Background Levothyroxine (LT4) is one of the most commonly prescribed medications. Although considered a life-long replacement therapy, LT4 therapy can be discontinued for some patients. This study aims at (i) reviewing the evidence on clinical outcomes of patients undergoing thyroid hormone replacement discontinuation, (ii) identifying the predictors of successful discontinuation, and (iii) systematically appraising frameworks used for deprescribing thyroid hormone. Methods We searched multiple bibliographic databases, including Ovid MEDLINE(R) and Epub Ahead of Print, In-Process & Other Non-Indexed Citations and Daily, Ovid Embase, Ovid Cochrane Central Register of Controlled Trials, Ovid Cochrane Database of Systematic Reviews, and Scopus, from inception to February 2020 for studies in which thyroid hormone replacement was discontinued. Clinical outcomes assessed included proportion of patients that remained euthyroid or needed to restart thyroid hormone replacement after discontinuation and frequency of cat up to a third of patients remained euthyroid after thyroid hormone discontinuation, with a higher proportion of patients with an initial diagnosis of SCH remaining euthyroid than patients with an initial diagnosis of OH. A deprescribing framework focusing on adequate selection of patients for deprescribing LT4 and a systematic process is warranted to guide clinicians in re-evaluating the need for LT4 in their patients.The occurrence of occupational accidents and injuries has always been a major concern for industrial management. Such undesirable incidences are higher in developing countries, especially in India, than in developed countries. This research aims to identify, analyze and evaluate the faulty behavior risks (FBRs) that trigger occupational accidents and injuries. Using a data triangulation strategy, this study identified 19 FBR factors under five categories. An integrated approach comprising the fuzzy analytic network process (ANP) and the decision-making trial and evaluation laboratory (DEMATEL) method is proposed for assessing these FBRs. The five most prominent critical risk factors are the absence of continuous monitoring, defective equipment and maintenance, cognitive bias, proper signage and adverse ambient working conditions. The study postulates some implications for industrial management to mitigate occupational accidents and injuries based on the outcomes.Mosquito-borne illnesses present a public health threat. This analysis quantifies general online mosquito media, and the Zika virus [ZIKV) specifically, from 7-20-2016 to 10-20-2018 in five U.S. geographies. The ZIKV sub-search comprised a shrinking share of online media about mosquitoes over time. Net sentiment, numerical scoring of search result positivity/negativity bounded between -100 and +100, was assessed. Mean net sentiment for the general mosquitoes search was -51; -55 for ZIKV. The ZIKV search revealed more variation in weekly net sentiment with a standard deviation of 14, compared to 10 for mosquitoes. Seventy-seven percent of the weeks had a net sentiment for the mosquito search that was more positive than the ZIKV search. For the 23% of the time the ZIKV search net sentiment was more positive than the general mosquito search, there were mentions of scientific advances, such as the potential for vaccine development associated with the post. Greater emphasis on public health threats from mosquitoes may be necessary to stimulate public action on mosquito-borne illness control. This analysis serves as an illustration of the potential for online/social media analysis to inform health officials of public interest/focus, and perhaps inform effective communication campaigns to combat public health threats.Objective Malignancy-associated hemophagocytic lymphohistiocytosis (M-HLH) in children is a relatively rare but life-threatening secondary hemophagocytic lymphohistiocytosis (sHLH). Until now, only a limited number of cases regarding children with M-HLH has been reported. Methods We conducted a retrospective study of 27 children with M-HLH, who admitted to our center between July 2007 and October 2019. The clinical data and laboratory data were analyzed. Results The median age of the children with M-HLH was 7 years. Underlying diseases included myeloid malignancy (n = 6), lymphoid malignancy (n = 18) and unknown type lymphoma (n = 3). The one-year mortality rate was 56%. All patients had persistent fever. The clinical manifestations included hepatomegaly (89%), splenomegaly (67%) and central nervous system symptoms (56%). Thirteen children (48%) had Epstein-Barr virus (EBV) infection. No significant differences were observed between EBV-positive and negative M-HLH patients in terms of most clinical indicators. However, EBV-positive M-HLH patients showed prolonged activated partial thromboplastin time (APTT) and more hemophagocytosis in the bone marrow (BM) in contrast to EBV-negative patients. Eighteen patients (67%) received the HLH-94/04 regimen as the initial treatment. There were no significant differences in the overall survival (OS) between EBV-positive and negative patients. Patients with prolonged APTT had a significantly poorer OS than other patients (p = 0.012). Conclusions The M-HLH children with EBV infection are more likely to have prolonged APTT and more hemophagocytosis in BM. However, little is known about how these pathways interact. Two preregistered experiments (total N = 1,510) examined effects of EC procedures (i.e., stimulus pairings) and EC instructions (i.e., instructions about stimulus pairings) on automatic and self-reported evaluations of social groups in the presence of more diagnostic information about the evaluative traits of those groups. Interestingly, both EC procedures