Creating a story is a challenging task due to the the complex relations between the parts that make it up, which is why many new stories are built on those cohesive elements or patterns, called tropes that have been shown to work in the past. A trope is a recurring storytelling device or pattern, or sometimes a meta-element, used by the authors to express ideas that the audience can recognize or relate to, such as the Hero's Journey. Discovering tropes and how they cluster in popular works and doing it at scale to generate new plots may benefit writers; in this paper, we analyze them and use a principled procedure to identify trope combinations, or communities, that could possible be successful. The degree of development of these different communities can help us identify areas that are under-developed and, thus, susceptible to such a type of development. To detect these communities, with their associated degree of development and interest, we propose a methodology based on scientometric and complex network analysis techniques. As a secondary objective, we will obtain a general perspective in the trope and films network the tropesphere. We have used a dataset of 10,766 movies and 25,776 tropes associated with them, together with rating, genres and popularity. Our analysis has shown that not only there are different trope communities associated with specific genres, and that there are significant differences between the rating and popularity of these communities but also there are differences on the level of development between them emerging/declining, specific, transversal or motor.Combinatorial gut hormone therapy is one of the more promising strategies for identifying improved treatments for metabolic disease. Many approaches combine the established benefits of glucagon-like peptide-1 (GLP-1) agonism with one or more additional molecules with the aim of improving metabolic outcomes. Recent attention has been drawn to the glucose-dependent insulinotropic polypeptide (GIP) system due to compelling pre-clinical evidence describing the metabolic benefits of antagonising the GIP receptor (GIPR). We rationalised that benefit might be accrued from combining GIPR antagonism with GLP-1 agonism. Two GIPR peptide antagonists, GIPA-1 (mouse GIP(3-30)NH2) and GIPA-2 (NαAc-K10[γEγE-C16]-Arg18-hGIP(5-42)), were pharmacologically characterised and both exhibited potent antagonist properties. Acute in vivo administration of GIPA-1 during an oral glucose tolerance test (OGTT) had negligible effects on glucose tolerance and insulin in lean mice. In contrast, GIPA-2 impaired glucose tolerance and attenuated circulating insulin levels. A mouse model of diet-induced obesity (DIO) was used to investigate the potential metabolic benefits of chronic dosing of each antagonist, alone or in combination with liraglutide. Chronic administration studies showed expected effects of liraglutide, lowering food intake, body weight, fasting blood glucose and plasma insulin concentrations while improving glucose sensitivity, whereas delivery of either GIPR antagonist alone had negligible effects on these parameters. Interestingly, chronic dual therapy augmented insulin sensitizing effects and lowered plasma triglycerides and free-fatty acids, with more notable effects observed with GIPA-1 compared to GIPA-2. Vaccination status becomes more equitable when interventions are carried out to eliminate poverty or to improve levels in maternal education. Low-income countries need to identify interventions that would have a more immediate and equitable effect. The present study aimed to identify rapidly modifiable factors associated with full vaccination status among children in Niamey, Niger.

A cross-sectional, random cluster household survey was conducted in Niamey's five health districts. Data on vaccination coverage and socioeconomic household characteristics were collected. Logistic regression analysis was conducted with data on 445 mothers and their children aged 12-23 months.

Of 445 children, 38% were fully vaccinated. Mothers who were satisfied with their health worker's attitude and had correct vaccination calendar knowledge (adjusted odds ratio [aOR] 5.32, 95% confidence interval [CI] 2.05-13.82) were more likely to have fully vaccinated children. Mothers who had completed secondary school (aOR 2.04, 95% ter interpersonal communication between health workers and mothers. Strengthened communication interventions may be effective in improving both the acceptability of health services and low vaccination coverage.

To characterize the functional impairments of a cohort of patients undergoing inpatient rehabilitation after surviving severe COVID-19 illness, in order to better understand the ongoing needs of this patient population.

This study consisted of a retrospective chart review of consecutive patients hospitalized for COVID-19 and admitted to a regional inpatient rehabilitation hospital from April 29th to May 22nd, 2020. Patient demographics, clinical characteristics and complications from acute hospitalization were examined. Measures of fall risk (Berg Balance Scale), endurance (6 Minute Walk Test), gait speed (10 Meter Walk Test), mobility (transfer and ambulation independence), cognition, speech and swallowing (American Speech and Hearing Association National Outcomes Measurement System Functional Communication Measures) were assessed at rehabilitation admission and discharge.

The study population included 29 patients and was 70% male, 58.6% white and with a mean age of 59.5. The mean length of acute hospi and swallowing at admission and improved significantly in all of these domains by discharge. However, a significant number of patients exhibited residual deficits at discharge highlighting the post-acute care needs of this patient population.
