Immunotherapy shows prospects in treating advanced medullary thyroid carcinoma although controversial reports are present. Recently, histological grading has been applied to medullary thyroid carcinoma by the Ki-67 index, mitotic figures, and tumor necrosis. However, the interrelation of PD-L1 expression, the Ki-67 index, and major genetic alterations of sporadic medullary thyroid carcinoma has not been fully reported. We examined the expression of PD-L1 (SP142 and 22C3) and the Ki-67 index immunohistologically and detected the major genetic alterations by next-generation sequencing in a cohort of sporadic medullary thyroid carcinomas, studied their survival impact, and discussed their interrelation. We identified that a high Ki-67 index (> 2%) and positive RET M918T mutation were correlated with poor disease-free survival but were not correlated with PD-L1 expression. All PD-L1 positive tumors were RET M918T mutation negative, and PD-L1 expression was positively correlated with HRAS mutation. The Ki-67 index was correlated with neither PD-L1 expression nor major genetic alterations. Our results indicate that immunotherapy targeting PD-L1/PD-1 might be more effective for patients with sporadic medullary thyroid carcinoma harboring HRAS mutations.Tree mycorrhizal type plays an important role in promoting plant species diversity and coexistence, via its mediating role in conspecific negative density dependence (CNDD), i.e., the process by which an individual's performance is impaired by the density of conspecific plants. Previous findings suggest that ectomycorrhizal (EM) tree species are generally less susceptible to CNDD than arbuscular mycorrhizal (AM) tree species, due to the chemical and physical protection that EM fungi provide their host with. We examined how CNDD effects on leaf herbivory, seedling growth, and survival differ between AM and EM seedlings of ten tree species collected over 3 years in an old-growth temperate forest in northeastern China. We found that AM and EM seedlings differed in how conspecific density affected their leaf herbivory, seedling growth, and survival. Specifically, AM seedlings leaf herbivory rates significantly increased with increasing conspecific seedling and adult density, and their growth and survival rates decreased with increasing conspecific adult density, these patterns were, however, absent in EM seedlings. Our work suggests that AM seedlings have a performance disadvantage relative to EM seedlings related to the negative effects from conspecific neighbors. We highlight the importance of integrating information on seedling leaf herbivory, seedling growth, to provide further understanding on potential mechanisms driving differences in CNDD between AM and EM tree seedlings.Evolutionary game theory is a powerful mathematical framework to study how phenotypes evolve by natural selection. To examine the sources and temporal variation of PBDEs in Chaohu Lake in eastern China, the surface sediments from Nanfei River (NFR) and core sediments from four estuaries were measured. It showed that low-brominated congeners were dominant, from MonoBDEs to HeptaBDEs (referred to as Σ39PBDE). Concentrations of ∑39PBDE and the ratios of (BDE-47 + BDE-99 + BDE-100)/(BDE-153 + BDE-154) were much greater in surface sediments than in core sediments. The highest concentration was observed in a site close to the outfall of a municipal sewage treatment plant (MSTP), and the ratio was significantly correlated with ∑39PBDE. These results suggested that PentaBDE and OctaBDE commercial mixtures were widely used around Chaohu Lake and the effluent of municipal sewage was a dominant source of PBDEs to surface sediment. Compared to data from other freshwater systems around the world, the concentrations of BDE-47 and BDE-99 in this study were in the middle of the range of global data, but BDE-183 concentrations were at the high end of the range. Due to restrictions on the usage of PentanBDE and OctaBDE commercial mixtures, reductions of PBDE levels from subsurface to superficial layer were observed in all estuaries. Elevated contribution by MonoBDEs to ∑39PBDE in the estuary of the only outflow river suggests significant congener fractionation. TriBDEs, TetraBDEs, and HexaBDEs appeared to pose low risks in all surface sediments, but moderate to high risks may be expected for PentaBDEs. UV-Vis spectrometry is widely used in place of traditional analytical methods because it is cost effective and fast and there is no chemical waste. In recent years, artificial neural networks have been extensively studied and used in various areas. In this study, we plan to simplify water quality monitoring with UV-Vis spectrometry and artificial neural networks. Samples were collected and immediately taken back to a laboratory for analysis. The absorption spectra of the water sample were acquired within a wavelength range from 200 to 800 nm. Convolutional neural network (CNN) and partial least squares (PLS) methods are used to calculate water parameters and obtain accurate results. The experimental results of this study show that both PLS and CNN methods may obtain an accurate result linear correlation coefficient (R2) between predicted value and true values of TOC concentrations is 0.927 with PLS model and 0.953 with CNN model, R2 between predicted value and true values of TSS concentrations is 0.827 with PLS model and 0.915 with CNN model. CNN method may obtain a better linear correlation coefficient (R2) even with small number of samples and can be used for online water quality monitoring combined with UV-Vis spectrometry in agricultural catchment.

