The objectives of the study were to determine the effect of coated or noncoated hormone implants on growth performance, carcass characteristics, and serum estradiol-17β (E2) concentrations of Holstein steers fed a grain-based diet for 112 d. Seventy-nine Holstein steers [average initial body weight (BW) = 452 ± 5.5 kg] were stratified by BW and allotted to one of two treatments 1) Holstein steers implanted with a coated implant containing 200 mg of trenbolone acetate (TBA) and 40 mg E2 (Revalor-XS (Merck Animal Health; Summit, NJ)] on day 0 (XS) or 2) Holstein steers implanted two times (days 0 and 56) with a noncoated implant containing 80 mg of TBA and 16 mg of E2 [(2IS) Revalor-IS (Merck Animal Health)]. Data were analyzed using the MIXED procedure of SAS (SAS Inst. Inc., Cary, NC). There was no effect (P ≥ 0.71) of implant strategy on initial, middle, and final BW. No effect (P ≥ 0.12) of implant strategy was observed on average daily gain, dry matter intake, or gain-to-feed ratio. There were no effects (P ≥ 0.11) of implant strategy on carcass characteristics. There was an implant × day interaction (P less then 0.01) for the circulation of serum E2 concentrations. Serum E2 concentration increased similarly 14 d after Holstein steers were implanted, regardless of implant strategy. At 28 d, after steers were implanted, steers in the XS group had less serum E2 concentration than Holstein steers in the 2IS group. However, at 56 d after the first implantation, both groups, once again, had similar serum E2 concentrations and E2 concentrations were less on day 56 than day 28 for both strategies. Holstein steers implanted with 2IS had greater serum E2 concentration on day 70 and E2 concentrations remained greater than serum E2 of Holstein steers implanted XS for the duration of the trial (day 112). In summary, there was no effect of coated or two doses of noncoated implant on growth performance or carcass characteristics of Holstein steers.A digestibility experiment was conducted to evaluate the effects of dietary exogenous monocomponent protease on the coefficient of apparent total tract digestibility (ATTD) and coefficient apparent ileal digestibility (AID) of amino acids in meat and bones meal (MBM), poultry byproducts meal (PBPM), and feather meal (FM). A total of 512 Cobb-500 male broiler chickens (aged 14 d) were randomly placed into 64 metabolism cages (8 birds per pen) and were allocated to eight treatments with eight replicates in a semi-controlled environmental room. The experimental diets consisted of the basal diet (corn/SBM) and the replacement of 300 g/kg on a weight basis with MBM, PBPM, or FM. The excreta were collected during 3 d (19 to 21 d), and the ileal digesta (using Celite as an indigestible marker) only 1 d (21 d). The protease contained 75,000 PROT units/g. The use of the enzyme increased (P less then 0.05) ATTD for alanine, cysteine, glycine, and threonine in the basal diet and AID for the amino acids alanine, cysteine, glycine, lysine, threonine, and valine for the basal diet (vegetable). Regarding meals, there was an increase (P less then 0.05) in the amino acid digestibility in large part due to the amino acids of MBM (14) and PBPM (9), with only five amino acids for FM. The amino acids glycine and threonine showed increases (P less then 0.05) in both total and ileal digestibility for all animal meals and for the diet based on corn and soybean meal, which indicates a high specificity of the enzyme for these amino acids. The overall results obtained in this study found satisfactory effectiveness of this exogenous protease. One of the largest factors negatively influencing reproduction and economics in the sheep industry is gastrointestinal nematode (GIN) parasites. Due to anthelmintic resistance of these parasites, animals are selected for parasite resistance using FEC and FAMACHA scores. Data were used from the Virginia Tech Southwest Agricultural Research and Extension Center Ram Test in Glade Spring, VA, from the year 2012 to 2018 in which animals were tested in 14-d intervals for 70 d. Mixed models for repeated weight measurements were made from backward stepwise selection to evaluate the relationships between weight and GIN FEC. A total of 576 animals within 23 contemporary groups derived from test year and pasture group were analyzed. Ram, contemporary group, and consignor were considered random effects, and fixed effects were birth type, test day, age, age squared, starting weight, FEC, and FAMACHA score. Pairwise contrasts were used in the statistical analysis of parameters and their interactions. Weight and age were found to have a quadratic relationship. Increased FEC was associated with weight loss at a rate of 0.00030 kg/FEC (P less then 0.0001). Animals dewormed at any point during the trial weighed less than those that were not and increased with test day to a maximum difference of 4.66 kg (P less then 0.001). FAMACHA score was found to be significant (P less then 0.05), but a direct relationship with weight was not conclusive. To describe telemedicine utilization in neurosurgery at a single tertiary institution to provide outpatient care during the coronavirus disease 2019 (COVID-19) pandemic, with 315 telemedicine visits performed by the neurosurgery department.

