Differential methylation among tumor entities relates to differences in cellular origin and to de novo epigenetic alterations, which allowed us to build an accurate machine learning-based diagnostic algorithm. We identify extensive patient-specific methylation variability in silenced chromatin associated with the proliferative history of normal and neoplastic B cells. Mitotic activity generally leaves both hyper- and hypomethylation imprints, but some B-cell neoplasms preferentially gain or lose DNA methylation. Subsequently, we construct a DNA methylation-based mitotic clock called epiCMIT, whose lapse magnitude represents a strong independent prognostic variable in B-cell tumors and is associated with particular driver genetic alterations. Our findings reveal DNA methylation as a holistic tracer of B-cell tumor developmental history, with implications in the differential diagnosis and prediction of clinical outcome.A new species of entomopathogenic nematodes, Steinernema sandneri n. sp., was recovered by baiting from Poland. Its morphological traits indicate that the new species is a member of the feltiae-kraussei group. The results provided in this report indicate that G. mutator is involved in the transmission cycle of Gongylonema sp. to vertebrates in Europe.In conventional coffee farming, soil fauna can be negatively affected by the intensive management practices adopted and the use of an agroforestry system (AFS) is an alternative to reduce these impacts. In coffee AFS, soil nutrition is provided mainly using organic fertilizers. This soil management favors the microbiota and can alter the population dynamics of some organisms. Our objective was to evaluate the effect of organic fertilizers on the nematode community in coffee AFS and to determine their impact on soil ecology. Soil samples were collected from three coffee AFS and a nearby Atlantic rainforest fragment. Nematodes were extracted from the samples and identified to the genus. The identified populations were compared using several community and diversity indices to determine the environmental conditions of the systems under evaluation. No differences in total abundance among nematode communities were found in the four areas evaluated. Regarding trophic groups, the coffee AFS treated with either cow manure or poultry litter favored the trophic group of bacterivores. Plant-parasitic nematodes were more abundant in soils of both the naturally fertilized coffee AFS and the Atlantic rainforest fragment. The maturity and structural indexes indicated that the Atlantic rainforest fragment and the naturally fertilized coffee AFS had similar ecological functions. On the other hand, soils fertilized with cow manure were less diverse, had higher dominance in the community, and showed less ecological stability. The nematode communities found in the AFS were similar to those seen in the forest fragment indicating that is possible to produce coffee sustainably without negatively affecting soil quality.

To compare patient management and outcome during the first and second waves of the coronavirus 2019 pandemic.

Single-center prospective cohort study.

All adult patients admitted in either the first (from March 15 to May 15, 2020) or second (from October 1 to November 30, 2020) wave of coronavirus disease 2019.

Primary outcome was 30-day mortality. During the second wave of the coronavirus disease 2019 pandemic, 33 patients (4.8%) were transferred due to overcrowding and excluded from analysis. There were 341 (first wave of the coronavirus disease 2019 pandemic) and 695 (second wave of the coronavirus disease 2019 pandemic) coronavirus disease 2019 patients admitted to the hospital, with median age first wave of the coronavirus disease 2019 pandemic as 68 (57-80) and second wave of the coronavirus disease 2019 pandemic as 71 (60-80) (

The current standard of care to deliver invasive mechanical ventilation support is the protective ventilation approach. One pillar of this approach is the limitation of tidal volume to less than 6 mL/Kg of predicted body weight. Predicted body weight is calculated from patient's height. Yet, little is known about the potential impact of errors arising from visual height estimation, a common practice, to calculate tidal volumes. The aim of this study was to evaluate that impact on tidal volume calculation to use during protective ventilation.

Prospective observational study.

An eight-bed polyvalent ICU.

Tidal volumes were calculated from visual height estimates made by physicians, nurses, and patients themselves and compared with tidal volumes calculated from measured heights. Comparisons were made using the paired t test. Modified Bland-Altman plots were used to assess agreement between height estimates and measurements. One-hundred patients were recruited. Regardlete tidal volumes. Additionally, this approach can be harmful and potentially increase mortality by exposing patients to tidal volumes greater than or equal to 6.5 mL/Kg predicted body weight. In the interest of patient safety, every ICU patient should have his or her height accurately measured.

To understand the unmet needs of caregivers of ICU survivors, how they accessed support post ICU, and the key components of beneficial ICU recovery support systems as identified from a caregiver perspective.

International, qualitative study.

We conducted 20 semistructured interviews with a diverse group of caregivers in the United States, the United Kingdom, and Australia, 11 of whom had interacted with an ICU recovery program.

Content analysis was used to explore prevalent themes related to unmet needs, as well as perceived strategies to improve ICU outcomes. Post-ICU care was perceived to be generally inadequate. Desired caregiver support fell into two main categories practical support and emotional support. Successful care delivery initiatives included structured programs, such as post discharge telephone calls, home health programs, post-ICU clinics, and peer support groups, and standing information resources, such as written educational materials and online resources.

This qualitative, multicenter, international study of caregivers of critical illness survivors identified consistently unmet needs, means by which caregivers accessed support post ICU, and several care mechanisms identified by caregivers as supporting optimal ICU recovery.
