Since the outbreak of the coronavirus pandemic in 2019 (COVID-19), healthcare systems around the world have been hit to varying degrees. The association between advanced age and postoperative morbidity and mortality after major gastroenterological surgeries remains unclear. This study aimed to assess the association between old age and the short-term postoperative outcomes of gastroenterological surgeries.

We evaluated 327,642 patients who underwent any of the seven major gastroenterological surgeries-esophagectomy, total gastrectomy, distal gastrectomy, right hemicolectomy, low anterior resection, hepatectomy, and pancreatoduodenectomy-and were registered with the Japanese national surgical registry between January 2011 and December 2013. Perioperative characteristics, frequency/nature of postoperative morbidities, and postoperative mortality were compared according to age at the time of surgery.

Overall, 18% (59,182/327,642) of the entire cohort were aged ≥ 80years. The overall mortality rates in the entire cohort and in those aged ≥ 80years were 1.7% and 3.3%, respectively. This prospective cohort study used data from the US National Health and Nutrition Examination Survey. Diet was assessed using 24h dietary recalls. We created three plant-based diet indices including an overall plant-based diet index (PDI), a healthful plant-based diet index (hPDI), and an unhealthful plant-based diet index (uPDI). Deaths from baseline until December 31, 2015, were identified. Multivariable-adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) were calculated using Cox regression.

We documented 4904 deaths among 40,074 participants after a median follow-up of 7.8years. Greater adherence to PDI was associated with lower risk of overall (HR comparing extreme quintiles 0.80, 95% CI 0.73, 0.89, p

The increase in PM2.5 above the background level around a mine represents a potential disturbance to caribou. To quantify the spatial distribution of the elevated PM2.5, we investigated three different sampling schemes to measure PM2.5 concentration using a portable monitor. We found that the best sampling scheme was to use the regional background PM2.5 as the reference and analyze the anomaly of PM2.5 measured at sites around the mine complex from the background level. The regional background PM2.5 values were measured at the Daring Lake Tundra Research Station during 2018 and 2019. Our results indicated that the background PM2.5 was not a low and constant value but varied with rain events, wind direction, and the impacts of forest W-SE axis. A similar result was found in the downwind direction when the wind came from north of the NW-SE axis, with Zepm = 4.4 km. In the upwind direction, the value of Zepm was much smaller, being 0.7 km (or 1.0 km) when the wind came from the north (or south) of the NW-SE axis. For the period of tar between 96 and 192 hours, Zepm further expanded to 21.2 km when the wind from the south of the NW-SE axis. The results from this study indicated that this reference paradigm that uses the regional background PM2.5 as the reference in combination with a portable PM2.5 monitor worked well for quantifying the tempo-spatial patterns of PM2.5 at locations in remote and mostly pristine Arctic. To establish the CQS inter-rater reliability and rating time and to compare both against that of the Jadad scale and Cochrane's Risk of Bias Tool (ROBT).

Four independent raters rated 45 trial reports. The inter-rater reliability was established by use of the Brennan-Prediger coefficient (BPC). The coefficients were compared using the two-sample z-test. Secondary analysis included comparison of the inter-rater reliability of the randomization component of all tools, as well as of the allocation concealment component of the CQS to that of the ROBT. The mean rating time with standard deviation (SD) for each tool was determined using one-way repeated measures analysis of variance. Post hoc comparisons were made using the Tukey-Kramer adjustment for three pair-wise multiple comparisons.

The inter-rater reliability was significantly higher for the CQS (BPC, 95% CI 0.95, 0.87-1.00) compared to Jadad (0.70, 0.58-0.82) (adjusted p = 0.0005) and most components of ROBT. The mean (SD) time to complete the CQS (4.0 (1.0) min) did not differ significantly from that of the Jadad scale 4.8 (1.1) min (adjusted p = 0.11), but was significantly shorter compared to that of the ROBT 15.3 (5.9) min (adjusted p < 0.0001).

The results suggest the CQS to be a very reliable and fast trial appraisal tool. Clinical relevance The higher the inter-rater reliability, the higher the probability that trial results reflect therapeutic truth. The CQS will need to take further bias sources into consideration, in order to increase its utility.
