In 2015 a new regulation and guidelines for the universal newborn hearing screening by AABR measurement have been implemented in Hungary. The aim of our study was to analyse (1) the past 5years of data from our diagnostic centre about the incidence and types of congenital hearing losses, and (2) the first experiences with the National Newborn Hearing Screening Registry, started in 2019, and (3) the influence of the screening on the pediatric cochlear implant program.

1269 children referred to our diagnostic centre between 2017 and 2021 were investigated. A third AABR measurement and full audiological evaluation were performed. Furthermore, one-year period data of the screening registry, and the number of implanted children at or under the age of 3 were analysed using the national databases.

Altogether 276 newborns (22% of the referred cases after the two-stage screening) had hearing loss, 134 (49%) out of them was conductive origin, almost twice frequent in male as in female. Permanent sensorineural hearing impairment was found in 142 (51%), 58 (40%) of them had bilateral, severe to profound hearing loss, occurring more frequently in male as in female. The national digital registration of the screening data within 12months concerned 68%. The number of early cochlear implantation in one year increased from 1 to 23 children in the past 15years.

A third AABR after the two-stage screening increased the efficiency and filtered the 78% false-positive cases. The audiological diagnostics verified and typed the hearing losses ensuring the early intervention.

To characterize the clinical features and outcomes of pediatric patients with retropharyngeal (RPA) or parapharyngeal abscesses (PPA) managed only with medical treatment and showing the importance of early symptoms and imaging studies in the diagnosis of deep neck space infections (DNIs) in children.

A retrospective analysis of all patients diagnosed with RPA and PPA between 2007 and 2017 was performed in Hospital Universitario Central de Asturias.

30 children were identified, with 11 RPA and 19 PPA. 23 children (76.7%) were under 5years old, and all were treated with intravenous amoxicillin/clavulanic acid and corticosteroids. Torticollis and fever were present in all patients. The mean length of hospital stay was 7.5days. There were no complications associated.

DNIs can be treated in a conservative way, reserving the surgical drainage for cases with a complication associated (airway compromise, lack of response to antibiotic therapy, immunocompromised patients). Treatment with intravenous antibiotics and corticosteroids is a safe option, reducing the duration of symptoms and the length of hospital stay.

Thyroid nodules are common. Ultrasonography (US) is the first investigation for thyroid nodules. Artificial Intelligence (AI) is widely integrated into medical diagnosis to provide additional information. The primary objective of this study was to accumulate the pooled sensitivity and specificity between all available AI and radiologists using thyroid US imaging. The secondary objective was to compare AI's diagnostic performance to that of radiologists.

A systematic review meta-analysis. PubMed, Scopus, Web of Science, and Cochrane Library data were searched for studies from inception until June 11, 2020.

Twenty five studies were included in this meta-analysis. The pooled sensitivity and specificity of AI were 0.86 (95% CI 0.81-0.91) and 0.78 (95% CI 0.73-0.83), respectively. The pooled sensitivity and specificity of radiologists were 0.85 (95% CI 0.80-0.89) and 0.82 (95% CI 0.77-0.86), respectively. The accuracy of AI and radiologists is equivalent in terms of AUC [AI 0.89 (95% CI 0.86-0.92), radiologist 0.91 (95% CI 0.88-0.93)]. The diagnostic odd ratio (DOR) between AI 23.10 (95% CI 14.20-37.58) and radiologists 27.12 (95% CI 17.45-42.16) had no statistically significant difference (P = 0.56). Meta-regression analysis revealed that Deep Learning AI had significantly greater sensitivity and specificity than classic machine learning AI (P < 0.001).

AI demonstrated comparable performance to radiologists in diagnosing benign and malignant thyroid nodules using ultrasonography. Additional research to establish its equivalency should be conducted.

Our study aimed to describe recovery of gustatory dysfunction (GD) and olfactory dysfunction (OD) in COVID-19 patients, and to analyze variables associated with early or late recovery.

Telephone surveys were administered during an 18-month follow-up after COVID-19 diagnosis. Selleckchem TPCA-1 One hundred and thirty-two included patients rated olfactory and gustatory function at each follow-up.

One hundred and twenty-nine patients reported GD, of whom 91 (70.5%) reported severe GD, and 99 patients reported OD, of whom 84 (84.9%) reported severe OD. Seventy-two/129 (55.8%) and 52/99 (52.5%) patients reported an improvement in GD and in OD during the first 7days from the onset, respectively. At 3-month follow-up, 110/120 patients (85.3%) recovered from GD, while 80/99 patients (80.8%) recovered from OD. At 18-month follow-up, a total of 120/129 patients (93.0%) recovered from GD and 86/99 patients (86.9%) recovered from OD; while 10 patients (7.0%) still reported GD and 13 patients (13.1%) still reported OD. Severe GD/OD atatively correlated to recovery, and improvement of taste and/or smell function within the first 7 days from symptom onset was significantly associated with early resolution.

