Dermatological and venereological expertise is indispensable for the medical care of our soldiers at home and during military missions and represents a major competence in military medicine. Therefore, in the new structure of the combined German Forces medical services, dermatological clinics have been set up at all Bundeswehr hospitals, which, as academic teaching hospitals, are regionally involved in the medical care of the civilian population. In addition to general dermatological and dermatosurgical care, wound treatment and tropical dermatology represent military medicine-related priorities. Outpatient dermatological treatment facilities complete the medical care at numerous locations where troops are based.PURPOSE There is no study exploring the positional relationship between the external acoustic meatus (EAM) and the sigmoid sinus (SS) in detail. The present study aimed to characterize the relationship using contrast magnetic resonance imaging (MRI). METHODS In total, 85 patients with an intact EAM, SS, and surrounding structures underwent thin-sliced, contrast MRI. Imaging data were transferred to a workstation for analysis. RESULTS In all patients, the EAM and SS were well depicted on both the sagittal and axial images. The relationships and distances between the EAM and SS, in addition to the shape of cross sections of the EAM, were highly variable with left-right asymmetry. The positional relationships between the EAM and the anterior edge of the SS were classified into superior, intervening, and inferior types. The intervening type was the most predominant, accounting for 85%. The shortest distance between the posterior wall of the EAM and the anterior margin of the SS was 12.3 ± 3.9 mm on the right side and 13.0 ± 2.9 mm on the left. In three women, the distance was less than 5 mm on the right side. CONCLUSIONS The positional relationship between the EAM and SS is highly variable and inconsistent. MATERIAL AND METHODS A total of 36 of 79 identified manuscripts met inclusion criteria, comprising 1863 ablations in 1798 patients. Inclusion criteria were (1) retrospective or prospective analysis of thermal ablation of osteoid osteomas in any location, (2) at least 6 months of clinical follow-up, (3) 10 or more patients, (4) patients not included in a second study included in this review, and (5) English language or English translation available. Success rate was defined as all ablations minus technical failures, clinical failures, and recurrences. Subset analysis of intra-articular and spinal tumors was performed. RESULTS Overall success rate was 91.9% (95% CI 91-93%). Technical failure, clinical failure, and recurrence rates were 0.3%, 2.1%, and 5.6% respectively. Complications were seen in 2.5% (95% CI 1.9-3.3%) patients. There was no significant difference when comparing radiofrequency ablation and cryoablation (p = 0.92). Success rates for intra-articular (radiofrequency ablation) and spinal tumors (radiofrequency and cryoablation) were 97% and 91.6% respectively. CONCLUSION Percutaneous ablation of osteoid osteomas was highly successful with low complication rates.
