Case description 4 cats (6 to 9 months old) were evaluated because of clinical signs consistent with a portosystemic shunt (PSS). Clinical findings Among the 4 cats, 3 had neurologic abnormalities including ataxia, head pressing, disorientation, and obtundation. One cat was evaluated because of urethral obstruction; a retrieved urethral stone was determined to have urate composition. Clinicopathologic findings (hypoproteinemia, low BUN concentration, and high serum bile acids concentration) were consistent with a PSS in all cats. A diagnosis of intrahepatic PSS (IHPSS) was made for all cats on the basis of ultrasonographic and CT findings. BMS794833 Treatment and outcome All cats underwent percutaneous transvenous coil embolization (PTCE). No major intraprocedural complications were encountered, and all cats were discharged from the hospital. For the 3 cats that were presented with neurologic signs, an evaluation performed at 12, 14, or 48 months after the procedure revealed resolution of the neurologic signs, and owners reported that the behavior of each cat appeared normal. One cat that initially had neurologic and gastrointestinal signs had lower urinary tract signs after PTCE and developed an acquired extrahepatic PSS. Clinical relevance Although IHPSSs in cats are uncommon, the outcomes of PTCE for the 4 cats of the present report suggested that this treatment may benefit cats with an IHPSS. No short-term complications were encountered, and all cats had improvement in clinical signs following PTCE, although an acquired extrahepatic PSS was later identified in 1 cat. Further investigation of the use of endovascular techniques for the treatment of IHPSSs in cats and other species is warranted.Objective To determine whether serum total thyroxine (TT4) concentration at admission to an intensive care unit (ICU) was associated with mortality rate and duration of hospitalization for critically ill dogs. Animals 166 client-owned dogs that were hospitalized in the ICU of a private veterinary practice from January 2013 through December 2016 and for which serum TT4 concentration had been measured at admission. Procedures Medical records were reviewed to collect data regarding patient signalment, concurrent illnesses, medications, reason for hospitalization, outcome (death, euthanasia, or survival to hospital discharge), duration of hospitalization, and initial serum TT4 concentration. Results Mean age of the 166 dogs was 8.6 years (range, 1 to 16 years). Overall mortality rate was 15.7%, with 26 dogs failing to survive to hospital discharge. Of these 26 dogs, 7 died and 19 were euthanized. No significant association was identified between serum TT4 concentration at admission and survival to discharge (yes or no) or duration of hospitalization. Age was significantly associated with survival to discharge, with older dogs less likely to survive than younger dogs. Duration of hospitalization was also associated with survival to discharge, with longer hospital stays associated with a lower likelihood of survival to discharge. Conclusions and clinical relevance Findings suggested that serum TT4 concentration at admission to an ICU had no prognostic value in this population of critically ill dogs.Objective To evaluate the prophylactic efficacy of triclosan-coated (TC) suture in preventing surgical site infections (SSIs) in dogs and cats undergoing surgical procedures. Animals 862 animals (794 dogs and 68 cats). Procedures Incidences of SSI of surgical wounds closed with TC suture and surgical wounds closed with non-TC suture occurring between the time of surgery and 1 month postoperatively were compared. Animals were randomly assigned to TC or non-TC suture groups. When TC suture was assigned, all suture available as TC material was used rather than non-TC suture. Presence of an SSI was determined by an owner questionnaire or direct examination. Results Overall, 50 of 862 (5.8%) animals developed SSIs. Incidence of SSI was 6% (24/428) in the non-TC suture group and 6% (26/434) in the TC suture group. No significant difference was found in the incidence of SSI between groups. No significant difference was detected in the incidence of incisional problems (eg, redness, dehiscence, and seroma formation) between animals in which TC suture was used and those in which non-TC suture was used. On multivariable analysis, other factors were associated with increased SSI rates, including an incision length > 10 cm, surgery performed by the soft tissue surgery department, and anesthesia duration of > 240 minutes. Conclusions and clinical relevance No significant difference in incidence of SSI was detected between animals undergoing surgical procedures performed with non-TC versus TC suture.Objective To assess production animal medicine veterinarians' prescription practices and identify factors influencing their use of antimicrobial drugs (AMDs) and their perceptions of and attitudes toward antimicrobial resistance (AMR). Sample 157 production animal veterinarians in the United States. Procedures An online cross-sectional survey