At present, the choice of treatment modalities for ruptured hepatocellular carcinoma patients in BCLC stage A remains controversial, and this study compared the overall survival of ruptured HCC patients undergoing TACE or hepatectomy.

A total of 283 ruptured HCC patients treated at our liver surgery center were included in our study, of which 175 were treated with hepatectomy and 108 were treated with TACE. To reduce selection bias, we used a propensity score matching (PSM) model, which yielded a total of 88 pairs of patients. We used the Kaplan-Meier method to compare the long-term prognosis, and the Landmark method was used to compare the short-term and long-term prognoses of patients after PSM. Finally, we performed subgroup analysis according to whether it met the Milan criteria.

After PSM, in the hepatectomy group, the 1-, 3-, and 5year OS rates were 73.4%, 45.4%, and 33.9%, respectively. In the TACE group, the 1-, 3-, and 5year OS rates were 58.5%, 40.6%, and 23.2%, respectively. Within one year, nosis than those treated by TACE, but there was no difference in the long-term prognosis between the two treatment groups.

Many insurance companies mandate medically supervised weight loss programs (MSWLPs) prior to bariatric surgery. This retrospective study aims to elucidate whether the average 6-month preoperative medical-management period decreases preoperative BMI for those with BMI ≥ 50.

All adult patients with bariatric consultation at any time at the New York University Langone Health campuses during the period 2015 to 2021 were evaluated via electronic medical records. Only patients with ≥ BMI 50, without previous bariatric surgeries, and those with 6-month insurance-mandated medical visits were included. A paired t-test was performed on the difference in BMI and percent-weight loss among the subjects at least 6months before surgery and on the day of surgery.

Of the 130 patients with BMI ≥ 50, undergoing preoperative 6-month office weigh-ins, the mean difference in BMI was -1.51 (P < 0.01). The mean total body weight loss was 4.8% (P < 0.01). There were no intraoperative complications nor 30-day complications or mortality in the group.

Complete closure of gastric wall defects is critical following endoscopic full-thickness resection (EFTR). The aim of this study was to evaluate the efficacy, safety, and clinical outcome of a new endoscopic method closing while dissecting.

Twenty-seven patients who underwent EFTR were retrospectively analyzed between January 2020 and March 2021. Gastric defects resulting from EFTR were closed using the "interrupted-close during dissection" technique with endoclips. Tumor characteristics, en bloc resection rates, and postoperative adverse events were evaluated.

All submucosal tumors were successfully resected and complete resection was histologically confirmed. The mean maximal tumor diameter was 1.3cm (ranging from 0.8 to 3.0cm). The majority of these tumors were gastrointestinal stromal tumors (n = 20), Leiomyoma (n = 3), schwannomas (n = 2), others included fibroma (n = 1) and neurogenic tumor (n = 1). There were no cases of hemorrhage, peritonitis, or delayed perforation. Four patients complained of slight abdominal pain, but no hyperpyrexia or serious elevated white blood cell count was detected in the first 24h after treatment.
