A combination of the taxane anticancer drug paclitaxel (PTX) and molecular target drug cetuximab (cMab) is effective for the treatment of head and neck squamous cell carcinoma (HNSCC). However, its use is associated with serious side effects, such as neuropathy and myelosuppression. In addition, it is administered regardless of patient sensitivity because biomarkers indicating its efficacy are unavailable. Therefore, we investigated the usefulness of setting the indicated contact concentration of PTX and predicted the antitumor effect of combined contact with cMab using the collagen gel droplet-embedded culture drug sensitivity test (CD-DST).

Twelve human oral squamous cell carcinoma (OSCC) cell lines (i.e., SAS, HSC-2, HSC-3, HSC-4, OSC-19, OSC-20, KON, HO-1-N-1, HO-1-u-1, SAT, SCC-4, and Nialym) were used. Using the CD-DST, we calculated the optimal contact concentration of the cells with PTX based on the clinical response rate of HNSCC and evaluated the combined contact with cMab. Hepatitis B virus-associated glomerulonephritis (HBV-GN) can occur in patients with negative HBV serological antigens. Little is known about the treatment of seronegative HBV-GN (sn HBV-GN). The aim of this prospective study was to evaluate the efficacy and safety of corticosteroids in the treatment of sn HBV-GN.

Twenty-six patients with nephrotic syndrome induced by seronegative HBV-associated membranous nephropathy were enrolled. The patients were given methylprednisolone (0.8 mg/kg/day) for 12-24 weeks, tapered by a 2-mg reduction every 1-3 months. Patients were followed up for 6-36 months. General practitioners (GPs) encounter daily challenges to make cost-effective differential diagnostic and therapeutic decisions, avoiding needless and costly investigation or referral. They should provide long-term effective control of symptoms and esophageal healing in a personalized, symptom based, patient-centered and evidence-based manner. GPs should use a practical system of triage, in order to distinguish the high majority of patients with self-limiting conditions, from the minority with alarm symptoms with potentially severe disorder. They should also discriminate between troublesome and non-troublesome reflux symptoms. Most GERD is uncomplicated and can be treated using management algorithms that make the best use of resources. Some strategies such as "step-down", "intermittent" or "on-demand" therapy can cost-effectively improve the long-term management and quality of life of patients with recurrent GERD. Herein we performed the first-principles calculations with the projector augmented-wave (PAW) method to study the formation energies, elastic constants, electronic structures, and electronic transport properties of Al2Fe3Si3. We discussed the thermodynamical stability of Al2Fe3Si3against other ternary crystalline compounds in Al-Fe-Si phase. The band gap of Al2Fe3Si3was particularly examined using the semilocal and hybrid functionals and the on-site Hubbard correction, which were also applied to β-FeSi2to calibrate the prediction reliability of our employed computational methods. Our calculations show that Al2Fe3Si3is a narrow-gap semiconductor. The semilocal functional within generalized gradient approximation (GGA) shows an exceptional agreement between the predicted band gap of Al2Fe3Si3and the available experiment data, which is in contrast to the typical trend and rationally understood through a comprehensive comparison. We found that both HSE06 and PBE0 hybrid functionals with a standard setup overestimated the band gaps of Al2Fe3Si3and β-FeSi2too much. The underlying reasons may be ascribed to a large electronic screening, which arises from the unique characteristics of Fe 3dstates appearing in both sides of band gaps of Al2Fe3Si3and β-FeSi2, and to a reduced delocalization error thanks to the covalent Fe-Si and Si-Si bonding nature. The chemical bonding and elasticity of Al2Fe3Si3were compared with those of β-FeSi2and FeAl2. In Al2Fe3Si3the Fe-Al bonding is more ionic and the Fe-Si bonding is more covalent. The elastic moduli of Al2Fe3Si3are comparable to those of β-FeSi2and larger than those of FeAl2. link3 Our calculation results indicate that the mechanical strength of Al2Fe3Si3could be strong enough for the practical application in thermoelectric device.
