Cardiac resynchronization therapy (CRT) and left ventricular assist devices (LVAD) improve outcomes in heart failure patients. Early ventricular arrhythmias (VA) are common after LVAD and are associated with increased mortality. The association between left ventricular pacing (LVP) with CRT and VAs in the early post-LVAD period remains unclear.
This was a retrospective study of all patients undergoing LVAD implantation from 1/2016 to 12/2019. Patients were divided into those with CRT and active LVP (CRT-LVP) immediately post-LVAD implant versus those without CRT-LVP. Implantable cardiac defibrillator electrograms were reviewed and early VAs were defined as sustained ventricular tachycardia (VT)/ventricular fibrillation occurring within 30 days of LVAD implantation.
Of 186 included patients (mean age 53 years, 75% male, mean body mass index 28), 72 had CRT devices, 63 of whom had LV pacing enabled after LVAD implant (CRT-LVP group). Patients with CRT-LVP were more likely to have VA in the early postoperative period (21% vs. 4%; p = .0001). All 9 patients with CRT in whom LVP was disabled had no early VA. Among those with early VA, patients with CRT-LVP were more likely to have monomorphic VT (77% vs. 40%; p = .07). In multiple logistic regression, CRT-LVP pacing remained an independent predictor of early VA after adjustment for history of VA and AF.

Dairy manuresheds can be contained on-farm but may need to involve additional farms that can assimilate excess nutrients. We present case studies reviewing challenges and opportunities to manureshed management in four major dairy-producing states using available information on local manuresheds. Additionally, geographic information system software was used with data from regulated Minnesota dairies to assess cropland assimilative capacities and transport needs surrounding large dairies. Manureshed requirements vary across regions, but increased import of feed and soil phosphorus accumulation constrain on-farm manure utilization across the United States. In Minnesota, a growing proportion of Jersey cattle and differences in continuous corn (Zea mays L.) vs. corn-alfafa (Medicago sativa L.) rotations contribute to the amount of land needed to absorb dairy manure nutrients. Farm-gate budgets reveal that N-based manuresheds can be contained within Idaho dairies, but P-based manuresheds extend beyond the farm. In New Mexico, relocation of surplus manure nutrients off the farm is common via informal networks, but incentives to strengthen these networks could ensure sustainable manureshed management. Evaluation of manureshed requirements in Pennsylvania is often complicated by the need for additional nutrient management planning and greater understanding of nutrient balances on the preponderance of small dairies. Nutrient imbalances with highly concentrated dairy production often lead to the need for manure transport off-farm. However, advances in herd and cropland management offer opportunities to improve on-farm nutrient efficiencies, and emerging networks and technologies promise to facilitate manure export when needed.
Recent reports suggest an increased rate of early structural valve degeneration (SVD) in the Trifecta bioprosthesis (Abbott Cardiovascular). We sought to compare the intermediate-term outcomes of the Magna (Edwards Life Sciences) and Trifecta valves.
All surgical aortic valve replacements (SAVRs) with Trifecta or Magna/Magna Ease bioprostheses at an academic medical center were extracted from an institutional database. Patients who survived until after discharge (2011-2019) were included. The primary outcome was valve failure for any reason requiring reintervention or contributing to death, identified by reintervention or review of cause of death. Time to failure was estimated with Kaplan-Meier analysis and Cox Proportional Hazards Modeling.
Out of 1444 patients, 521 (36%) underwent Trifecta and 923 (64%) underwent Magna implantation with a median follow-up of 27.6 months. Trifecta patients had larger median valve size (25vs. 23 mm, p < .001) and lower median gradient (8.0vs. 10.9 mmHg, p < .001). Trifecta patients had higher 48-month estimated failure rates (20.2 ± 7.6% vs. 2.6 ± 0.7%, p < .0001), with failure rates of 21.4 versus 9.2 failures per 1000 person-years (p < .001). After risk-adjustment, Trifecta patients had a 5.3 times hazard of failure (95% confidence interval 2.78-12.34, p < .001) compared to Magna patients. Only Trifecta valves failed due to sudden aortic regurgitation, 8 out of 521 (1.5%).
