Uncontrolled and inadequately managed asthma substantially reduces Quality of Life (QOL) and can lead to premature death. The aim of this study is to understand the role of montelukast in improving quality of life in asthmatic patients by comparing it with placebo.

This prospective, single-arm, interventional study was conducted from September 2019 to February 2020 in the pulmonology outpatient department of a tertiary care hospital in Pakistan. All patients were prescribed montelukast (10 mg once daily).

At day 28, participants had a higher score on the Asthma Quality of Life Questionnaire-Standard (AQLQ-S) overall and in all sub-domains compared to day 0. The improvement was significant overall and for the sub-domains of symptoms, activity limitations, and environmental stimuli.

Montelukast has an effective role in asthma control as well as in improving the quality of life in the Pakistani population.

Much attention has been paid to the association of low serum 25-hydroxyvitamin D (25(OH)D) levels and various chronic diseases, especially heart failure (HF). A clear role of vitamin D deficiency has been established, with increased mortality and morbidity in heart failures. Ivacaftor However, previous randomized control trials have failed to show improvement in clinical outcomes with calciferol supplementation in these patients. Therefore, it is still unclear whether calciferol therapy can be added to the standard care in congestive heart failure (CHF) patients with deficiency. Hence, to evaluate the role of vitamin D supplementation in CHF patients with low serum 25(OH)D, we conducted an extensive search in the PubMed and Google Scholar databases using various combinations of keywords. All potentially eligible studies that evaluated the effects of vitamin D supplementation on clinical outcomes in HF patients were retrieved and extensively studied. We also checked the references of all eligible studies to identify additional relevant publications. In this study, we reviewed various mechanisms of vitamin D affecting the cardiovascular system and examined the impact of deficiency on heart failures in terms of mortality and hospitalizations. In conclusion, vitamin D supplementation has failed to improve the clinical outcomes in HF patients. The possible long-term benefits of supplementation cannot be excluded. Therefore, for future clinical trials, we recommend considering large sample sizes, longer follow-up durations, along with optimal dosage and appropriate dosing frequency.Dyshidrotic eczema (DE) or acute palmoplantar eczema is a common cause of hand and foot dermatitis in adults. It is a recurrent vesicular eruption affecting the soles, palms, or both. It is very pruriginous and generally appears suddenly. It creates vesicles that, on physical examination, can look similar to "tapioca pudding", which is the characteristic clinical feature of this disorder. It is more common in young adults and affects men and women equally. In this report, we present the case of a 56-year-old man with no relevant past medical history who presented to the hospital with vesicular lesions in his hands and maculopapular lesions in his arms and legs. The patient had characteristic lesions in his right hand consistent with DE and negative workup for bullous pemphigoid, scabies, and bacterial, fungal, and viral infections.Introduction We aimed to characterize the most influential radiation oncologists on Twitter, the correlation between their Twitter activity and their academic profiles as measured by the Scopus H-index as well as their activity around the American Society for Radiation Oncologists (ASTRO) 2018 meeting. Methods We defined radiation oncology influencers as any radiation oncologist with 500 or more followers on Twitter through the first two weeks of August 2019. We collected their available characteristics, their Scopus H-index, and Twitter metrics. We examined their general Twitter activity as well as their specific activity before, during, and after the 2018 ASTRO annual meeting. We identified the most frequent tweet content categories for each influencer. Results We identified 48 radiation oncologist influencers; 79% were male, 75% were based in the United States, and 94% were affiliated with an academic center. Among them, 44% had high H-indices of ≥21, an average value in academic faculty for full professors or department heads. There were no correlations between H-index and Twitter metrics such as the number of individuals the influencer was following (p = 0.58), the number of followers (p = 0.66), the number of tweets (p = 0.88), and the number of likes (p = 0.54). During the period around ASTRO 2018, the mean number of tweets per influencer was 4437 (range 87-93,000). Conclusion Current radiation oncology influencers are predominantly North American males from academic institutions. A correlation between academic productivity as measured by the H-index and Twitter metrics was not demonstrated. The fact that some influencers had a low H-index supports that a high academic profile as measured by traditional metrics is not necessary to have a voice in the Twitter radiation oncology community.Introduction Studies have reported conflicting results regarding the effect of smoking on outcome in coronavirus disease 2019 (COVID-19) patients, but the results have been conflicting. In this meta-analysis, we systematically examined the association between smoking and mortality in COVID-19. Methods PubMed database was searched to look for relevant articles. Inclusion criteria were as follows (1) cohort studies or case series studies; (2) study population included individuals with a confirmed COVID-19 infection; (3) the status of smoking was reported, regardless if it was current or in the past; and (4) mortality among smokers was reported in the study or could be calculated and compared to non-smokers. Mortality rates were pooled using a random effects model. Risk ratio (RR) and its 95% confidence interval (CI) were also calculated using the same model. Another meta-analysis was then performed to assess the difference in mortality between current and former smokers. Results Ten studies with a total of 11,189 patients were included.
