The safety profile of direct oral anticoagulants is better or at least similar to warfarin, but several recent reports are focusing on spontaneous hemorrhages with direct oral anticoagulants. This narrative review aims to summarize the incidence of spontaneous hemorrhage in patients treated with direct oral anticoagulants and also offers practical management strategies for clinicians when patients receiving direct oral anticoagulants present with bleeding complications.Efficient separation technologies are crucial to the environment and world economy. Polymer brushes are a certain kind of thin polymer films, where polymer chains are covalently grafted to a substrate and straighten up to form a brush structure. Here we report on differences and similarities between polymer brushes and spin-coated polymer films from polystyrene and polymethyl methacrylate with special emphasis on surface roughness and roughness correlation. The phenomenon of roughness correlation or conformality describes the replication of the roughness profile from the substrate surface to the polymer surface. It is of high interest for polymer physics of brush layers as well as applications, in which a homogeneous polymer layer thickness is required. We demonstrate that spin-coated films as well as polymer brushes show roughness correlation, but in contrast to spin-coated films, the correlation in brushes is stable to solvent vapor annealing. These higher bleeding rates have provoked trials investigating de-escalation from ticagrelor or prasugrel to clopidogrel in the hope of reducing bleeding without increasing thrombotic event rates. In this review, we sought to present an overview of the major trials investigating several different options for de-escalation; unguided, platelet function testing- and genotype-guided. Based on these results, and on other established literature sources, such as guidelines and expert consensus papers, we provide an overview to help decide when and how to de-escalate antiplatelet therapy in ACS patients undergoing PCI.

The atherogenic index of plasma (AIP) reflects the levels of triglycerides (TG) and high-density lipoprotein (HDL) cholesterol. The purpose of this study was to assess the relationship between the AIP and severe acute pancreatitis (SAP).

Patients with acute pancreatitis (AP) were prospectively enrolled from March 2015 to June 2019. The severity of AP was classified according to the 2012 revised Atlanta classification. Mild and moderately severe AP were categorized as non-SAP. The AIP is calculated as log(TG/HDL).

A total of 323 patients were enrolled. The etiologies of AP were gallstone in 171 patients (52.9%), alcohol in 122 patients (37.8%), and hypertriglyceridemia in 30 patients (9.3%). Twenty-four patients (7.4%) were classified as SAP. The AIP was significantly higher in the SAP group compared to the non-SAP group (

< 0.001). The AIP was positively correlated with the Atlanta classification (

= 0.256,

< 0.001). In multivariate analysis, the AIP was found to be an independent predictive factor for SAP (OR = 4.571; CI = 1.913-10.922;

= 0.001).

Due to the paucity of data in the literature on periodontitis, the aim of this cross-sectional study was to assess the relationship between mineral elements in saliva and periodontal status in patients with untreated and treated periodontitis compared to periodontally healthy controls. Salivary samples from 66 nonsmoker healthy patients (20 periodontally healthy, 24 untreated severe periodontitis and 22 treated severe periodontitis) were analyzed by using inductively coupled plasma mass-spectrometry (ICP-MS). Significant increases in copper (Cu), sodium (Na), iron (Fe) and manganese (Mn) concentrations occurred in saliva of severe periodontitis subjects compared to periodontally healthy controls. No differences were detected between healthy controls and treated periodontitis patients apart from levels of zinc (Zn) and lithium (Li) that were found to be increased and reduced, respectively, in periodontitis group. Most subjects were correctly separated by cluster analysis into active periodontitis and periodontally healthy individuals. Treated periodontitis individuals were classified as healthy subjects. This paper analyzes the measures used to manage pesticides in Malawi. Malawi's regulatory authority of pesticides, the Pesticides Control Board (PCB), faces a number of challenges including lack of facilities for analyzing pesticides and inadequate personnel to conduct risk assessment of pesticides. The PCB needs to provide access to information and opportunities among the public to make contributions regarding requirements, processes and policies for assessing pesticide risk and efficacy. There is also a need to enhance the capacity of PCB to assess pesticide poisoning in workers, monitor pesticide residues in food and environmental contamination, as well as to control the illegal importation and sale of pesticides. Just like in other countries such as South Africa, India and Sri Lanka, Malawi urgently needs to implement measures that can restrict the importation, production, sale and use of very toxic pesticides. With the recent advancements in Artificial Intelligence (AI) and the Internet of things (IoT), several maintenance tasks can be automated, which significantly improves productivity and safety. This work proposes a real-time remote insect trap monitoring system and insect detection method using IoT and Deep Learning (DL) frameworks. The remote trap monitoring system framework is constructed using IoT and the Faster RCNN (Region-based Convolutional Neural Networks) Residual neural Networks 50 (ResNet50) unified object detection framework. The Faster RCNN ResNet 50 object detection framework was trained with built environment insects and farm field insect images and deployed in IoT. The proposed system was tested in real-time using four-layer IoT with built environment insects image captured through sticky trap sheets. Further, farm field insects were tested through a separate insect image database. The experimental results proved that the proposed system could automatically identify the built environment insects and farm field insects with an average of 94% accuracy.

Conventional colonoscopy (CC) allows access for colonic investigation and intervention; in the small group in whom CC is unsuccessful alternative imaging is often sufficient.