and digital diary were used to gather information regarding perceptions on AMD use and AMR and on treatment recommendations for production setting-specific disease scenarios. Results were compared across respondents grouped by their selected production setting scenarios and reported years as veterinarians. Results The most commonly selected production setting disease scenarios were dairy cattle (96/157 [61.1%]), backgrounding cattle (32/157 [20.4%]), and feedlot cattle (20/157 [12.7%]). Because few respondents selected swine (5/157 [3.2%]) or poultry (4/157 [2.5%]) scenarios, those responses were excluded from statistical analysis of AMD prescription practices. Most remaining respondents (147/148 [99.3%]) reported that they would recommend AMD treatment for an individual ill animal; however, responses differed for respondents grouped by their selected production setting scenarios and reported years as veterinarians when asked about AMD treatment of an exposed group or high-risk disease-free group. Most respondents reported that government regulations influenced their AMD prescribing, that owner and producer compliance was a veterinary-related factor that contributed to AMR, and that environmental modifications to prevent disease could be effective to mitigate AMR. Conclusions and clinical relevance Results of the present study helped fill important knowledge gaps pertaining to prescription practices and influencing factors for AMD use in production animal medicine and provided baseline information for future assessments. This information could be used to inform future interventions and training tools to mitigate the public health threat of AMR.Extracorporeal shockwave lithotripsy (ESWL) is a common and effective treatment method for most renal and upper ureteral calculi. Aortic rupture after ESWL is an extremely rare complication. Seven cases of aortic rupture have been reported to date, and only one case involved the rupture of a calcified abdominal aorta. We herein describe a Chinese patient who was hospitalized for rupture of the abdominal aorta 5 days after ESWL for right ureteral calculi. The patient was transferred to the Department of Vascular Surgery and underwent emergency endovascular aortic repair. The patient's recovery was unremarkable. One week after the operation, enhanced computed tomography showed that the size of the hematoma around the periaortic area was constant, and repeat enhanced computed tomography 1 month later showed that the hematoma had been significantly absorbed. ESWL may cause rupture of a heavily calcified abdominal aorta. We suggest that all patients with atherosclerosis being considered for ESWL should be evaluated by imaging examinations both preoperatively and during follow-up.Introduction Multikinase inhibitors have clinical activity in radioactive iodine-refractory (RAIR) differentiated thyroid cancers (DTCs) but are not curative; optimal management and salvage therapies remain unclear. This study assessed clinical effects of pazopanib therapy in RAIR-DTC patients with progressive disease, examining in parallel biomarker that might forecast/precede therapeutic response. Methods Assessment of responses and toxicities, and of any association between thyroglobulin (Tg) changes cycle 1 and RECIST (response evaluation criteria in solid tumors) response to pazopanib therapy, were prospectively undertaken in Tg antibody negative RAIR-DTC patients. RECIST progressive metastatic disease 50% after cycle 1 versus those that did not were not significantly correlated (-23.5%, 95%CI -55.3% to 8.3%; Fisher's test p-value =0.27). RECIST response was also not correlated with/predicted by early MCV change, receipt of prior therapy, or tumor mutational status. Conclusions This trial prospectively confirmed pazopanib to have clinical activity and manageable toxicities in patients with progressive RAIR-DTC. Response to pazopanib, however, was not robustly forecast by early associated changes in Tg or MCV, by prior therapy, or by tumor mutational status.This study describes asymmetry in the main running mechanical variables during repeated treadmill sprints in elite female athletes and examines whether inter-limb differences in sprinting mechanics increase with fatigue. Eighteen elite female players (French national Rugby Sevens team) performed 8 × 5-s sprints (25-s rest) on an instrumented motorised sprint treadmill. The group mean 'symmetry angle' (SA) scores were ~1-2% for contact time (1.6 ± 0.6%), aerial time (2.1 ± 0.8%), step frequency (1.3 ± 0.5%) and step length (1.6 ± 0.6%). Mean vertical and horizontal forces, vertical and leg stiffness presented SA values of 1.7 ± 1.5%, 2.4 ± 1.2%, 2.6 ± 0.2% and 2.5 ± 0.2%, respectively. The SA scores were ~2-8% for duration of braking (6.9 ± 5.0%) and propulsive (6.5 ± 4.4%) phases, peak braking (6.5 ± 2.5%) and propulsive (1.6 ± 0.9%) forces as well as net (5.8 ± 5.6%), braking (7.7 ± 5.3%) and propulsive (2.7 ± 1.6%) impulses. However, there was no influence of sprint repetition number on SA scores for tested variables (P > 0.05).
