New management, risk stratification and treatment strategies have become available over the last years for patients with acute pulmonary embolism (PE), potentially leading to changes in clinical practice and improvement of patients' outcome.

The COntemporary management of Pulmonary Embolism (COPE) is a prospective, non-interventional, multicentre study in patients with acute PE evaluated at internal medicine, cardiology and emergency departments in Italy. The aim of the COPE study is to assess contemporary management strategies in patients with acute, symptomatic, objectively confirmed PE concerning diagnosis, risk stratification, hospitalization and treatment and to assess rates and predictors of in-hospital and 30-day mortality. The composite of death (either overall or PE-related) or clinical deterioration at 30days from the diagnosis of PE, major bleeding occurring in hospital and up to 30days from the diagnosis of PE and adherence to guidelines of the European Society of Cardiology (ESC) are secondary study outcomes. Participation in controlled trials on the management of acute PE is the only exclusion criteria. Expecting a 10-15%, 3% and 0.5% incidence of death for patients with high, intermediate or low-risk PE, respectively, it is estimated that 400 patients with high, 2100 patients with intermediate and 2500 with low-risk PE should be included in the study. This will allow to have about 100 deaths in study patients and will empower assessment of independent predictors of death.

COPE will provide contemporary data on in-hospital and 30-day mortality of patients with documented PE as well as information on guidelines adherence and its impact on clinical outcomes.

Nowadays, the integration of Artificial intelligence algorithms and quantified radiographic imaging-based diagnostic procedures is hailing amplified deliberation particularly in assessment of skeletal maturity. So we intend to formulate a logistic regression model for intelligent and quantitative estimation of Fishman skeletal maturation index (SMI) based on the parameters attained from the cervical vertebrae CBCT images of Chinese girls.

From 709 hand wrist radiographs and CBCT images, 447 samples were randomly selected (called as G1) to build a logistic regression model. The reliability and reproducibility were assessed by the intraclass correlation coefficient (ICC) and weighted Cohen's kappa, followed by Spearman's rank correlation coefficient to identify the parameters significantly associated with the SMI. Two hundred and sixty-two other subjects (named G2) were recruited for external examination of the models by direct visual comparison and the receiver operating characteristic (ROC) curve. Prediction of type 2 diabetes (T2DM) remission is an important part of risk-benefit assessment before bariatric surgery.

Advanced-DiaRem (Ad-DiaRem) and ABCD diabetes remission scores for sleeve gastrectomy (SG) and Roux-en-Y gastric bypass (RYGB) were calculated using baseline data. Differences in model discrimination using area under the curve of receiver operating curve (AUC-ROC) and model calibration were tested for complete remission (HbA1c ≤ 6.0% without antidiabetic medications) in the two groups. Optimal cutoff scores were calculated using the Youden index.

We randomized 109 patients to either SG or RYGB. With one patient lost to follow-up in each group, the scores were calculated for 54 patients in the SG group and 53 patients in the RYGB group. Both models showed moderate predictive power without any significant difference between the groups AUC-ROCs (95% CI) for the Ad-DiaRem score (SG versus RYGB) were 0.872 (0.780-0.964) versus 0.843 (0.733-0.954), p = 0.69, and for the ABCD score 0.849 (0.752-0.946) versus 0.750 (0.580-0.920), p = 0.32, respectively. Using optimal cutoff points derived from the whole study population, the actual proportion of diabetes remission was significantly higher than predicted for both the Ad-DiaRem and ABCD scores in the RYGB group. Diabetes duration and glycated haemoglobin predicted diabetes remission in the entire Oseberg population.

Our aim was to summarize the evidence on the frequency of complex B vitamin deficiencies in studies comparing Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG). We included 25 studies for qualitative synthesis and 21 studies for quantitative synthesis. Relevant data was extracted, including proportion of patients with deficiency and mean serum vitamin values in 3 different timeframes. B12 and folate were the most prevalent deficiencies. B12 deficiency was more common after RYGB and folate serum mean levels were higher after RYGB. SG causes less nutrient deficiency and is therefore a better technique from this point of view. Congenital loss of function of BDNF causes weight gain in both humans and rodents; however, it is not clear whether acquired loss of function of BDNF also affects body weight. Thus, we exploited mutant mice in which the Bdnf expression level is regulated by the tetracycline-dependent transcriptional silencer (tTS)-tetracycline operator sequence (tetO) system. Time-controlled Bdnf expression using this system allowed us to establish congenital and acquired loss of function of Bdnf in mice. We demonstrated that changes in Bdnf expression influenced body weight during not only the developmental stage but also the adult stage of mice. Previous researches on METH abuse have mainly focused on neurotransmitters, such as dopamine and glutamate. However, there is growing evidence that neuroinflammation also plays an important role in the etiology and pathophysiology of brain dysfunction induced by METH abuse. This has cast a spotlight on the research of microglia and astrocyte, which are critical mediators of neuroimmune pathology in recent years. In the central nervous system (CNS) immunity, abnormalities of the microglia and astrocytes have been observed in METH abusers from both postmortem and preclinical studies. The bidirectional communication between neurons and glia is essential for the homeostasis and biological function of the CNS while activation of glia induces the release of cytokines and chemokines during pathological conditions, which will affect the neuron-glia interactions and lead to adverse behavioral consequences. However, the underlying mechanisms of interaction between neurons and glia in METH-induced neuroinflammation remain elusive. Notably, discovering and further understanding glial activity and functions, as well as the crosstalk between neurons and glia may help to explain the pathogenesis of METH abuse and behavioral changes in abusers. In this review, we will discuss the current understanding of the crosstalk between neurons and glia in METH-induced neuroinflammation. We also review the existing microglia-astrocyte interaction under METH exposure. Researching the prediction and evaluation of the remaining life of used spindles has essential engineering significance for improving the accuracy and economics of remanufacturability assessment. Aiming at the fatigue fracture, wear and excessive deformation of the wasted spindle, and considering the factors affecting crack closure and crack development, a residual life prediction evaluation model based on a nonlinear continuous fatigue damage model was proposed. Take the spindle of CAK5085 CNC lathe of a machine tool company as an example to evaluate its remaining life. The accuracy and feasibility of the modified model are verified by comparing the data of the test with the calculated results of the model. The results show that the proposed prediction model can calculate a more accurate stress-life curve, which provides a theoretical basis for the life prediction of remanufactured parts.In this paper, the atmospheric flow field characteristics in a deep open-pit mine are investigated numerically and theoretically. A theoretical model on the recirculation length based on the energy equation is used, and a total variation diminishing (TVD) difference scheme with second-order accuracy is used to solve the NS equations with a standard two-equation k-ε turbulence model. The effects of elevated inner dump on the flow field characteristics of the open-pit mine are analyzed detailedly with the same inflow parameters. The results show that a recirculation zone exists in the open-pit mine due to the reflux from the high-pressure zone to the low-pressure zone. As the height of the inner dump increases, the flow becomes more complicated, and the low-pressure zone and the recirculation zone become bigger. The elevated inner dump makes it difficult for the internal fluid to flow to the outside, which results in the increase of the dust concentration. At last, the influences of key parameters on flow filed are conducted by normalizing the depth of the lowest direct current flow on the windward slope.