Glottal insufficiency, mostly caused by unilateral vocal fold paralysis (UVFP) or vocal fold atrophy can be treated by injection laryngoplasty (IL). Materials such as hyaluronic acid (HA) gels are now widely available and used to improve voice quality. Various durability and effects of HA injection laryngoplasties have been reported. The aim of this study is to provide a systematic review of the literature for the use of HA in injection laryngoplasties.

A systematic literature search was conducted in PubMed and Cochrane Libraries. Three reviewers assessed original research studies concerning vocal fold augmentation with HA for eligibility. English written full-text human studies from 2000 to 2020 with EBM level 1b-2 were included. In vitro studies, animal studies, case reports, case-control studies, correspondence and review articles, and articles with other injection materials were excluded. Demographical data, indication, type of HA, evaluation methods, follow-up, durability of implanted material and coe quality, improves results of voice therapy, and can be recommended as overlap treatment before permanent medialization surgery. Compared to other injected materials, HA seems to have more favorable effect on vibratory functions of the vocal folds and thus on the voice quality parameters.

Report our experience in the management of posterior petrous surface meningiomas (PPSMs), and identify features that affect hearing, facial nerve (FN) function, and control of the disease.

Retrospective case series of 131 patients surgically managed for PPSMs. FN status, hearing and tumour radicality were assessed and compared between patients with tumours of different locations (Desgeorges classification) and internal auditory canal involvement (IAC).

At the time of surgery 74.8% of patients had a hearing loss. Hearing was mostly unserviceable in tumors attached to the meatus. Pure tone audiometry did not correlate to IAC extension, while speech discrimination scores were statistically worse when the tumor occupied the IAC (unpaired t test, p = 0.0152). Similarly, extrameatal tumors undergoing removal by otic preserving techniques maintained postoperative hearing, whereas hearing worsened significantly in tumors involving the IAC (paired t test, p = 0.048). The FN was affected preoperatively in 11.4% of cases. Postoperative FN palsy was significantly correlated to the IAC involvement (Fisher's exact test, p = 0.0013), while it was not correlated to tumor size. According to the Desgeorges classification, a postoperative FN palsy complicated the majority of anteriorly extending tumors and, two-fifths of meatus centred tumors. 75% of posterior located tumors had a postoperative FN grade I HB.

Since the involvement of the IAC by the tumor affects both hearing and FN function, the IAC is of primary importance in PPSMs and should be studied and addressed as much as the tumor location in the CPA.

Spore clusters (sporocarp-like structures) often formed in pot cultures or in vitro conditions are supposed to be clonal populations, while sporocarps in natural habitats with a fungal peridium are morphologically similar to those of epigeous sexual (zygosporic) sporocarps of Endogone species. Thus, in this study, we explored the genetics of sporocarpic spores of two AM fungi with a view to possibilities of clonal or sexual reproduction during sporocarps formation. To examine these possibilities, we investigated single-nucleotide polymorphisms (SNPs) in reduced genomic libraries of spores isolated from sporocarps molecularly identified as Rhizophagus irregularis and Diversispora epigaea. In addition, partial sequences of the MAT locus HD2 gene of R. irregularis were phylogenetically analyzed to determine the nuclear status of the spores. We found that most SNPs were shared among the spores isolated from each sporocarp in both species. Furthermore, all HD2 sequences from spores isolated from three R. irregularis sporocarps were identical. These results indicate that those sporocarps comprise clonal spores. Therefore, sporocarps with clonal spores may have different functions than sexual reproduction, such as massive spore production or spore dispersal via mycophagy.Climatic warming is forcing species to shift their ranges poleward, which has been demonstrated for many taxa globally. Yet, the influence of habitat types on within- and among-species variations of distribution shifts has rarely been studied, especially during the non-breeding season. Here, we investigated habitat-specific shift distances of northern range margins and directions of the distribution center based on long-term data of overwintering birds in Finland. Specifically, we explored influences of habitat type, species' snow depth tolerance, species' climatic niche and habitat specialization on range shifts during the past 40 years in 81 bird species. Birds overwintering in arable land shifted more clearly toward north compared to birds of the same species in rural and forest habitats, while the northern range margin shift distances did not significantly differ among the habitat types. Range shifts were more linked with the species' snow depth tolerance rather than species' climatic niche. Snow depth tolerance of species was negatively associated with the eastward shift direction across all habitats, while we found habitat-specific patterns with snow depth for northward shift directions and northern margin shift distances.
