As a preliminary step of this examination, sleep stages are systematically determined. In practice, sleep stage classification relies on the visual inspection of 30-second epochs of polysomnography signals. Numerous automatic approaches have been developed to replace this tedious and expensive task. Although these methods demonstrated better performance than human sleep experts on specific datasets, they remain largely unused in sleep clinics. The main reason is that each sleep clinic uses a specific PSG montage that most automatic approaches cannot handle out-of-the-box. Moreover, even when the PSG montage is compatible, publications have shown that automatic approaches perform poorly on unseen data with different demographics. To address these issues, we introduce RobustSleepNet, a deep learning model for automatic sleep stage classification able to handle arbitrary PSG montages. We trained and evaluated this model in a leave-one-out-dataset fashion on a large corpus of 8 heterogeneous sleep staging datasets to make it robust to demographic changes. When evaluated on an unseen dataset, RobustSleepNet reaches 97% of the F1 of a model explicitly trained on this dataset. Hence, RobustSleepNet unlocks the possibility to perform high-quality out-of-the-box automatic sleep staging with any clinical setup. We further show that finetuning RobustSleepNet, using a part of the unseen dataset, increases the F1 by 2% when compared to a model trained specifically for this dataset. Therefore, finetuning might be used to reach a state-of-the-art level of performance on a specific population.Images from social media can reflect diverse viewpoints, heated arguments, and expressions of creativity, adding new complexity to retrieval tasks. Researchers working on Content-Based Image Retrieval (CBIR) have traditionally tuned their algorithms to match filtered results with user search intent. However, we are now bombarded with composite images of unknown origin, authenticity, and even meaning. With such uncertainty, users may not have an initial idea of what the search query results should look like. For instance, hidden people, spliced objects, and subtly altered scenes can be difficult for a user to detect initially in a meme image, but may contribute significantly to its composition. It is pertinent to design systems that retrieve images with these nuanced relationships in addition to providing more traditional results, such as duplicates and near-duplicates - and to do so with enough efficiency at large scale. mTOR activation We propose a new approach for spatial verification that aims at modeling object-level regions using image keypoints retrieved from an image index, which is then used to accurately weight small contributing objects within the results, without the need for costly object detection steps. The limitations of spatial resolution due to bandwidth are of great concern in ultrasound medical imaging. The method of acoustic impedance matching between the piezoelectric element and medium is commonly used to obtain broad bandwidth and high resolution. In this study, an optimized backing layer design was proposed to broaden the bandwidth by adding a tunable acoustic impedance matching layer of backing (AIMLB) between the backing layer and the piezoelectric ceramic element. The Mason equivalent circuit method was used to analyze the effect of the backing material composition and its structure on the bandwidth of the transducer. The optimized transducer was simulated using the finite-element method with the PZFlex software. Based on the PZFlex simulations, a 20-MHz ultrasonic transducer using the AIMLB with a bandwidth of approximately 92.29% was fabricated. The experimental results were in good agreement with the simulations. The ultrasonic imaging indicated that the designed ultrasonic transducer with an additional AIMLB had high performance with good imaging capability.
Psychoactive substance use may be risky for adults with functional impairments. This study investigates cannabis use and prescription opioid and tranquilizer/sedative (mis)use among adults aged ≥50 years reporting functional impairments in the U.S.
This cross-sectional analysis of adults aged ≥50 years from the 2015-2019 cohorts of the National Survey on Drug Use and Health estimates the prevalence of past-year medical and nonmedical cannabis use and prescription opioid and tranquilizer/sedative use and misuse according to the number of functional impairments reported. The adjusted odds of medical and nonmedical use or misuse of each substance in relation to any impairment, the number of impairments, and specific impairments were estimated using logistic regression. Analyses were conducted in December 2020.
