This study, however, assesses the perspectives of the partners providing support, and specifically their perceptions of the relational context and their support strategy use. An MTurk sample of 283 partners of individuals trying to lose weight reported on relational context characteristics (i.e., having a team effort, opposing approaches, and imposition regarding the weight loss pursuits) and their use of encouragement, direct influence, and coercion. Analyses largely supported the hypotheses regarding the associations between the relational characteristics and strategy use. For example, higher team effort and lower opposing approaches about weight management were linked with more encouragement, and greater opposing approaches and imposition were associated with more coercion. Four relational weight loss climates based on the collective of these characteristics were also linked with strategy use. All associations were found after accounting for general relationship satisfaction as well as other control variables. Overall, the findings suggest that perceptions of the weight loss in the context of the relationship were connected to the strategies partners employed in helping targets achieve weight loss. Because strategies vary in effectiveness, understanding which relational characteristics, as well as which broader relational climates, facilitate partner strategy choice can ultimately help augment individuals' weight loss success.The aim of this study was to investigate the microbial quality of indoor air environment and hygienic practices of food handlers in Haramaya University cafeteria, Ethiopia. A total of 36 air samples were collected from the cafeteria using passive air sampling technique. Furthermore, 42 swab samples were taken from the hands and clothes of food handlers for microbial analysis. The results showed that air quality in the cafeteria is below satisfactory. Total coliform counts from all the swab samples were also above the acceptable microbial target value. Significantly higher microbial load was recorded in food handlers involved in serving food than these deployed in baking and sauce making roles. This study generally showed the importance of indoor air quality, and hands and clothes of food handlers as potential sources of bacterial contamination in the University's cafeteria. Therefore, improved housing condition, regular training, and close supervision are recommended.While photochemical oxidants (Ox = O3+ NO2) are known to increase asthma flare-ups, there is a paucity of studies of the Japanese population, especially for Tokyo residents. We used data on asthma cases (n = 7,455) from ambulance dispatches in Tokyo, 2015-2016. Variables included date and time of incidence, age, sex, occurrence location at the ward (ku) level, and the symptom/cause of dispatch as recorded by paramedics. Ox data were obtained from the nearest air quality monitoring station to the occurrence location, then linked them with the outcomes based on occurrence date. We directly incorporated a distributed lag model into a bi-directional case-crossover study design controlling for ambient temperature and day of week. A 10-ppb increase in Ox for lag days 0-3 was associated with a 5.51% (95% CI 0.13 to 11.18) increase in ambulance dispatches related to asthma. The association was strongest on lag day 1 (4.67%, 95% CI 0.51 to 9.00). Exposure to high levels of Ox was associated with increased ambulance dispatches related to asthma exacerbations in Tokyo, Japan.

To describe the end-of-life care characteristics of children and adolescents with solid tumors (ST) or hematologic malignancies (HM) who died from tumor progression in the Île-de-France area.

This is a regional, multicentric, retrospective review of medical files of all children and adolescents with cancer who died over a 1-year period. Extensive data from the last 3 months of life were collected.

A total of 99 eligible patients died at a median age of 9.8 years (range, 0.3-24 years). The most frequent terminal symptoms were pain (n = 86), fatigue (n = 84), dyspnea (n = 49), and anorexia (n = 41). Median number of medications per patient was 8 (range, 3-18). Patients required administration of opioids (n = 91), oxygen (n = 36), and/or sedation (n = 61). Decision for palliative care was present in all medical records and do-not-resuscitate orders in 90/99 cases. Symptom prevalence was comparable between children and adolescents with ST and HM. A wish regarding the place of death had been expressed for 64 patients and could be respected in 42 cases. Death occurred in hospital for 75 patients.

End-of-life remains an active period of care requiring coordination of multiple care teams.The objective of this study was to compare the silk suture with a cyanoacrylate adhesive to stabilize the free gingival graft in conjunction with Er YAG laser-assisted recipient site preparation to augment the keratinized tissue in gingival recession cases. This randomized trial comprised of 300 recession defects patients. All the included patients were diagnosed using Miller class I and II gingival recession defects classification. Group I sites were treated with a free gingival graft (FGG) harvested using an Er YAG laser and further sutured with silk. Group II sites were stabilized with isoamyl 2 cyanoacrylate bio-adhesive material. Clinical parameters, such as gingival recession depth, clinical attachment level, gain in gingival tissue thickness, and width of keratinized gingiva were recorded at baseline, and at third month, sixth month, and 12th month postoperatively. The mean changes in gingival recession from months 3 to 6 and months and 6 to 12 were significant (p  0.05). Cyanoacrylate could be used as a substitute to silk sutures to stabilize FGGs. Cyanoacrylate was easy to apply, consumed less operating time, and was considered equally efficacious for stabilizing FGGs.

To report our experience and compare the results of percutaneous endovascular aortic aneurysm repair (PEVAR) performed under monitored anesthesia care (MAC) to PEVAR under general anesthesia (GA).

A retrospective review of patients who underwent non-emergency endovascular abdominal aortic aneurysm repair (EVAR) was completed. Patients were excluded if they had a complex repair, including fenestrated, branched, or parallel endografting. Demographics, operative data, 30-day mortality/morbidity and postoperative outcomes were analyzed.

A total of 159 patients were identified with a median age of 69. 115 patients had PEVAR, 45 (39.1%) PEVAR MAC and 70 (60.9%) PEVAR GA. PEVAR MAC compared to PEVAR GA had decreased operative time (106 vs. 134 min,

 < 0.001), time in the operating room (163 vs. 245 min,

 = 0.016), and estimated blood loss (EBL) (115 vs. 176 mL

 = 0.012). There was no statistically significant difference in the hospital length of stay (LOS) (1.9 vs. 2.7 days,

 = 0.133), and post-operative complications including pulmonary (2.2 vs. 2.9%,

 = 0.835). Forty-four patients had EVAR with a femoral cutdown (FC), including 14 PEVAR conversions. PEVAR conversion was associated with higher EBL (543 vs. 323 mL,

 = 0.03), operative time (230 vs. 178 min,

 = 0.01), and operating room time (307 vs. 275 min,

 = 0.01) compared to planned EVAR with FC.

PEVAR under MAC is associated with shorter time in the operating room compared to PEVAR under GA. PEVAR under MAC does however not decrease overall morbidities, including postoperative pulmonary complications.

We performed a pilot study to understand how living with a hernia impacts on the quality of life. Twelve semistructured interviews were performed. A number of themes were identified. These demonstrated the significant impact on physical health, ability to work, psychological health and social relationships. Further work is required to better understand the patient perspective and ensure that hernia operations improve quality of life in these settings.
