Single-stage approach with bile duct exploration is considered the most efficient and cost-effective method of bile duct clearance. In Australia, apart from centres with subspecialty interests, notably in Brisbane, Queensland, a multi-stage approach with endoscopic retrograde cholangiopancreatography (ERCP) is used more frequently. We aim to evaluate the impact of single stage laparoscopic trans-cystic exploration (LTCE) versus multi-stage approach for choledocholithiasis.

This was a retrospective cohort study. Medicare Benefits Schedule codings were used to identify patients who had the following procedures between December 2011 and December 2019 laparoscopic cholecystectomy (LC) and ERCP, LC and LTCE, LC and LTCE and ERCP. Primary outcomes were number of hospital procedures, admissions and additive length of stay (aLOS), the cumulative hospital stay from admission to discharge.

Of 607 patients, 204 (34%) patients received a single-stage LTCE, while 403 (66%) patients had a multi-stage approach. In the LTCE group, 82% (168) patients and 93% (190) patients had one procedure and one admission respectively for stone clearance (P=0.001). The median aLOS was 4 days for LTCE versus 7 days for multi-stage approach (P=0.001; 95% CI for difference - 3 to -2). In the multi-stage group, 16% (65) patients had three or more procedures and 49% (199) patients required two or more hospital admissions to achieve stone clearance.

Our aims were to evaluate the ultrasound scan (USS) criteria in diagnosing hypertrophic pyloric stenosis (HPS), determine the best diagnostic threshold values for pyloric muscle thickness (PMT) and pyloric canal length (PCL), and assess the accuracy of flow through the pylorus.

All patients who underwent pyloromyotomy at our tertiary paediatric surgery centre between July 2013 and June 2019 were identified (Group 1). All patients undergoing an USS to investigate for a possible HPS and did not undergo pyloromyotomy were also identified (Group 2). Accuracy was determined by Youden's Index (J) with the highest J determining the ideal cut-off value.

Two hundred and eighty-four patients (142 patients in each group) were included in the analysis. Using only the last USS before surgery, PMT provided an area under the curve (AUC) of 0.987 (n= 275), and PCL an AUC of 0.977 (n= 267). Ideal threshold values were 3.0 mm for PMT (J= 0.92), and 14.5 mm for PCL (J= 0.87). Combining PMT ≥3.0 mm with a PCL ≥14.5 mm resulted in a sensitivity of 95% and specificity of 99% (J=0.94). The absence of flow through the pylorus on dynamic USS provided a sensitivity of 99% and specificity of 91% (J= 0.91; n= 277).

We used the Langmuir-Schaeffer technique to generate the continuous and compact assembly of CdSe/CdS chiral film. We assembled achiral CdSe/CdS nanorods by controlling the number of layers and angles between different layers. This allowed us to tailor chiroptical properties to achieve high CPL signals. The chiral film was symmetrical and had the highest circular dichroism (CD) response and CPL signals with ten layers (RH (right-handed)-/LH (left-handed)-5 + 5 layers) and a 45° inter-angle. Specifically, RH-5+5 of the chiral film exhibited 1431 mdeg of CD activity and strong CPL signals with a dissymmetry factor (glum) of 0.0997. The helical stacked crystal plates with linear birefringence resulted in strong circular birefringence, as determined by the Reusch model. Electromagnetic simulations indicated that such remarkable optical activity was attributed to the birefringence and dichroism of the well-aligned CdSe/CdS nanorod layers in the chiral films. Under right/left circular polarized (RCP/LCP) light excitation, the well aligned semiconductor nanorods exhibited differences in the coupling efficiencies to RCP and LCP light. Classical ecological theory powerfully represents mechanistic interactions predicting environmental dependence of microbiome effects on host ecology, but these models are notoriously difficult to evaluate empirically. Alternatively, host-microbiome feedback theory represents impacts of microbiome dynamics on host fitness as simple net effects that are easily amenable to experimental evaluation. The feedback framework enabled rapid progress in understanding microbiomes' impacts on plant ecology, and can also be applied to animal hosts. We conceptually integrate these two frameworks by deriving expressions for net feedback in terms of mechanistic model parameters. The present study aimed to measure the prevalence and severity of food insecurity in people with a severe mental illness, defined as schizophrenia and related psychoses, and bipolar disorder; and explore relationships between food insecurity status, and sociodemographic and clinical characteristics.

This cross-sectional study recruited community-dwelling people with severe mental illness receiving clozapine and/or a long-acting injectable antipsychotic medication within three mental health services in Sydney, Australia. Participants completed the 18-item Household Food Insecurity Access Scale. Sociodemographic and medical information was obtained from participants' medical records. Independent samples t-test and chi-square analyses were used to test for between group differences based on food insecurity status. Binary logistic regression analyses adjusting for age and gender were used to determine the odds ratio.

One-hundred and eighty-eight people completed the assessment 63% were male, mean age was 49.2 ± 12.4 years, and the majority (85%) had a diagnosis of schizophrenia. Food insecurity was detected in 31% of participants. Of those who were food insecure, 12% were classified as severe, 13% as moderate and 7% as mild. Tobacco smoking was higher in food insecure people compared to food secure people (odds ratio=3.1, 95% CI 1.3 to 7.1, p=0.01). Food insecurity status was not associated with demographic, diagnostic or other clinical data.

Despite their importance in plant pathogen interactions and tolerance to diseases, it remains challenging to detect polyphenols in complex plant tissues. Here, we create molecular sensors for plant polyphenol imaging that are based on near-infrared (NIR) fluorescent single wall carbon nanotubes (SWCNTs). We identified polyethylene glycol- phospholipids that render (6,5)-SWCNTs sensitive ( K d = 90 nM) to plant polyphenols (tannins, flavonoids, …), which red-shift (up to 20 nm) and quench their emission (~1000 nm). These sensors report changes in total polyphenol level after herbivore or pathogen challenge in crop plant systems (Soybean Glycine max ) and leaf tissue extracts ( Tococa spp). We furthermore demonstrate remote chemical imaging of pathogen-induced polyphenol release from roots of soybean seedlings over the time course of 24h. This approach allows in situ visualization and understanding of the chemical plant defense in real time and paves the way for plant phenotyping for optimized polyphenol secretion.Electrocatalytic reduction of CO2 (CO2RR) to value-added chemicals is of great significance for CO2 utilization. Due to the slow proton-feeding rates from sluggish water dissociation kinetics, however, the CO2RR process involving multi-electron and proton transfer is greatly limited by poor selectivity and low yield. Herein, we develop an atomically dispersed monovalent zinc anchored on nitrogenated carbon nanosheets (Zn/NC NSs) as an efficient catalyst for CO2RR. Benefiting from the unique coordination environment and atomic dispersion, the optimized Zn/NC NSs exhibits a superior CO2RR performance, featured by a high current density up to 50 mA cm-2 with an outstanding CO Faradaic efficiency of ~95%. The center Zn(I) atom coordinated with three N atoms and one N atom that bridge over two adjacent graphitic edge (Zn-N3+1) is identified as the catalytically active site by thorough structural characterizations. In-situ attenuated total reflectance infrared absorption spectroscopy results reveal that the twisted Zn-N3+1 structure accelerates the CO2 activation and protonation in the rate-determining step of *CO2 to *COOH on the rationally engineered proton-feeding centers, while theoretical calculations elucidate that atomically dispersed Zn-N3+1 moieties decrease the potential barriers for the intermediate COOH* formation, promoting the proton-coupled CO2RR kinetics and boosting the overall catalytic performance.