Compared with those reporting no impairments, those reporting any impairment were more likely to report the use of cannabis and the (mis)use of prescription opioids and tranquilizers/sedatives (all p<0.05). Prevalence of (mis)use increased for each drug as the number of impairments increased (all p<0.001). Having any impairment was associated with increased odds for medical cannabis use (AOR=2.28, 95% CI=1.57, 3.30) but not for nonmedical use and with increased odds for misuse of prescription opioids (AOR=1.62, 95% CI=1.38, 1.91) and tranquilizers/sedatives (AOR=1.59, 95% CI=1.20, 2.11). Impaired thinking was associated with increased odds for the use and misuse of each substance, and impaired ability to do errands was associated with increased odds for prescription opioid misuse (AOR=1.34, 95% CI=1.01, 1.78).

Prescription drug misuse is linked to functional impairments among adults aged ≥50 years and may pose a potential risk for this vulnerable population.A subset of plant NLR immune receptors carry unconventional integrated domains in addition to their canonical domain architecture. One example is rice Pik-1 that comprises an integrated heavy metal-associated (HMA) domain. Here, we reconstructed the evolutionary history of Pik-1 and its NLR partner, Pik-2, and tested hypotheses about adaptive evolution of the HMA domain. Phylogenetic analyses revealed that the HMA domain integrated into Pik-1 before Oryzinae speciation over 15 million years ago and has been under diversifying selection. Ancestral sequence reconstruction coupled with functional studies showed that two Pik-1 allelic variants independently evolved from a weakly binding ancestral state to high-affinity binding of the blast fungus effector AVR-PikD. We conclude that for most of its evolutionary history the Pik-1 HMA domain did not sense AVR-PikD, and that different Pik-1 receptors have recently evolved through distinct biochemical paths to produce similar phenotypic outcomes. These findings highlight the dynamic nature of the evolutionary mechanisms underpinning NLR adaptation to plant pathogens.High-risk skin cancers can be difficult to treat for the dermatologist and Mohs surgeon. Residents gain invaluable experience in managing patients with difficult or advanced skin cancers through participation in a tumor board.Antimelanoma differentiation-associated gene 5 dermatomyositis (anti-MDA5 DM) is an amyopathic subtype of DM that presents with the classic cutaneous findings of DM, such as a heliotrope rash, Gottron papules, and the shawl sign, combined with mucocutaneous ulcerations. This subtype of DM also is highly associated with rapidly progressive interstitial lung disease (ILD). We report 2 cases of a heretofore novel presentation of the overlap of these cutaneous features in the form of an ulcerative heliotrope rash; furthermore, the rash was associated with progression of ILD in both cases. Finally, we review the current state of clinical care and research related to anti-MDA5 DM in terms of clinical presentations, diagnostic methods, and treatment options.A gene expression signature has been validated as an adjunct to traditional methods of differentiating malignant and benign melanocytic neoplasms, and its use in clinical practice warrants further study. This study followed patients whose melanocytic neoplasms were managed according to a benign result from the gene expression signature (N=25). Eligible patients whose tested lesions were classified as benign by the gene expression signature and were subsequently treated as benign by their dermatology providers were observed for a mean follow-up period of 38.5 months. Results suggest that many patients with melanocytic neoplasms classified as benign by the gene expression signature may safely forego additional surgical excision.Skin cancer rates have been steadily increasing over the last 20 years despite persistent efforts to educate the public on skin cancer prevention and sun safety. Adolescents and young adults are an especially important demographic to reach, as increased UV exposure during these years leads to greatly increased risks of developing skin cancer. Our survey aims to investigate the attitudes and behaviors regarding sun protection and tanning among adolescents and young adults (age range, 13-27 years).Desmoplastic melanoma (DMM) is a rare variant of melanoma that presents major diagnostic challenges. It can mimic both benign or other malignant tumors and may be easily confused by clinicians and pathologists. We report a patient who presented with a pink-colored papule on the left lateral neck. Histopathology revealed a nodular spindle cell tumor in the dermis resembling a neurofibroma at low power. However, higher-power evaluation and the use of immunohistochemical staining confirmed the diagnosis of DMM.We present this case to highlight the ongoing challenges of diagnosing DMM both clinically and histologically and to review the salient features of this often benign-appearing tumor. Advancing age, male gender, and head and neck location are associated with an increased risk of DMM-specific death. It is important that new or changing lesions in the correct cohort and location are biopsied promptly.
