All rights reserved.INTRODUCTION In women with T1-2 breast cancer and one to two positive axillary lymph nodes (LN) at low risk for recurrence, postmastectomy radiation therapy (PMRT) may provide insufficient benefit to justify its toxicity. This study evaluated the interaction of factors associated with overall survival (OS) after PMRT in these patients. METHODS The National Cancer Database was queried for women with T1-2 breast cancer undergoing mastectomy with one to two positive LN identified on lymphadenectomy. Patients were grouped according to number of positive LN and then stratified by PMRT use. Differences in OS were evaluated. RESULTS Multivariable modeling demonstrated an interaction effect of age on the efficacy of PMRT. In patients more than or equal to 60 years old, PMRT was associated with improved survival when adjusting for age and tumor grade in patients with 1 to 2 positive LN (risk ratio = 0.62, 95% confidence interval = 0.40-0.93, P = .018). In patients less than 60 years old, tumor size and grade, but not PMRT, were associated with improved OS. CONCLUSION For women with T1-2 breast cancer and one to two positive LN, PMRT's association with OS is influenced by age, tumor grade, and number of positive LN. PMRT appears to be associated with improvements in OS in older patients, but not younger patients, regardless of tumor size or nodal status. © 2020 Wiley Periodicals, Inc.OBJECTIVE  The aim of this study was to validate impedance technique (IT) by investigating the agreement in cardiac output measurements performed by IT and echocardiography (ECHO). STUDY DESIGN  This is a prospective observational study, including a total of 30 neonates who underwent hemodynamic measurements by IT and ECHO. To determine the agreement between both methods, we performed IT to measure stroke volume (SV-IT) and cardiac output (CO-IT) immediately before or after ECHO to measure SV (SV-ECHO) and CO (CO-ECHO). The precision and accuracy of the IT relative to ECHO were assessed. RESULTS  SV-ECHO and SV-IT were (4.45 ± 0.78) and (4.54 ± 0.81) mL, respectively. The bias and limits of agreement of SV-IT were 0.09 mL and ( -1.92 to 1.73) mL, respectively. The true precision of SV-IT was 27.3%. Furthermore, CO-ECHO and CO-IT were (0.62 ± 0.12) and (0.61 ± 0.12) L/min, respectively. The bias and LoA of CO-IT were 0.01L/min and (-0.33 to 0.31) L/min, respectively. The true precision of CO-IT was 28.3%. CONCLUSION  Agreement between the IT and ECHO in the cardiac output measurement appeared acceptable. However, the accuracy and precision of the IT approach should be further investigated using a larger sample. Literature search on the secondary metabolites of endophytes have shown several pharmacologically active compounds especially anti-inflammatory compounds, which have been reviewed in the present paper. The article is structured based on the chemical classification of secondary metabolites. The compounds were identified to possess activity against a total of 16 anti-inflammatory targets. The most common targets involved were NO, TNF-α, and inhibition of total ROS. Further, the article gives a detailed insight into the compounds, their endophytic source, and anti-inflammatory target as well as potency. The contents of the article cover all the scientific reports published until Feb. 2019. Thus 118 compounds and 6 extracts have been reported to be obtained from endophytic sources showing anti-inflammatory activities. METHODS  A literature review was performed using PubMed for articles related to "abdominal wall transplantation (AWT)." The authors of this study sorted through this search for relevant publications that describe abdominal wall transplant anatomy, technical descriptions, and outcomes of various techniques. RESULTS  A total of four distinct revascularization techniques were found in the literature. Each of these techniques was described by ntinues to grow. Resveratrol exerts a range of protective biologic effects, but its mechanism(s) are not well understood. The aim of this study was to investigate how resveratrol could affect biologic pathways in oxidatively stressed RPE cells. Methods Cultured human RPE cells were treated with hydroquinone (HQ) in the presence or absence of resveratrol. Cell viability was determined with WST-1 reagent and trypan blue exclusion. Mitochondrial function was measured with the XFe24 Extracellular Flux Analyzer. Expression of heme oxygenase-1 (HO-1) and glutamate cysteine ligase catalytic subunit was evaluated by qPCR. Endoplasmic reticulum stress protein expression was measured by Western blot. Potential reactions between HQ and resveratrol were investigated using high-performance liquid chromatography mass spectrometry with resveratrol and additional oxidants for comparison. Results RPE cells treated with the combination of resveratrol and HQ had significantly increased cell viability and improved mitochondrial function when compared with HQ-treated cells alone. Resveratrol in combination with HQ significantly upregulated HO-1 mRNA expression above that of HQ-treated cells alone. Resveratrol in combination with HQ upregulated C/EBP homologous protein and spliced X-box binding protein 1. Additionally, new compounds were formed from resveratrol and HQ coincubation. Conclusions Resveratrol can ameliorate HQ-induced toxicity in RPE cells through improved mitochondrial bioenergetics, upregulated antioxidant genes, stimulated unfolded protein response, and direct oxidant interaction. This study provides insight into pathways through which resveratrol can protect RPE cells from oxidative damage, a factor thought to contribute to AMD pathogenesis.Purpose To examine 2-year progression rate and associated biometric changes in highly myopic eyes. Methods This is a longitudinal, observational cohort study that included 657 participants aged 7 to 70 years with bilateral high myopia (≤-6.00 diopters [D]) and followed for 2 years. All participants underwent ocular biometry and cycloplegic refraction examinations. Main outcome measures were changes in spherical equivalent refraction (SE) and ocular biometry in the right eyes. Results Mean age of participants was 21.6 ± 12.2 years. At baseline, mean SE was -9.82 ± 3.28 D and ocular biometric measurements were 27.40 ± 1.56 mm for axial length, 3.16 ± 0.27 mm for anterior chamber depth, 3.60 ± 0.35 mm for lens thickness, and 20.09 ± 1.50 mm for vitreous chamber depth. After 2 years of follow-up, there was a trend toward more myopia and greater axial elongation in all age groups. Younger participants (≤20 years) had significantly (P 1.00-D/y myopic shift (odds ratio, 1.11; 95% confidence interval, 1.04-1.18; P = 0.002). Conclusions Longitudinal data from this large Chinese cohort suggest that highly myopic eyes continue to progress in SE throughout life, with the greatest rates of progression observed in younger participants.
