The effect of prone positioning (PP) on respiratory mechanics remains uncertain in patients with severe acute respiratory distress syndrome (ARDS) requiring venovenous extracorporeal membrane oxygenation (VV-ECMO).

We prospectively analyzed the effects of PP on respiratory mechanics from continuous data with over a thousand time points during 16-h PP sessions in patients with COVID-19 and ARDS under VV-ECMO conditions. The evolution of respiratory mechanical and oxygenation parameters during the PP sessions was evaluated by dividing each PP session into four time quartiles first quartile 0-4 h, second quartile 4-8 h, third quartile 8-12 h, and fourth quartile 12-16 h.

In this study, we focused on circulating monocyte and lymphocyte counts and their variation tendency in the early stage of ILD. A total of 351 patients from two inception anti-MDA5 antibody-positive cohorts were included in this study, with various treatment choices. Lymphocyte count remained lower in the first month after admission in the non-survivor patients. Achieving a goal of serum urate levels in patients with gout is an important way to prevent gout and its complications while it remains difficult with a low targeting rate worldwidely. Currently, hyperuricemia classification has not been widely applied to the management of gout owing to insufficient clinical evidences. This study aimed to evaluate the effectiveness of achieving target urate based on hyperuricemia classification in Chinese patients with gout.

In this prospective study, patients with gout receiving urate lowering therapy with benzbromarone were assigned to two groups, a renal underexcretion and an unclassified type. The primary endpoint was the proportion of patients achieving the serum urate target (<360 μmol/L) during the 12-week study. The frequency of acute gout attacks as well as physical and chemical indicators were secondary endpoints.

In this large-scale longitudinal cohort study, we tried to evaluate the association between different HRT types/methods use and risk of EC, and reveal this risk within different body mass index (BMI) groups. In whole cohort, 677 EC occurred during mean 11.6 years follow-up. Cox proportional hazards regression was used to estimate multivariable-adjusted hazards ratios (HRs) and 95% confidence intervals (CIs) with HRT status (never, former, or current) for risk of EC incidence. Current HRT use was not significantly associated with EC risk (HR for current vs. never HRT use 1.13; 95% CI 0.92, 1.38) in the whole cohort, but presented a dose-response effect on increased EC risk (HR for >10-year use vs. never HRT use 1.73; 95% CI 1.35, 2.21). Moreover, EC risk differed in distinct regimens or subsets (all P interaction 30 kg/m2) who currently use estrogen-only harbored decreased EC risk (HR 0.56; 95% CI 0.38, 0.82) compared to counterparts without HRT use. Estrogen-only use is associated with increased EC risk, and precise monitoring of EC development for postmenopausal women with long-term HRT use are urgently needed. BMI could serve as an important surrogate to assess this risk.

To explore the long-term efficacy of novel choroidal suturing methods including trans-scleral mattress suturing (TSS) and intraocular suturing (IOS) in the treatment of choroidal avulsion.

Prospective cohort, hospital-based study.

A total of 24 patients who were diagnosed with choroidal avulsion were enrolled in this study. The demographic characteristics, baseline information of trauma, best-corrected visual acuity (BCVA), and intraocular pressure (IOP) were collected before surgery, and the anatomic abnormities of the globe were recorded before or during surgery. All patients were diagnosed with choroidal avulsion and underwent choroid suturing treatment during vitrectomy, postoperative functional variables including BCVA and IOP, anatomic variables including retinal and choroidal reattachment rate, and silicone oil migration rate, which were recorded at the regular follow-ups at least 1 year after surgery.

This review was designed to compile the evidence on the prognosis of patients with colorectal cancer and apical lymph node (APN) metastasis and the long-term benefit of inferior mesenteric artery lymph node (IMA-LN) resection.

We searched the PubMed Central, Cochrane library, EMBASE, and MEDLINE databases from inception until May 2021 for relevant publications. We assess the quality of the studies using the Newcastle Ottawa scale. We conducted a random-effects model meta-analysis and report pooled hazard ratios (HRs) with 95% confidence intervals (CIs).

We analyzed data from 13 studies conducted in Japan, China, and Korea with 6,193 participants. Most studies were retrospective in nature and of low quality. We found that patients with APN metastasis had shorter OSs (pooled HR, 2.41; 95% CI, 1.92-3.02) and PFSs (pooled HR, 2.42; 95% CI, 1.90-3.09) than the patients without the metastasis. We identified significant heterogeneity without publication bias for both outcomes. Moreover, our sensitivity analysis revealed robust estimates were robust for the individual effects.
