Background Bed bugs are common urban pests associated with stress, anxiety, depression, and some reports of suicidality. The emergency department (ED) treats patients with both acute mental illness and bed bug infestations. There have been few studies examining associations between bed bug infestations and mental illness. Methods A case-control study involving 332 adult ED patients infested with bed bugs and 4,952 uninfested control patients matched on sex, age (±1 year at the time of the ED visit), and the specific ED was completed. All clinic encounters occurred in northeast Ohio between February 1, 2011, and February 1, 2017, from a single health system. Univariable and multivariable regression analysis looked for associations between bed bug infestation and different psychiatric diagnoses and medications. Results Bed bug infested patients were more likely than uninfested patients to screen positive for an unsafe home and needing an abuse consult at ED triage (P≤.03 for both). ED psychiatric evaluations wea were more common among ED patients with bed bugs, but these associations were no longer significant on multivariable analysis. These associations likely reflect the complex relationships between socioeconomic factors, health disparities, mental illness, and having a bed bug infestation.Objectives The objective of this study is to compare various dual blade vitrectomy cutters for their efficiency in an in vitro setting. Methods In this in vitro experimental study, we compared various vitrectomy systems including EVA (Dutch Ophthalmic Research Center, Zuidland, The Netherlands), REVOLUTION (Optikon 2000, Inc., Rome, Italy), and OS4 (Oertli Instrumente AG, Berneck, Switzerland) in terms of efficiency in vitreous cutting and aspiration for various vitreous substitutes. These substitutes included water, chicken egg albumin, and goat vitreous. We only used 23-gauge dual blade cutters across all platforms to maintain uniformity. The cutting and aspiration efficiency was measured across various cut and vacuum settings of vitrectomy machines and measured as mass aspirated in a given time. Data analysis included comparing the amount of mass aspirated by these machines at preset cut and vacuum settings. Results Scatter plots showed a comparable mass of water aspirated by the EVA and REVOLUTION at 1000 to 5000 cuts per minute at a constant vacuum of 500 mm Hg whereas OS4 aspirated lesser mass at similar settings. Same trends were noted for goat vitreous for EVA and REVOLUTION but aspirated mass of albumin fluctuated widely across various platforms. At peak machine settings, REVOLUTION showed superiority across all three vitreous models due to its higher peak settings. Material and methods Fifteen consecutive patients with VAH (mean age 54 ± 21 years) and 15 sex-matched controls without VAH (mean age 54 ± 22 years) were selected for the study. The vertebral arteries (VA) were examined with Doppler ultrasound. We also explored each group for sex and age differences (young age ≤ 50, old age >50). Results The mean RI (MRI), right RI (RRI), left RI (LRI), non-dominant-side RI, and dominant-side RI were significantly higher in the Case Group than the Control Group. In the Case Group, the affected-side RI (A.RI) was significantly higher than the normal side, while the normal side peak systolic velocity was significantly higher than the affected side. The MRI and A.RI were significantly higher in older patients. We also found a significant negative correlation between the mean diameter (MD) and MRI. MRI and A.RI both correlated positively with age in the Case Group, while left peak systolic velocity decreased significantly with age in the Control Group [p-values less then 0.05]. Conclusion The dominant VA had a higher RI in the Case Group than the Control Group. Within this literature review, we highlight the importance, efficacy, and safety of influenza vaccination in individuals with autoimmune diseases, including systemic lupus erythematosus (SLE), Sjogren syndrome (SS), rheumatoid arthritis (RA), and inflammatory bowel disease (IBD) in both vaccinated and unvaccinated individuals. Overall, vaccination is generally well tolerated by SLE patients and the literature recommends the inactivated influenza vaccine to SLE patients according to the recommendations and schedules for the general population and annually against seasonal influenza viruses. While the data are still unclear in patients with SS, there does seem to be a general consensus to vaccinate these individuals to prevent harmful risks of influenza disease. In patients with RA and IBD, vaccination efficacy with the inactivated influenza vaccine should be determined on a case-by-case basis, taking patient therapy into account. In light of the current pandemic and global coronavirus disease 2019 (COVID-19) crisis, it is crucial to emphasize the safety and immunogenicity of influenza vaccination in vulnerable individuals suffering from autoimmune diseases. Public health measures are recommended to protect these individuals with vaccinations, keeping in mind the possibility of the multiple COVID-19 vaccines that are currently available.
