Patients with Barcelona Clinic Liver Cancer Stage C (BCLC-C) hepatocellular carcinoma (HCC) can be markedly heterogeneous with varying prognosis. This study aims to establish a new subclassification system for BCLC-C HCC to better predict overall survival (OS) and to tailor therapy.

We retrospectively studied 1856 BCLC-C HCC patients between 2006 and 2017 from E-Da Hospital, Taiwan (n = 622, training cohort), Kaohsiung Medical University Hospital, Taiwan (n = 774, Taiwan validation cohort), and Stanford University Medical Center and Mayo Clinic (United States), Hanyang University Hospital (South Korea), and Ogaki Municipal Hospital (Japan) to make up the international validation cohort (n = 460).

In the training cohort, significant factors associated with OS were largest tumor size ≥ 10cm, extrahepatic spread, macrovascular invasion, and Child-Pugh class, which provided the basis, together with aged ≥ 75years, for the substaging, through C0 to C4, of BCLC-C HCC patients. The median OS for substages C0, C1, C2, C3, and C4 were 43.8months (95% confidence interval [CI] 32.2-53.7), 20.6months (CI 14.1-25.9), 11.5months (CI 8.02-14.1), 5.7months (CI 4.02-5.98), and 3.2months (CI 2.41-3.59), respectively, (p < 0.05). OS remained distinct among the proposed substages in the Taiwan validation cohort as well as the international validation cohort. The distinction between the substages persisted in subgroup analysis by substage combined with treatment modality. In substage C0-C3, patients receiving HCC curative therapy had a significantly better median OS than those receiving sorafenib or palliative therapy.

However, despite these important antitumor activities, the compound has the disadvantage to have a hydrophobic characteristic, hindering in vivo administration. To surpass this issue, in this study we have created a [10]-gingerol-loaded nanoemulsion (10GNE) in order to increase the stability and solubility of the compound. The nanoemulsion was characterized and tested for its cytotoxic, cytostatic, and apoptotic effects on a panel of murine and human TNBC cell lines, as well as non-tumor cells, and compared with a [10]-gingerol-free nanoemulsion (NE) and with [10]-gingerol itself. Except for the murine 4T1.13 cell line, the IC50 of the free 10G molecule, after 72 h of incubation, was higher in all cell lines tested, both murine and human, demonstrating therefore the efficacy of the 10GNE regarding cytotoxicity. In murine tumor cells, 60 μM 10GNE was able to arrest cell cycle at sub-G0 phase and induce apoptosis, leading to 48% and 78% of total cell death in 4T1.13 and 4T1Br4 murine tumor cells, respectively. This represents an improvement compared to 10G-free molecule that only induced 74% of total apoptosis at 100 μM in 4T1Br4 cells. Taken together, our results show that nanoformulation preserved the [10]-gingerol cytotoxic and cytostatic properties and improved its apoptotic function on murine TNBC cell lines. These data open new perspectives to a more suitable drug-delivery approach for [10]-gingerol for TNBC treatment that should be further demonstrated using in vivo assays.

To conduct a comparative analysis of the prevalence of urolithiasis in the Russian Federation.

We analysed urolithiasis prevalence and incidence data from 2005 to 2019 (15years) for the entire population of Russia. Data were provided by the 'Ministry of Health' of the Russian Federation. The prevalence and incidence of urolithiasis were collected and analysed for both adults and children for each region of the Russian Federation over this 15-year period.Statistical analysis was performed using the SPSS Statistics 21 software package (SPSS). Intergroup correlations and differences between samples in the studied parameters were considered significant atp < 0.05.

A total of 656,911 and 889,891 urolithiasis cases were observed in 2005 and 2019, respectively, an increase in urolithiasis prevalence of 35.4% for the study period, with the growth rate that was fairly uniform.The incidence of urolithiasis in the Russian Federation was 176,773 in 2005, while 205,414 new urolithiasis cases were recorded in 2019, with a clear tendency to a rising incidence of urolithiasis, an increase of 16.2% during the study period. Theincidenceper 100,000 in children remained stable during the entire period of analysis.
