate (FACT-P) and McGill-Melzack (Present Pain Intensity scale) questionnaires. Outcomes included definitive FACT-P Total Score (TS) improvements and longitudinal assessment of FACT-P TS.
In PROSELICA and FIRSTANA, the proportion of patients receiving taxane chemotherapy with a definitive FACT-P TS improvement was significantly higher among patients with versus without a pain or PSA response (pain PROSELICA 67% vs. 33.5%; p<.001; FIRSTANA 75.2% vs. py on the HRQL of patients with mCRPC and allows for a better understanding of the relationship between treatment, response, and HRQL, supporting therapeutic decision making.
To describe the clinical and pathological characteristics, diagnosis, treatment, and outcomes of juvenile granulosa cell tumor (JGCT).
We retrospectively analyzed the data of 19 patients with histopathologically confirmed juvenile granulosa cell tumors treated in two medical centers in Wuhan city of China between 1999 and 2019.
Totally, 19 patients were included during the period. The median age at diagnosis was 8.25 years (range, 0.25-28 years). The most common clinical presentation was abdominal pain, five out of 10 prepubertal children presented with precocious puberty. Three patients underwent radical surgery (including total hysterectomy, bilateral salpingo-oophorectomy, omentectomy, and pelvic and para-aortic lymphadenectomy), the other 16 patients had fertility-sparing surgery (cystectomy or ipsilateral salpingo-oophorectomy with or without omentectomy and lymphadenectomy). see more Eighteen patients had the international federation of gynecology and obstetrics (FIGO) stage I tumors, one patient had FIGO in stage Ic patients is still unknown. Fertility sparing surgery should be considered in young patients who wish to bear children. While we have been looking for alternative educational materials for our undergraduate students, we found ourself in Covid-19 pandemic and had to think postgraduate education. On the first day of restrictions, our major problem was to learn and find the best communication tool. The first experiences for online meetings were boring, we were not feeling our emotions but as days past we got used to it. We all understand the importance of having alternatives and have to be ready for second choices. During those challenging days, all had more time to work on our completed data, time to write, complete the proofreading of written materials. Other vital scientific activities for career development and profile building such as reading (every meeting with new articles/reviews), publishing and preparing oral presentation were fully completed during the online meetings as a part of lab meeting activities. They learned to improve communication skills, be active participants, and fight. Many diseases are polygenic and can only be treated efficiently with drugs that modulate multiple targets. However, rational design of compounds with multi-target profiles is rarely pursued because it is considered too difficult, in particular if the drug must enter the central nervous system. Here, a structure-based strategy to identify dual-target ligands of G-protein-coupled receptors is presented. We use this approach to design compounds that both antagonize the A2A adenosine receptor and activate the D2 dopamine receptor, which have excellent potential as antiparkinson drugs. Atomic resolution models of the receptors guided generation of a chemical library with compounds designed to occupy orthosteric and secondary binding pockets in both targets. Structure-based virtual screens identified ten compounds, of which three had affinity for both targets. One of these scaffolds was optimized to nanomolar dual-target activity and showed the predicted pharmacodynamic effect in a rat model of Parkinsonism.