Despite lower postoperative mean gradients, the Trifecta bioprosthesis may have an increased risk of intermediate-term SVD. Further research is warranted to confirm the potential for sudden valve failure.

Further research is warranted to confirm the potential for sudden valve failure.In recent years, the performance of perovskite quantum dots (QDs) and QD-based light-emitting diodes (QLEDs) has improved greatly, with electroluminescence (EL) efficiency of green and red emission exceeding 20%. However, the development of perovskite near-infrared (NIR) QLEDs has reached stagnation, where the reported maximum EL efficiency is still below 6%, limiting their further applications. In this work, new NIR-emissive FAPbI3 QDs are developed by post-treating long alkyl-encapsulated QDs with 2-phenylethylammonium iodide (PEAI). The incorporation of PEAI reduces the QD surface defects for giving a high photoluminescence quantum yield up to 61.6%. The n-octane solution of PEAI-passivated FAPbI3 QDs is spin coated on top of the PEDOTPSS-treated ITO electrode modified with a thermally crosslinked hole-transporting layer to give a full-coverage, smooth, and dense QD film. Incorporating with an effective electron-transporting material, CN-T2T, which has deep lowest unoccupied molecular orbital and good electron mobility, the optimal device with EL λmax at 772 nm achieves an external quantum efficiency up to 15.4% at a current density of 0.54 mA cm-2 (2.6 V), which is the highest efficiency ever reported for perovskite-based NIR QLEDs. Therefore, examining the associations between parental care and color patterns may provide key insights into evolutionary patterns and selection pressures for parental care. Our comparative analysis of 988 anuran species reveals that dichromatic species are less likely to provide parental care, irrespective of the caregiving sex, and are more likely to breed in aquatic habitats. We then examined whether dorsal colors and patterns that enhance crypticity or function as aposematic signals are associated with the caregiving sex, and the modality of care (transport or stationary). Only caregiving males are more likely to have dorsal Stripes, but none of the colors (Green-Brown, Red, Yellow, Blue-Black) and other patterns (Plain, Bands, Spots, Mottled-Patches) were associated with caregiving females or the modality of care. Overall, sexual dichromatism, breeding ecology, and parental care are associated, but the evolution of caregiving behavior does not appear to influence the myriad colors and patterns characteristic of anurans globally.Medial proximal sesamoid bones (PSBs) from Thoroughbred racehorses that did (Case) or did not (Control) experience unilateral biaxial PSB fracture were evaluated for bone volume fraction (BVF), apparent mineral density (AMD), tissue mineral density (TMD), and microdamage in Case fractured, Case contralateral limb intact, and Control bones. A majority of Case bones had a subchondral lesion with high microdamage density, and low BVF, AMD, and TMD. Lesion microdamage and densitometric measures were associated with training history by robust linear regression. Exercise intensity was negatively related to BVF (0.07 ≤ R2 ≤ 0.12) and positively related to microcrack areal density (0.21 ≤ R2 ≤ 0.29) in the lesion; however, in an undamaged site, the relationships were opposite in direction. Regardless of location, TMD decreased with event frequency for both Case and Control, suggesting increased bone remodeling with exercise. Measures of how often animals were removed from active training (layups) predicted a decrease in TMD, AMD, BVF, and microdamage at regions away from the lesion site. A steady-state compartment model was used to organize the differences in the correlations between variables within the data set. The overall conclusions are that at the osteopenic lesion site, repair of microdamage by remodeling was not successful (e.g., lower bone mass, increased damage, and lower mineralization) but that in regions away from the lesion remodeling successfully controlled damage (e.g., higher bone mass, less microdamage, and lower mineralization).In this paper, we take up the call to further examine structural injustice in health, and racial inequalities in particular. We examine the many facets of racism structural, interpersonal and institutional as they appeared in the COVID-19 pandemic in the UK, and emphasize the relevance of their systemic character. We suggest that such inequalities were entirely foreseeable, for their causal mechanisms are deeply ingrained in our social structures. It is by recognizing the conventional, un-extraordinary nature of racism within social systems that we can begin to address socially mediated health inequalities.
