The pooled OR was assessed in MTHFR C677T, PT G20210A, and FVL G1691A polymorphism. This random-effects meta-analysis demonstrated that MTHFR C677T, PT G20210A, and FVL G1691A gene polymorphism did not significantly associate with β-thalassemia major. Moreover, the heterogeneity was significantly found in genotype CC vs CT+TT C677T (I2=61%) and allele C vs T (I2=71%) of MTHFR and genotype GG vs GA (I2=95%), GG vs GA+AA (I2=95%), GA vs GG+AA (I2=95%), and allele G vs A (I2=93%) of FVL G1691A. Conclusion The results of this meta-analysis show that MTHFR C677T, prothrombin G20210A, and Factor V Leiden (G1691A) gene polymorphism are not a risk factor for β-thalassemia major.Pseudocholinesterase (butyrylcholinesterase) deficiency is an inherited or acquired condition in which the serum pseudocholinesterase levels are absent or lower than normal. The enzyme is produced by the liver; decreased levels of the enzyme in an individual cause increased sensitivity to anesthetic agents, like succinylcholine and mivacurium. Pseudocholinesterase deficiency is caused by butyrylcholinesterase (BCHE) gene mutation, a gene that provides instructions for making the pseudocholinesterase enzyme. Succinylcholine is a depolarizing muscle relaxant that provides a quicker onset and a brief duration of muscle relaxation during general anesthesia. In this article, we would like to discuss a case report of prolonged intubation and ventilation in a patient with pseudocholinesterase deficiency and the necessity of succinylcholine during intubation in comparison to possible alternatives (rocuronium).

Between the end of February and the beginning of June 2020, Italy was certainly one of the worst affected countries in the world by the coronavirus disease 2019 (COVID-19) pandemic. During this period, Webinterest in the novel coronavirus underwent a drastic surge.

The aim of this study was to quantitatively analyze the impact of COVID-19 on Web searches related to hygiene-preventive measures and emotional-psychological aspects as well as to estimate the effectiveness and limits of online information during an epidemic. We looked for significant correlations between COVID-19 relative search volumes and cases per region to understand the interest of the average Italian Web user during international, national, and regional COVID-19 situations. By doing so, it will be possible to deduce the mental and physical health of the population.

We used the Google Trends tool, which returns normalized values called relative search volumes (RSV), ranging from 0 to 100 according to the Web popularity of a group of qfore, to identify mental and physical health problems among the population, it suffices to observe slight variations in the trend of related Web queries.

Metaplastic breast cancer (MBC) is a rare, aggressive variant of breast cancer, usually triple negative disease and chemotherapy refractory. Despite this, the standard of care remains the same as invasive ductal breast cancer. We sought to analyze patterns of care and outcomes among patients with metastatic MBC.

Patients over 18 years diagnosed with metastatic MBC from 2004-2015 were identified in the National Cancer Database (NCDB). Clinical and demographic details were compared between two groups (chemotherapy vs no chemotherapy). Logistic regression was performed to assess for predictors of receiving chemotherapy. The Kaplan-Meier method was used to assess overall survival (OS) and Cox regression analysis was used to assess the impact of covariates on OS.

There were 7,580 patients with MBC of which 417 (5.5%) presented with metastatic disease. Median age was 65 years (interquartile range (IQR) 54-76) and median follow up for living patients was 48 months (IQR 31-77). One hundred and fifty-six (37.4%)nce (HR 0.34-0.47, p less then 0.05) were associated with improved survival. Patients who received chemotherapy had improved median (twelve versus eight months), one-year (51% versus 38%), and two-year (35% versus 21%) OS, as compared to those who did not receive chemotherapy (p=0.006). Conclusions In this study of MBC patients, there was a survival benefit with palliative chemotherapy in the setting of metastatic disease. As expected, treatment was most often given to younger patients.Over the past several months we have learned about how pre-existent comorbidities may influence the outcome of patients infected with the coronavirus disease 2019 (COVID-19), with diabetes mellitus and obesity being commonly reported in association with increased mortality among these patients. In this report, the authors explore the case of a patient with Fragile-X Syndrome (FXS) who developed pneumonia due to COVID-19. FXS is an inherited cause of intellectual disability with potential neurologic and physiologic sequelae. In this narrative, we aim to describe how FXS may potentially play a role in the clinical course of patients with COVID-19.This case report accompanies a video of the treatment of a Tuberculum sellae meningioma (TSM) in a woman that presented with left eye near-blindness. The senior author conducted the operation via a pterional retractorless approach, and the patient had a full recovery. This case report discusses the unique anatomy of conchal sphenoid sinus, ossified TSM with very calcified consistency, and retractorless brain microsurgery. The resection of ossified TSM is still safe and viable with adequate microsurgical techniques and skull base instruments without compromising the neurovascular structure and with good neurological and visual outcomes for the patient.Shingles is a very well-known viral infection caused by the varicella-zoster virus leading to painful skin rash. Although shingles can occur anywhere, it most often appears as a single stripe of blisters around the hypochondriac region. We are presenting a rare atypical presentation of shingles, as our patient presented with a picture of the acute abdomen a couple of days prior to the eruption of skin lesions.Adult-onset Still's disease (AOSD) is a rare inflammatory disorder involving multiple systems. It can present a wide range of symptoms like maculopapular rash, fever, and arthralgia, which may overlap with many other disorders, making it difficult to diagnose. selleck chemical Unknown etiology and no diagnostic tests further make it complex to establish the diagnosis of AOSD. We report the case of a 30-year-old female who presented with persistent rash, joint pain, and fever, along with positive antinuclear antibodies (ANA), diagnosed with this condition. The patient improved with corticosteroids and the plan is to start disease-modifying antirheumatic drugs (DMARDs) after tapering off steroids.