In response to the COVID-19 pandemic national stay-at-home orders and postponed elective surgeries, we converted upcoming clinic visits into telemedicine visits and rescheduled other patients thought not to be markedly affected by surgical postponement. We reviewed the charts of all patients who had telehealth visits from April 1 through April 30, 2020, and collected demographic information, diagnosis, type of visit, and whether they received surgery; a satisfaction questionnaire was also administered.

In March 2020, 94% (644 of 685) of the neurosurgery clinic visits were face-to-face, whereas in April 2020, 55% (315 of 573) of the visits were telemedicine (

In April, of the 315 telemedicine visits, 172 (55%) were phone consults and 143 (45%) video consul tool for preoperative consultation, postoperative and follow-up visits during the COVID-19 pandemic, and decreased risks of exposure to severe acute respiratory syndrome coronavirus 2 to patients and health care staff. Future larger studies should investigate the cost-effectiveness of telemedicine used to triage surgical from nonsurgical patients, potential cost-savings from reducing travel burdens and lost work time, improved access, reduced wait times, and impact on patient satisfaction.Lambert-Eaton myasthenic syndrome (LEMS) is a rare, autoimmune or paraneoplastic condition characterized by muscle weakness and fatigability. In cancer therapy, immune checkpoint inhibitors (ICI) sensitize the immune system for tumor antigens. We report a 62-year-old, female patient with paraneoplastic LEMS as first manifestation of Merkel cell carcinoma. Under avelumab, the LEMS exacerbated with worsening of limb weakness and a severely reduced vital capacity ( less then  1 l). To treat this immunological side effect, we added a regimen with intravenous immunoglobulins. Hereby, the LEMS improved significantly. As we were able to continue the cancer treatment, the Merkel cell carcinoma has been in remission so far. This is the first description of paraneoplastic LEMS, avelumab, and Merkel cell carcinoma. link3 We conclude that immunoglobulins are an option to control an ICI-associated deterioration of paraneoplastic symptoms.

Sphingosin-1-phosphate (S1P) plays a crucial role as a signaling molecule in the immune system and the vasculature. Previous studies suggested a role as a vasoconstrictor of cerebral arteries via the S1P3-Receptor. Cerebral vasospasm (VS) following aneurysmal subarachnoid hemorrhage (SAH) is a major cause of disability and poor neurological outcome. Early detection of vasospasm could facilitate the prevention of cerebral ischemia in SAH patients. The aim of this prospective case-control study was to characterize the dynamics of S1P in the cerebrospinal fluid (CSF) of patients with SAH in relation to hemorrhage volume, the occurrence of VS, and neurological outcome.

S1P levels in CSF of 18 control subjects and 18 SAH patients with placement of an external ventricular drainage (EVD) were determined by high sensitivity mass spectrometry from day 1 through 14 after SAH onset. Hemorrhage volume, development of asymptomatic vasospasm (aVS) and symptomatic vasospasm (sVS), and neurological outcome were correlated to day 1 S1P levels.