and EC instructions still influenced automatic and self-reported evaluations when participants had read more diagnostic persuasive information. In line with predictions of propositional accounts of evaluation, EC instruction effects on automatic evaluations were not mediated by corresponding changes in self-reported evaluations. These results have theoretical implications and also highlight the important role that (instructions about) stimulus pairings have in social learning.The emergence of COVID-19 has changed the world as we know it, arguably none more so than for older people. In Sweden, the majority of COVID-19-related fatalities have been among people aged ⩾70 years, many of whom were receiving health and social care services. The pandemic has illuminated aspects within the care continuum requiring evaluative research, such as decision-making processes, the structure and organisation of care, and interventions within the complex public-health system. This short communication highlights several key areas for future interdisciplinary and multi-sectorial collaboration to improve health and social care services in Sweden. It also underlines that a valid, reliable and experiential evidence base is the sine qua non for evaluative research and effective public-health systems.This qualitative study explores perceptions of chronic kidney disease (CKD) among adults with abnormal estimated glomerular filtration rate (eGFR) in Guatemala, where the burden of CKD is rising. Qualitative semi-structured interviews were conducted with 39 individuals screened for CKD and found to have abnormal eGFR (defined as less then 90 mL/min/1.73 m2, per Kidney Disease Improving Global Outcomes [KDIGO] guidelines). Interviews occurred in participants' homes in Spanish or Kaqchikel Mayan. Interview notes were coded for dominant themes through an inductive approach. Interviewees had limited awareness of diabetes and hypertension as CKD risk factors, but appreciated the progressive nature of the disease. While most reported willingness to pursue renal replacement therapies, if necessary, they anticipated economic and geographic barriers. Public health interventions should focus on the association between diabetes, hypertension, and CKD. Improvement of primary care and screening infrastructure is imperative in CKD prevention in low- and middle-income countries (LMICs).The study was planned as a descriptive qualitative study to determine the emotional responses of neonatal intensive care nurses to work in the neonatal unit and to neonatal deaths. The sample of the study consisted of 7 nurses who work at the neonatal intensive care unit since data saturation was achieved. The data were collected using the "Semi-Structured In-Depth Interview Guide for Nurses". The data were analyzed using the content analysis method. Following codes were reached as a result of the study 'happiness-sadness', 'professional satisfaction-exhaustion', 'empathy', 'responsibility-guilt', 'hope-despair' under the theme of being a nurse at neonatal unit'; 'unforgettable first loss', 'professionalism in intervention-and then sadness, 'mature-premature difference', 'difficulty in giving hurtful news-inability to associate with death' and 'attachment-burnout' under the theme of experiencing neonatal loss.It was seen that nurses' emotions about working at neonatal intensive care unit were generally positive; however, these emotions changed to negative after neonate's relapsing and death.
A complicated mild traumatic brain injury (MTBI) is defined as mild by all clinical severity indicators but is complicated due to a traumatic intracranial abnormality visible on neuroimaging. Researchers have reported mixed findings regarding whether neuropsychological and functional outcomes following complicated MTBI are worse than, or similar to, outcomes following uncomplicated MTBI. This study examined patients referred from a Taiwanese emergency department to a neurosurgical outpatient clinic. Participants with complicated MTBI, uncomplicated MTBI, and those who did not undergo head computed tomography (CT) were compared on psychological, neuropsychological, and post-concussion symptom outcomes within 21days of injury.

=172) completed the Paced Auditory Serial Attention Test, Taiwanese Word Sequence Learning Test, a semantic Verbal Fluency Test, the Checklist of Post-Concussion Symptoms, and the Beck Depressmparable outcomes between those with complicated and uncomplicated MTBIs. This study is limited by small sample size and minimal information on intracranial abnormalities, broadly categorizing groups based on positive or negative neuroimaging as opposed to specific lesion types and locations.An experimental novel antiaging intervention strategy is based on the concept of parabiosis, which involves long-term treatment with factors derived from young blood facilitating rejuvenation of old individuals. In this study, we employed blood plasma from young rats as an intervention strategy to evaluate whether this could impact aging biomarkers in aged rats. The biomarkers studied include reactive oxygen species, the ferric reducing ability of plasma, plasma membrane redox system, reduced glutathione, malondialdehyde, protein carbonyl, and advanced oxidation protein products in blood. Additionally, the level of tumor necrosis factor-α and interleukin-6 were also estimated in blood. We found that old rats injected with plasma from young rats were protected from oxidative stress. Thus, this study provides some evidence of the rejuvenating effects of young plasma. We hypothesize that young plasma may contain certain "factors," which may be responsible for the observed effects. The mechanism of action is not clearly understood and is open to further studies.
