For many second generation KAs, identity and community were tied to religious life. We found that KAs perceived professional treatment and recovery resources to be underutilised. We offer insights about church-based participatory research as an approach to studying drinking among KAs.The Orchidaceae family presents one of the most extravagant pollination mechanisms deception. While many studies on reproductive success have been performed on food-deception orchids, less have been performed on sexually deceptive orchids. Here, we focused on Ophrys balearica P. Delforge, an endemic orchid of the Balearic Islands, to study its reproductive ecology, the spatio-temporal variation of its reproductive success and the individual (floral display and geospatial position) and population parameters (patch size, shape and density) that affect its reproductive success. We performed hand-pollination experiments, along with the recording of floral display parameters and GPS position of over 1,100 individuals from seven populations in two consecutive years. We applied, for the first time, GIS tools to analyse the effects of individual's position within the population on the reproductive success. The results confirm that this species is pollinator-dependent and mostly allogamous, but also self-compatible. This species showed high values for the cumulative inbreeding depression index and high pollen limitation. Male fitness was almost equal to female fitness between years and populations, and reproductive success exhibited huge spatio-temporal variation. Although we did not find strong correlations between floral display and reproductive success, patches with low-plant density and individuals in the external portion of the population showed significantly higher plant fitness. These findings must be considered in conservation actions for endangered orchid species, especially considering that most orchids are strongly dependent on pollinators for their species' fitness.Lamioideae comprised the second-largest subfamily in Lamiaceae. Although considerable progress has recently been made in the taxonomic study of Lamioideae, the subfamily remains one of the most poorly investigated subfamily in Lamiaceae. Therefore, the present study was designed with the aim to document the pollen micromorphology of some selected Lamioideae taxa and its taxonomic significance from Pakistan. Pollen micromorphological features were observed using scanning electron microscopy. The pollen grains are monad, tricolpate, radially/bilateral symmetrical. The pollen grains were small to medium-sized having oblate, oblate/subspheroidal, and subspheroidal shape. Exine sculpturing was observed as reticulate, microreticulate, and bireticulate. The colpus surface ornamentation was found as verrucate, gemmate, scabrate, and psilate. There was a considerable variation between the species in the micromorphology, that is, the coarseness of the reticulum, thickness of the muri comprising the reticulum and the number of secondary lumina per primary lumen. Hence, this study documented the pollen morphology of some selected taxa of the subfamily Lamioideae from Pakistan and strengthens the taxonomic identification of subfamily based on pollen characters, which helps in the correct identification, discrimination of the species of Lamioideae at generic and species level.
The "Bipolar Disorders Improving Diagnosis, Guidance, and Education" (BRIDGE-II-Mix) study aimed to estimate the frequency of mixed states in patients with a major depressive episode (MDE) according to different definitions and to compare their clinical validity, looking into specific features such as rapid cycling (RC).
Psychiatric symptoms, socio-demographic, and clinical variables were collected from a sample of 2811 MDE patients, of which 726 (25.8%) were diagnosed with bipolar disorder (BD). The characteristics of bipolar patients with RC (BD-RC) and without (BD-NRC) RC were compared.
Of 726 BD patients, 159 (21.9%) met DSM-5 criteria for RC. BD-RC group presented a higher number of lifetime depressive episodes (p<0.001) with shorter duration of depressive episodes, and more psychiatric comorbidities, as well as higher rates of atypical features (p=0.016) and concomitant (hypo)manic symptoms (irritable mood (p=0.001); risky behavior (p=0.005); impulsivity (p=0.006); and psychomotor agitation (p=esity, and had to be treated with more drugs.Evidence of self-help interventions for adjustment disorder (AjD) is limited. This study aims at testing in a randomized controlled trial (RCT) the effectiveness of a disorder-specific, Internet-delivered cognitive-behavioural therapy (ICBT) intervention for AjD. Participants were randomly allocated to either an ICBT with brief weekly telephone support (n = 34) or a waiting list group (n = 34). Beck's inventories for depression and anxiety were used as primary outcomes. The secondary outcomes were AjD symptoms, post-traumatic growth, positive and negative affect, and quality of life. In all, 76.5% of the participants completed the intervention. Compared with the control group, participants in the intervention condition showed significantly greater improvement in all outcomes (Cohen's d ranged from 0.54 to 1.21) except in anxiety symptoms measured by Beck Anxiety Inventory (d = 0.27). Only ICBT group showed a significant improvement in post-traumatic growth, positive and negative affect, and quality of life. The number of cases that achieved clinically meaningful change in all outcome measures was also higher in the ICBT group. All therapeutic gains were maintained at 3-, 6- and 12-month follow-ups. The current study provides evidence on the effectiveness of ICBT interventions to reduce the impact of AjD. Results suggest that brief self-help intervention with minimal therapist support is more effective than the mere passage of time in reducing the distress symptoms associated to the disorder and also can confer additional benefits.
Better understanding the incidence, predictors and mechanisms of early neurological deterioration (END) following intravenous thrombolysis (IVT) for acute stroke with mild symptoms and isolated internal carotid artery occlusion (iICAo) may inform therapeutic decisions.
From a multicenter retrospective database, we extracted all patients with both National Institutes of Health Stroke Scale (NIHSS) score <6 and iICAo (i.e. not involving the Willis circle) on admission imaging, intended for IVT alone. END was defined as ≥4 NIHSS points increase within 24h. END and no-END patients were compared for (i) pre-treatment clinical and imaging variables and (ii) occurrence of intracranial occlusion, carotid recanalization and parenchymal hemorrhage on follow-up imaging.
